Advances in trans-sphenoidal pituitary surgery.

Pituitary surgery is a continuous evolving speciality of the neurosurgeons' armamentarium, requiring precise anatomical knowledge, technical skills and integrated appreciation of the pituitary pathophysiology. Actually, it could be considered the result of a close cooperation between different specialists, i.e. the ophthalmologist, the neuroradiologist, the endocrinologist, the neurosurgeon, the pathologist, etc. In this teamwork environment each member plays his own role, offering his contribute to the final result; every effort is performed to provide patients with the best possible procedur…

Unexpected seizure attack in a patient with spinal metastasis diagnosed as posterior reversible encephalopathy syndrome.

Seizure is a foreseeable risk in patients with brain lesion. However, seizure during treating non-brain lesion is not a familiar situation to neurosurgeon. Posterior reversible encephalopathy syndrome (PRES) is a relatively common situation after systemic chemotherapy. The aim of this study is to make neurosurgeons aware of this potential medical problem. A 52-year-old woman with advanced gastric cancer, presented with low back pain due to spinal metastasis at the 4th lumbar vertebra. Ten cycles of chemotherapy with FOLFOX (5-Fluoruracil/Oxaliplatin) had been completed 23 days ago. Two days be…

Vestibular schwannoma management: current practice amongst UK otolaryngologists--time for a national prospective audit.

INTRODUCTION: It is generally agreed that the successful management of a vestibular schwannoma (VS) usually involves close collaboration between a neuro-otologist and neurosurgeon. In addition, it is accepted that the experience of the team managing such tumours is one of the key determinants of outcome after surgical intervention. The aim of this study was to identify current practice in the management of such tumours amongst otolaryngologists in the UK and to observe whether such collaborative working practices exist. MATERIALS AND METHODS: A cross sectional postal questionnaire survey of c

Persistence of the otic artery with neurological sequelae: case report.

Persistence of intracranial fetal vasculature may be encountered by the neurosurgeon. Of these, the otic artery is extremely rare and to some, a true case has to date, not been authenticated. We report an adult patient found to harbor an otic artery. Moreover, neurological sequelae of this fetal vascular connection are believed to have occurred. This case and a review of germane literature are presented.

Management of acoustic neuromas, 1978-1983.

The surgical aspects of 162 consecutive procedures for removal of acoustic neuromas, performed from 1978 through 1983, are reviewed. Nearly all of the procedures were done through a retrosigmoid suboccipital craniectomy. Most used the combined skills of a neurosurgeon and an otological surgeon. Total tumor removal was accomplished in 98% of cases. There have been two recurrences and one postoperative death. The facial nerve was preserved in 81% of procedures. Facial function returned in nearly all of these patients, but the degree of return was variable. The cochlear nerve was preserved in 55

Addressing connectomic constraints in early practice of awake-guided neurosurgery: a four-year prospective case-matched analysis.

PURPOSE: Awake functional-guided resection (AFGR) is the gold-standard approach in glioma surgery. The learning curve associated with mastering white matter tracts (WMT) identification using direct electrostimulation (DES) mapping remains poorly characterized. The goal of this study was to compare subcortical DES mapping performance between early-career and expert neurosurgeons. METHODS: In this prospective, monocentric, case-matched study, patients undergoing AFGR (2022-2025) were operated on either by an early-career neurosurgeon during the first four years of independent practice (cohortA)…

Reliability of the clinical examination in the diagnosis of neurogenic versus vascular claudication.

BACKGROUND CONTEXT: As research increasingly challenges the diagnostic accuracy of advanced imaging for lumbar spinal stenosis, the impression gleaned from the office evaluation becomes more important. Neurogenic claudication is a hallmark of lumbar spinal stenosis, but the reliability of clinical impression of claudication has not been studied. PURPOSE: To determine the reliability of the clinical examination for neurogenic claudication in an idealized setting. STUDY DESIGN: Prospective masked controlled trial. PATIENT SAMPLE: Persons aged 55 to 90 years were recruited to form three groups

Genetics in neuro-oncology.

This review provides a framework for the neurosurgeon in understanding the increasingly important role of genetics in the study of nervous system tumors. The three tumors discussed (retinoblastoma, meningioma, and neurofibroma) are neither the most common nor the most clinically devastating tumors faced by neurosurgeons. Rather, the studies on these tumors are presented because of the important lessons each provides. Studies of retinoblastoma demonstrate the ability of a gene which is recessive at the cellular level to require a second mutation for tumorigenesis and therein to appear as a domi…

Ventriculoperitoneal Shunts and Other Neurosurgical Considerations in Children with Medical Complexity.

