Evaluation of changes in the width of gingiva in children and youth. Review of literature.

Comprehensive health care in children and youth includes periodic oral examinations. The mucogingival complex undergoes significant changes during development. The factors that impact the width of attached and keratinized gingiva during this period of life are: tooth eruption phase, the position in the arch, the type of frena attachment and oral hygiene. Along with the child's development the width of attached and keratinized gingiva increases, except for the period of tooth replacement, when a temporary narrowing of attached gingiva of erupting permanent teeth is observed. The understanding o

Case report--swallowed piece of archwire.

A fifteen year old healthy, male patient contacted our office stating that whilst eating a bread roll he had swallowed a piece of orthodontic archwire. He complained of stomach pain and was advised to contact his local medical practitioner who had the area radiographed. The radiographic examination revealed the piece of archwire to be lodged in the pylorus. The patient was then referred to a consultant physician who, using a gastroscope and with the orthodontist present, to assist with supervision of the orthodontic appliance during gastroscopy and to determine that the piece removed had not u

An algorithm for ordering pretreatment orthodontic radiographs.

A study was conducted to identify selection criteria for ordering pretreatment orthodontic radiographs. Thirty-nine orthodontists evaluated six test cases. They provided information on the rationale for ordering each specific radiograph and the impact of the radiograph on the diagnosis and treatment plan. Skeletal relationship of the jaws was the most common indication for a radiograph request, followed by root formation/length and molar position or development. Of the radiographs, 16% produced a change in diagnosis, and 20% produced a change in treatment plans. The criteria specified for the …

Evolving standards in orthodontic certification: The American Board of Orthodontics role in defining competency, advancing the specialty, and serving the public.

As a credentialing organization, the American Board of Orthodontics (ABO) has the responsibility to continually evaluate and refine its format and structure to ensure that they remain relevant to its stakeholders, including examination candidates, current board-certified orthodontists, and the general public. This article describes how the ABO's current credentialing process has evolved to align with contemporary credentialing trends, particularly within the broader healthcare field. Furthermore, the ABO explains its role regarding the concepts of standardization, curriculum validation, public…

Expert consensus on orthodontic treatment of patients with periodontal disease.

Patients with periodontal disease often require combined periodontal-orthodontic interventions to restore periodontal health, function, and aesthetics, ensuring both patient satisfaction and long-term stability. Managing these patients involving orthodontic tooth movement can be particularly challenging due to compromised periodontal soft and hard tissues, especially in severe cases. Therefore, close collaboration between orthodontists and periodontists for comprehensive diagnosis and sequential treatment, along with diligent patient compliance throughout the entire process, is crucial for ach…

Integrating skeletal anchorage into fixed and aligner biomechanics.

Routine alignment with fixed appliances and aligners is indeterminate mechanics because equilibrium equations are only applicable to two abutments: teeth, segments, or arches. Orthodontists must depend on compliance and resilience of materials (archwires and aligners) for initial alignment. However, stabilized segments and arches are "large multirooted teeth" that can be moved with determinate mechanics using temporary skeletal anchorage devices. Temporary skeletal anchorage devices have advanced from retromolar implants and inter-radicular miniscrews to extra-alveolar bone screws placed in th…

Uncovering and autonomous eruption of palatally impacted canines.

BACKGROUND: The palatal impaction of maxillary canine is a quite common yet challenging clinical condition that orthodontists have to face in the everyday practice. The purpose of this report is to describe and discuss a case involving the uncovering of an upper canine and it subsequent spontaneous eruption. CASE REPORT: The diagnostic and therapeutic protocol of a case with a palatally impacted canine is presented. The upper right canine was uncovered with an open surgical approach. Removal of the attached gingiva and bone covering the crown enhanced the tooth spontaneous eruption, while the…

Orthodontic Replacement of Lost Permanent Molar with Neighbor Molar: A Six-Year Follow-Up.

