Comparison of two microenvironments and nurse caregiving on thermal stability of ELBW infants.

PURPOSE: This study tested a novel bioinstrumented radiant warmer (RW), designed to provide a reduced-intervention nursing caregiving environment (RINCE) on the ambient and body surface temperature stability of extremely low birth-weight (ELBW) infants (< 1,000 g) during caregiver disruptions. The RINCE was compared with a control bed consisting of a standard RW, modified with a Plexiglas hood (Rohm and Haas, Philadelphia, PA) and an external humidity source. SUBJECTS: The sample consisted of 10 extremely low birth-weight infants. The mean gestational age was 25.7 weeks (+/- 1.4 weeks). Birth

Midwives' personal and professional attitudes towards women's delivery choices, interventions and neonatal care.

OBJECTIVES: Caesarean section (CS) rates in middle- and high-income countries are rising partly due to maternal request. This study aimed to explore the personal and professional attitudes of midwives and nurses towards women's delivery choices, interventions and neonatal care. METHODS: Midwifery and nursing staff at the Coombe hospital were asked to complete a questionnaire concerning decisions for elective CS and neonatal care. The midwives' responses were divided into multiparous and nulliparous according to their own parity. RESULTS: Multiparae and nulliparae did not differ on their pers

The significance of positional care combined with doula delivery during childbirth in the correction of abnormal fetal position.

OBJECTIVE: To explore the effects of positional care combined with doula delivery during childbirth in the correction of abnormal fetal position. METHODS: In this retrospective study, a total 108 pregnant women with abnormal fetal orientation were included from February 2018 to February 2021 in the Jinan City People's Hospital. Among them, 54 patients who received positional care combined with doula delivery were included in the intervention group (IG), while the other 54 patients who received routine nursing were included in the control group (CG). The data of the fetal orientation correctio

Neonatal respiratory support strategies for the management of extremely low gestational age infants: an Italian survey.

BACKGROUND: We aimed to survey Delivery Room and Neonatal Intensive Care Unit (NICU) respiratory strategies dedicated to the extremely low gestational age newborn (ELGAN - GA < 28 wks) in Italy. METHODS: A questionnaire was sent to 113 Italian level III centres. A lead physician and a nurse with expertise in mechanical ventilation (MV) were identified in each unit to answer. Information about those aspects of ventilatory support considered by center's staff as needing improvement was also collected. RESULTS: A 100% response rate was obtained. In the Delivery Room, sustained lung inflation wa

Term Newborn Care Recommendations Provided in a Kenyan Postnatal Ward: A Rapid, Focused Ethnographic Assessment.

BACKGROUND: Neonatal mortality (death within 0-28 d of life) in Kenya is high despite strong evidence that newborn care recommendations save lives. In public healthcare facilities, nurses counsel caregivers on term newborn care, but knowledge about the content and quality of nurses' recommendations is limited. PURPOSE: To describe the term newborn care recommendations provided at a tertiary-level, public referral hospital in Western Kenya, how they were provided, and related content taught at a university nursing school. METHODS: A rapid, focused ethnographic assessment, guided by the cultur

Neonatal care bundles are associated with a reduction in the incidence of intraventricular haemorrhage in preterm infants: a multicentre cohort study.

OBJECTIVE: To investigate the effect of a nursing intervention bundle, applied during the first 72 hours of life, on the incidence of germinal matrix-intraventricular haemorrhage (GMH-IVH) in very preterm infants. DESIGN: Multicentre cohort study. SETTING: Two Dutch tertiary neonatal intensive care units. PATIENTS: The intervention group consisted of 281 neonates, whereas 280 infants served as historical controls (gestational age for both groups <30 weeks). INTERVENTIONS: After a training period, the nursing intervention bundle was implemented and applied during the first 72 hours after bi

The early treatment for ROP (ETROP) randomized trial: study results and nursing care adaptations.

The Early Treatment for Retinopathy of Prematurity (ETROP) study, funded by the National Eye Institute, has shown that early treatment of high-risk prethreshold retinopathy of prematurity (ROP) improves retinal and visual outcomes at 9 months corrected age. These favorable study results have yielded new guidelines for treatment of infants with ROP This paper reviews the study methodology and results and outlines the neonatal and ophthalmic nursing interventions influenced by this new treatment schedule. Four critical phases, screening/examination, treatment, evaluation, and follow-up, are iden

Changed ophthalmic workload following introduction of digital retinal photography for retinopathy of prematurity screening.

BACKGROUND: ROP screening is vital in care of premature infants but is considered burdensome, difficult and time consuming for ophthalmologists. This study assessed the reduction in workload following the introduction of nurse-led WFDRI to a large neonatal nursery. METHODS: We report a retrospective audit of 628 infants screened for ROP in the years 2010, 2013 and 2019 at the Royal Women's Hospital, Victoria. The last complete year of screening for ROP using binocular indirect ophthalmoscopy (BIO) alone (2010) was compared with two subsequent years after the introduction of nurse-led WFDRI. T

Antimicrobial Use in Canadian Cow-Calf Herds.