As the number and complexity of hospitalized children increase, pediatric clinicians regularly address neurosurgical issues. Pediatric clinicians need to be familiar with the clinical presentation, diagnostic evaluation, and management of common pediatric neurosurgical issues, including cerebrospinal fluid (CSF) shunt issues, baclofen pumps, and epilepsy surgery. The care of these children represents the exciting intersection of hospital-based pediatric clinicians and neurosurgeons and an opportunity for collaboration in clinical care, quality improvement, and research to advance care.

Coexistence of Triphasic Diabetes Insipidus and Cerebral Salt Wasting Syndrome Following Extraction of Sellar/Suprasellar Grade I Pilocytic Astrocytoma.

Water homeostasis disorders, such as syndrome of inappropriate antidiuretic hormone secretion (SIADH), diabetes insipidus (DI), and cerebral salt-wasting syndrome (CSWS), can develop after neurosurgery. Additionally, DI, SIADH, and CSWS have been reported concurrently in association with some neurosurgical conditions, in particular after pituitary gland surgery or as sequelae of post-traumatic brain injury. Therefore, neurosurgeons should expect water homeostasis disorders after the removal of tumors of the sellar/suprasellar region and be prepared to aggressively manage them.

A novel technique for awake, minimally invasive transforaminal lumbar interbody fusion: technical note.

Minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) is associated with improved patient-reported outcomes in well-selected patients. Recently, some neurosurgeons have aimed to further improve outcomes by utilizing multimodal methods to avoid the use of general anesthesia. Here, the authors report on the use of a novel awake technique for MI-TLIF in two patients. They describe the successful use of liposomal bupivacaine in combination with a spinal anesthetic to allow for operative analgesia.

Spinal arteriovenous malformation associated with Schimmelpenning syndrome.

The authors report the presentation and management of a 13-year-old girl with Schimmelpenning syndrome, a rare neurocutaneous syndrome; this patient suffered hemorrhage of a spinal arteriovenous malformation. This is the first case of a spinal arteriovenous malformation reported in association with Schimmelpenning syndrome. Neurosurgeons should be aware of this rare phacomatosis as well as of the various neurological disorders associated with this diagnosis. The threshold for imaging the neuraxis in these patients should be low.

Treatment of a transorbital penetrating injury: a particular endovascular approach.

The management of craniocerebral penetrating injury currently represents a challenge for neurosurgeons and neuroradiologists and requires innovative planning. This report describes the case of a worker admitted to hospital with an intracranial piece of concrete-cutting saw stuck through the right eye. At the time of admission the patient was conscious and this fact influenced the choice of a particular approach. This patient escaped without neurological deficit or complications, except for the inevitable removal of an eye.

Contribution of MRI to the diagnosis and post-therapeutic monitoring of brain stem tumours.

MRI has become indispensable to the positive diagnosis of brain stem tumours which develop preferentially in young people. The prognosis of these tumours, especially infiltrating gliomas, being relatively sombre, it is important to make a precise morphological diagnosis of the lesions (increase in size, signal abnormalities) which will help both neurosurgeons and radiotherapists. As regards post-therapeutic monitoring, MRI with gadolinium contrast injection is harmless and will make it possible to follow these patients regularly and to detect recurrences.

Gardner neurovascular decompression of the trigeminal and facial nerves for tic douloureux and hemifacial spasm.

Neurovascular decompression of the trigeminal and facial nerves for tic douloureux and hemifacial spasm has been performed for the past two decades, starting with W. James Gardner in 1959. Although other neurosurgeons have refined and further perfected the operation, reporting large series of successful results, the credit for the concept and original procedures, including use of the surgical microscope, has generally escaped the attention of present-day neurological surgeons. This paper attempts to correct this inequity.

Decompression of the brain stem and superior cervical spine for congenital/acquired craniovertebral invagination: an interdisciplinary approach.

A transoropharyngeal/transpalatal approach to the clivus and anterior cervical spine in association with a midline labiomandibular glossotomy has proved successful in the treatment of craniovertebral instability. Nine cases at this institution between 1978 and 1986 were retrospectively reviewed. A head-and-neck surgeon, along with a neurosurgeon and/or orthopedic surgeon collaborated on these procedures. The indications, methods, operative techniques, results and postoperative complications are presented.

Multidisciplinary Surgical Teams for Cranioplasty: Indications and Outcomes at a Single Institution.

Cranioplasty is a common surgical procedure to repair skull defects. Due to cranioplasty's relatively high complication rates, multidisciplinary surgical teams (MSTs) of neurosurgeons and plastic surgeons have been suggested to improve patient outcomes. However, indications for MST remain undefined. To investigate when MSTs are used for cranioplasty and report patient population differences and outcomes compared with solo neurosurgeons (SN), a retrospective cohort study was performed at a single institution. Of 355 cranioplasties, 268 (75.5%) were performed by SN and 87 (24.5%) by a MST. Crani…