Extraction is very frequent indication in orthodontic planning, especially when there are crowding, biprotrusion, and aesthetically unpleasant profiles. Next to extraction comes space closure, which represents a challenge for orthodontists because of extended treatment time, discomfort created for the patient, tissue tolerance, and stability concerns. When it comes to what mechanics to choose for space closure, loops present two major advantages in relation to sliding mechanics: absence of abrasion and possibility to reach pure dental translation. A case is presented where an adult female pati…

Maxillary canine-first premolar transposition in the permanent dentition.

Maxillary canine premolar transposition is the most frequently reported transposition type, which many orthodontists face. Although correcting the transposed tooth order is not advised after the eruption of the permanent tooth, several articles published in the last decade demonstrated nonextraction treatment of transposition using fixed mechanics. This article describes the nonextraction treatment of a complete transposition between a maxillary left canine and a first premolar, using similar mechanics as suggested earlier. The correct tooth order was established with a functional Class I cani…

Modern restorative management of patients with congenitally missing teeth: 4. The role of implants.

UNLABELLED: In this fourth and final paper on the management of patients with congenitally missing teeth, the use of dental implants is considered. This group of patients often provides unique challenges when implant reconstruction is considered. Often a multidisciplinary approach is required involving orthodontists, oral surgeons and restorative dentists. The use of dental implant systems is a rapidly developing area of dentistry and its implications on hypodontia are discussed. This paper identifies the problems associated with a congenital absence of teeth and addresses their management in …

Acceptance rate of palatal implants: a questionnaire study.

Implants have gained popularity in orthodontics because they facilitate maximum anchorage with minimum patient cooperation. However, some orthodontists avoid using implants because psychological aspects of the anchorage systems have not been fully reported. In this study, 85 patients who received orthodontic treatment with palatal implants in 2 clinics in Austria completed questionnaires. The results show that most patients got used to their implants in about 2 weeks; 95% were satisfied with the treatment, and 86% would recommend the treatment to other patients. In addition, 75% of the patient…

Orthodontic and prosthodontic treatment of ectodermal dysplasia--a case report.

In addition to its other symptoms, ectodermal dysplasia causes anodontia and hypodontia intraorally. Partial or total anodontia results in some loss of function, such as chewing, and affects aesthetics. Prosthodontic rehabilitation can be accomplished with fixed, overdenture, complete, or implant-retained prostheses. For rehabilitation, it is crucial to know the age, number and condition of present teeth, and the state of growth of the patient. A 10-year-old male patient who visited our clinic was treated by a multi-disciplinary team of surgeons, orthodontists, and prosthodontists. An overdent…

Orthodontic treatment planning for inclusion of the third molar in the dental arches: Part II.

Marked lower anterior crowding (late crowding or post-adolescent crowding) often develops at about the time of third molar eruption. Many studies have implicated the presence of third molars as a cause of this crowding. Despite the fact that there is no clear evidence that this is the case, many orthodontists and their patients' referring dentists routinely remove third molars at the retention stage of treatment to prevent crowding later; Ricketts et al. even advocate enucleation of the third molar while the patient is very young to prevent future crowding. In the following report on two ortho…

Guidelines for managing the orthodontic-restorative patient.

Occasionally, patients require restorative treatment during or after orthodontic therapy. Patients with worn or abraded teeth, peg-shaped lateral incisors, fractured teeth, multiple edentulous spaces, or other restorative needs may require tooth positioning that is slightly different from a nonrestored, nonabraded, completely dentulous adolescent. Generally, orthodontists are not accustomed to dealing with patients who require restorative intervention. Should the objectives of orthodontic treatment differ for the restorative patient compared with the nonrestorative patient? How should the teet…

Periodontosis: a problem in orthodontics.

1. Periodontosis occurs in the same age group in which orthodontic therapy is most often done. It is a periodontal disease which in the early stages may show little or no gingival inflammation and is easily overlooked unless one is aware of the existence of this condition and looks for it. 2. Since malocclusions are common in adolescents, it is reasonable to expect that some adolescents with malocclusions will also develop periodontosis. 3. Periodontosis may have its onset during active orthodontic treatment. 4. Orthodontists, should routinely examine all of their patients, using adequate roen…