Despite growing concern surrounding antimicrobial use (AMU) and the importance of cow-calf herds to the Canadian livestock industry, surveillance of AMU in cow-calf herds to inform antimicrobial stewardship programs has been sporadic. Producers from the Canadian Cow-Calf Surveillance Network (87%, 146/168) provided data and almost all reported AMU in at least one animal (99%, 145/146 herds) in 2019-2020. The most common reasons for AMU were treatment of respiratory disease in nursing calves in 78% of herds and neonatal diarrhea in 67% of herds, as well as for lameness in cows in 83% of herds.

Intravenous administration of quinidine and metoprolol for treatment of atrial fibrillation in 2 neonatal foals.

Atrial fibrillation (AF) is a rarely reported arrhythmia in otherwise healthy newborn foals, with a single case of cardioversion using procainamide administration described in the literature. Two neonatal Thoroughbred colts were presented to an equine hospital because of an irregularly irregular tachyarrhythmia and poor latching when trying to nurse. History, physical examination, and initial diagnostic testing including ECG and echocardiography confirmed AF without structural heart disease. The 1st foal converted into normal sinus rhythm after treatment with IV metoprolol and quinidine. The 2

The obstetrics scope of practice of master's prepared FNPs.

Ninety-four family nurse practitioners provided information about the extent of their obstetrics practice within the general activity categories of: diagnosing and managing normal pregnancy; diagnosing and managing problems in pregnancy; parent education during pregnancy; labor and delivery management; neonatal management and evaluation; and post-partum evaluation of the mother. Analysis of the results revealed that respondents provided education-related care more frequently than the management aspects of problem pregnancies, labor and delivery and neonatal care. Routine prenatal care--diagnos

Role of mother-young interactions in the survival of offspring in domestic mammals.

The defining characteristic of mammals is that females nurse and care for their young; without this, the neonate has no chance to survive. Studies on wild and domestic species show that the neonatal period is the most critical step in the lifetime of a mammal. This review compares three well-studied species (the rabbit, pig and sheep) that differ in their parental strategies and in the problems that neonates have to overcome. As a general trend, mother-young interactions vary according to the maturity of the newborn, and the size of the litter. Neonatal survival relies to a great extent on an

The toxigen initiative: targeting oxygen saturation to avoid sequelae in very preterm infants.

UNLABELLED: Supplemental oxygen plays a critical role in the care of infants born at the lower limits of viability, but not without the risk of morbidity resulting from high levels or prolonged exposure. PURPOSE: The purpose of this quality improvement project was to reduce exposure to hyperoxia as evidenced by SpO2 values within the established target range (88%-92%) among very preterm infants (VPIs) in a level 3 neonatal intensive care unit (NICU). Reducing exposure to hyperoxia in this population of exquisitely vulnerable infants has been associated with reduced morbidity, including retino

[New progress in the diagnosis and treatment of natal teeth and neonatal teeth].

There were great individual differences in eruption time of the teeth. Generally speaking, the deciduous teeth begin to erupt at 6 months after birth, but some babies are born with erupted teeth, which are called natal teeth; in addition, teeth erupted within 30 days after the baby is born are called neonatal teeth. Natal teeth and neonatal teeth may cause ulceration, aspiration, and nipple pain or trauma in the mother's breast during the time of breastfeeding. Extraction of the teeth may cause complications such as neonatal osteomyelitis. To avoid the complications caused by these diseases, a

Neonatal Mortality of Planned Home Birth in the United States in Relation to Professional Certification of Birth Attendants.

INTRODUCTION: Over the last decade, planned home births in the United States (US) have increased, and have been associated with increased neonatal mortality and other morbidities. In a previous study we reported that neonatal mortality is increased in planned home births but we did not perform an analysis for the presence of professional certification status. PURPOSE: The objective of this study therefore was to undertake an analysis to determine whether the professional certification status of midwives or the home birth setting are more closely associated with the increased neonatal mortalit

Perinatal implications of sickle cell disease.

Sickle cell disease (SCD) affects millions of people across the globe. In the United States, approximately 70,000 to 100,000 people have the disease, and 2 million have the sickle cell trait. SCD occurs once in every 500 African American births, and once in 36,000 Hispanic American births. Women with SCD can have more adverse maternal outcomes such as preeclampsia, eclampsia, preterm labor, placental abruption, intrauterine growth restriction, and low birthweight. Providing comprehensive nursing care to women with SCD is a challenge, particularly during labor and birth, with nursing management

Epidermolysis bullosa: pathophysiology and nursing care.

Epidermolysis bullosa (EB) is defined as a group of skin disorders that presents as blistering of the skin in varying degrees of severity. Most cases are inherited, but rare cases may be acquired. There are three main types: simplex, junctional, and dystrophic. The dermal-epidermal junction of the newborn skin is a vital area of attachment. Any defects in the components that comprise this junction can lead to fragility of the skin. EB diagnosis and testing can be per formed both prenatally and postnatally. There is no cure for EB. Treatment revolves around supportive care and prevention of com
