The use of local muscle flaps in the treatment of chronic osteomyelitis is outlined. The defect treated must be in an anatomic location that permits rotation of the muscle flap. A recent review of 42 patients treated with this technique showed the local muscle flap to be efficacious in 39 of the 42 patients treated.

Actinomyces israelii is a part of the human oral flora and thus is more commensal than pathogenic. Oral trauma, accidental or purposeful, can precipitate its introduction into the soft tissues, to which infection is usually confined. The case presented is one in which, over a span of two decades (1956 to 1977), the extraction of a mandibular tooth resulted in a chronic actinomycotic osteomyelitis, destroying first the mandible, then both maxillae, and then the right zygoma, with decreased vision and proptosis despite repeated medical and surgical intervention. Adequate treatment required remov…

The authors analyze 14 cases of acute hematogenic osteomyelitis of the vertebral column in children. Three variations of the clinical picture are described: meningitis-like, local and lightening types. Roentgenological symptoms were late to appear and certain patients might have no symptoms at all. Formation of purulent tuberculous abscesses near cartilaginous ends of ribs were observed and considered to be a characteristic feature of osteomyelitis of thoracic vertebrae in infants. The authors point to difficulties in opening the purulent abscesses and to the importance of complex treatment. S…

BCG-osteomyelitis is a rare but serious late complication of BCG-immunisation. Be present clinical and immunological findings in 2 children aged 9 months and 5 years, who developed BCG-osteomyelitis, following immunisation with BCG-vaccine. The affected bones were the first metacarpal bone in one and the 6th rib in the other patient. Both children remained free of recurrences after surgical removal of the granulomatous tissue. Immunological investigations in both cases demonstrated normal humoral immunity (Complement CH50, C3, C4, immunoglobulins). Except for a diminished proportion of circula…

Sixty-nine cases of Osteomyelitis, and 27 of septic arthritis have been presented. 69/96 (71%) were children below 12. The sites frequently involved were tibia, femur and humerus. Septic arthritis commonly involved the knee. The commonest organism was Penicillin - resistant Staphylococcus aureus. Though we sought but failed to establish that overt clinical malnutrition was aetiologically important, since 54% were well-fed children, the virulence of the sepsis in which an entire bone shaft may die is disturbing but unexplained. Whereas arthrotomy plus antibiotics gave uniformly good results for…

Bone scans detect osteomyelitis earlier, sometimes several days earlier, than x-rays. Prompt institution of therapy, before significant bone damage has developed, thus becomes possible. A positive scintigram is, however, nonspecific, and firm diagnosis requires radiologic and other confirmation. Moreover, because the technique is so new, many open questions remain both about its optimal utilization and about its potential.

Neonatal osteomyelitis presents with few clinical signs despite multiple sites of involvement. Four cases of osteomyelitis due to Staphylococcus aureus or Candida albicans were encountered in a neonatal intensive care unit. Three were unsuspected clinically and were detected as incidental radiologic findings. The fourth presented with soft-tissue abscesses. Long bone metaphyses were most frequently affected. Other sites included iliac bones, clavicles, and spine. On follow-up the bones healed, but one patient was left with hip deformity secondary to destruction of the cartilaginous femoral hea…

A case of osteomyelitis of the mandible caused by actinomyces infection is described. Gram stain and cultures of the discharge in fluid thioglycolate medium and blood agar, as well as an analysis of the biochemical properties of the causative organism with API system, indicated the infection was caused by A israelii. Lesions in both hard and soft tissues responded to chemotherapy with cephalexin, minocycline, and aminobenzylpenicillin and drainage of the pus from the cutaneous abscesses.

Coccidioidomycotic osteomyelitis developed at the angle of the right side of the mandible in a 5-month-old Papago infant. The disease was successfully treated with a combination of amphotericin B, surgery, and transfer factor with complete immunological, microbiological, and radiological cure. At 4 years of age, the only residual effect is prominence of the right hemimandible with asymmetry of the jaw.
