Paget's disease of bone in patients with gout.

In a study designed to evaluate the radionuclide images in patients with gout, six (23%) of the 26 patients had clear evidence of Paget's disease of bone by technetium Tc 99m medronate imaging. A reference population consisting of 333 technetium Tc 99m medronate bone scans ordered for other reasons was reviewed, and only seven scans (2.1%) were found to have evidence of Paget's disease. This difference was found to be highly significant. All cases of Paget's disease were confirmed by independent radiologic evaluation. We conclude that there is a significant association between Paget's disease

Can the three-phase bone scan differentiate osteomyelitis from metabolic or metastatic bone disease?

The three-phase bone scan has been reported of value in the diagnosis of osteomyelitis. The use of a radionuclide angiogram, an immediate postinjection "blood pool" image, and 2 to 3 hour delayed image have been useful in separating nonosseous inflammatory disease from osteomyelitis. However, dependence on increased blood flow and focal hyperemia for this diagnosis may limit the use of the study if other processes are shown to produce similar results. To identify limitations of the three-phase bone scan we studied 14 patients with metastatic bone disease and nine patients with Paget's disease.

Total hip replacement for Paget's disease of the hip.

We reviewed our experience with total hip replacement for coxarthrosis due to Paget's disease. The clinical and radiographic results in twenty-one patients who had a total hip replacement between October 1972 and February 1982 were analyzed. The ages of the patients averaged 68.6 years (range, fifty-seven to eighty years) and there were twelve women and nine men. The follow-up averaged five years and two months (range, two years to eleven years and four months). A good or excellent result was achieved in eighteen patients. Of the other three patients, two required a revision operation at two a

Suppressive effect of elcatonin, an eel calcitonin analogue, on excessive urinary hydroxyproline excretion in polyostotic fibrous dysplasia (McCune-Albright's syndrome).

A 12-year-old Japanese girl with polyostotic fibrous dysplasia and endocrine concomitants, was treated with elcatonin, a synthetic eel calcitonin analogue, 10 MRC unit/twice a week given by intramuscular injection. Significant decreases in 24 hr urinary content of hydroxyproline and other amino acids from bone collagen were observed during the course of treatment over 5 months. This biochemical result suggests that the synthetic eel calcitonin analogue exhibits the therapeutic effect in patients with polyostotic fibrous dysplasia by inhibiting bone resorption.

Pharmacology and therapeutic use of calcitonin.

Calcitonin is a peptide hormone secreted by the C-cells of the thyroid gland. A major physiological function of the hormone appears to be the protection of the skeleton against resorption in humans. It thus opposes the resorptive actions of parathyroid hormone and 1,25 dihydroxyvitamin D. This action is utilised pharmacologically in order to treat diseases where increased bone resorption is a major component. The efficacy of calcitonin in the treatment of Paget's disease of bone is well established, especially as it is currently the most effective agent in the treatment of the osteolytic form

Effects of parathyroid hormone and calcitonin on plasma and nephrogenous cyclic adenosine-3',5'-monophosphate in normal subjects and in pathological conditions.

To evaluate the response of bone cells and kidney to parathyroid hormone (PTH) and calcitonin (CT), the acute effects of these hormones given intravenously on plasma cyclic adenosine 3',5'-monophosphate (cAMP), nephrogenous cAMP and clearance ratio (cAMP:creatinine), have been studied in normal subjects and in patients with Paget's disease, hypoparathyroidism and osteopetrosis. Twenty-five subjects were given bovine synthetic PTH, thirty-eight calcitonin (sixteen salmon, eighteen human and four porcine CT). In normal subjects and in patients with Paget's disease and hypoparathyroidism plasma c

The treatment of Paget's bone disease with sodium ethidronate.

Of 58 patients with Paget's disease treated with disodium ethane-1-hydroxy-1,1-diphosphonate (EHDP) in doses of 20 and 10 mg/kg/day, a group of 20 patients with the oligostotic form of the disease showed a significantly greater incidence of complications, such as worsening of bone pains, when compared with a group of 38 patients with the polyostotic disease (P less than 0.05). The group of 32 patients that received 20 mg/kg/day showed a greater although not significant incidence of clinical complications than the group of 26 patients treated with 10 mg/kg/day. Bone biopsies performed in one po

Evaluation of the Role of Interleukin 6 in Paget's Disease of Bone.

Raised levels of serum interleukin 6 (IL-6) have been found in some studies of Paget's disease of bone (PDB), and this cytokine has been suggested to act as a mediator of increased osteoclast activity in the disease. Here weevaluated serum IL-6 in two cohorts of patients with PDB and controls and related serum IL-6 levels in PDB patients to clinical features of the disease. Serum IL-6 was measured by ELISA in 553 individuals with PDB from the PIP study (n =  165), the PRISM study (n = 388) and 533 population-based controls. In the PDB cases, we studied associations between IL-6 and disease ext

Continuous decline in Paget's disease of bone: a One Health perspective on 2342 new diagnoses and their association with rural livestock.

OBJECTIVES: To characterize the incidence of clinically diagnosed Paget's disease of bone (PDB) in Castille and Leon, a region in the central plateau of Spain, from 2009 to 2019, and explores its association with environmental factors, particularly livestock activity. METHODS: We analysed primary care electronic medical records to assess PDB incidence trends over time and geographic differences across counties. An ecological study was conducted to explore associations between livestock density and PDB incidence in different regions. RESULTS: A total of 2342 new cases of PDB were identified,
