Atraumatic Splenic Rupture Caused by Splenic Vein Thrombosis in a Patient With Chronic Pancreatitis.

Atraumatic splenic rupture is a rare but potentially life-threatening complication of pancreatic disease. This report describes the case of a 62-year-old man with a prior history of chronic pancreatitis related to alcoholic pancreatitis who presented with left upper abdominal pain without any history of trauma. Contrast-enhanced computed tomography demonstrated splenomegaly, a large splenic laceration, and a perisplenic hematoma without active contrast extravasation, along with a pancreatic cystic lesion and narrowing of the splenic vein. Magnetic resonance imaging showed heterogeneous signal

Pancreatic neuroendocrine tumor detected by technetium-99m methoxy-2-isobutylisonitrile single photon emission computed tomography/computed tomography: A case report.

BACKGROUND: Pancreatic neuroendocrine tumors (NETs) account for about 1%-2% of pancreatic tumors and about 8% of all NETs. Computed tomography (CT), magnetic resonance imaging, and endoscopic ultrasound are common imaging modalities for the diagnosis of pancreatic NETs. Furthermore, somatostatin receptor imaging is of great value for diagnosing pancreatic NETs. Herein, we report the efficacy of technetium-99m methoxy-2-isobutylisonitrile (99mTc-MIBI) single photon emission CT (SPECT)/CT for detecting pancreatic NETs. CASE SUMMARY: A 57-year-old woman presented to our hospital with a 1-d histo

Exenatide-induced acute pancreatitis.

OBJECTIVE: To report acute pancreatitis in a patient with non-insulin-dependent diabetes mellitus (NIDDM) receiving exenatide and critically review previous reports. METHODS: We describe clinical and laboratory data of a woman with probable exenatide-induced pancreatitis and apply the same criteria to previously published cases. RESULTS: A 64-year-old, nonalcoholic woman with NIDDM presented with a 1-month history of epigastric pain beginning 2 days after starting exenatide. Serum lipase concentration was 2700 U/L (reference range, 114-320 U/L), and serum amylase concentration was 131 U/L (r

Elevated circulating free fatty acids levels causing pancreatic islet cell dysfunction through oxidative stress.

BACKGROUND: Elevated plasma free fatty acids (FFA) concentration is predictive of the conversion from normal glucose tolerance and impaired glucose tolerance to diabetes. AIMS: To evaluate the effects of prolonged exposure to FFA on basal and glucose-stimulated insulin secretion (GSIS) of pancreatic beta-cell, and to investigate the role of oxidative stress in FFA-induced decrease in beta-cell function. METHODS: Rats were assigned to 3 groups and underwent 96-h infusions of normal saline (NS), intralipid plus heparin (IH), or intralipid plus heparin and N-acetylcysteine (IH+NAC). The plasma

Steatorrhea and pancreatic insufficiency in Shwachman syndrome.

Fat absorption was assessed on two or more occasions in 12 of 14 patients with Shwachman syndrome. Of 11 children who initially had steatorrhea, 6 were subsequently found to have normal fat excretion. In 10 of these 11 patients the last estimate was smaller than the initial estimate after an interval of 0.3-12.7 yr. The most dramatic improvement occurred under 4 yr of age but improvement appeared to continue over a more extended period where sufficient follow-up was available. All patients had pronounced pancreatic insufficiency as tested by duodenal drainage corrected by nonabsorbable marker

PancDS in Real-World Practice: A Prospective Multicenter Validation of a Clinical Decision-Support System Bridging Experience Gaps in Pancreatic Lesion Diagnosis.

Distinguishing pancreatic ductal adenocarcinoma (PDAC) from mass-forming pancreatitis (MFP) is challenging due to imaging mimicry and reader-dependent variability. PancDS is developed as a biomimetic pancreatic decision-support system that integrates clinical predictors, a radiomics signature, and self-developed deep features (PANet). PancDS is enabled by TriFusionNet, an adaptive fusion strategy designed to emulate expert reasoning by dynamically reweighting modalities according to diagnostic relevance. In a retrospective multicenter cohort of 1006 consecutive patients with pathologically con

Altered Glut-2 accumulation and beta-cell function in mice lacking the exocrine-specific transcription factor, Mist1.

Mist1 is an exocrine-specific transcription factor that is necessary for the establishment of cell organization and function of pancreatic acinar cells. While Mist1 is not expressed in the endocrine pancreas, the disorganized phenotype of the exocrine component may affect endocrine function. Therefore, we examined endocrine tissue morphology and function in Mist1-knockout (Mist1(KO)) mice. Endocrine function was evaluated using a glucose-tolerance test on 2-10-month-old female mice and revealed a significant reduction in glucose-clearing ability in 10-month-old Mist1(KO) mice compared with wil

Functional assessment of pancreatic beta-cell area in humans.

OBJECTIVE: beta-Cell mass declines progressively during the course of diabetes, and various antidiabetic treatment regimens have been suggested to modulate beta-cell mass. However, imaging methods allowing the monitoring of changes in beta-cell mass in vivo have not yet become available. We address whether pancreatic beta-cell area can be assessed by functional test of insulin secretion in humans. RESEARCH DESIGN AND METHODS: A total of 33 patients with chronic pancreatitis (n = 17), benign pancreatic adenomas (n = 13), and tumors of the ampulla of Vater (n = 3) at various stages of glucose t

Systemic responses following brain injuries and inflammatory process activation induced by a neurotoxin of Androctonus scorpion venom.

OBJECTIVE: Kaliotoxin 2 (KTX2), a neurotoxin isolated from Androctonus australis hector scorpion venom, presents a high affinity with the voltage-gated potassium channels. The targets of KTX2 in the brain and its toxic effects on the cerebral cortex have been extensively studied; however, its deleterious systemic effects on other organ systems have not yet been investigated. Inflammatory response induced by KTX2 is supported by cytokine release which could provoke multiple organ dysfunction and diverse biological disorders in mammals. The possibility that inflammatory response and brain injuri

Long-term endocrine function of duct-ligated pancreas isotransplants in rats.

Duct-ligated pancreas transplants were used to study the long-term B cell function after total acinar atrophy. Vascularized whole-organ pancreas transplantation was performed in 47 streptozotocin-diabetic inbred Wistar rats. Of 31 recipients which survived the first week, 30 were permanently cured of the diabetic state with restoration of normal blood glucose levels within 24 h. Metabolic and morphologic studies were performed for up to 16 months after transplantation, i.e. for most of the normal life span of the rats. The recipients of duct-ligated, heterotopic transplants demonstrated plasma

The tales of two neighbours: when cholecystitis does not preclude pancreatitis.

An 83-year-old lady with no previous history of gallstones, presented with a sudden-onset severe epigastric pain radiating through to the back associated with nausea and vomiting. On examination, the patient's vital signs were normal. There was severe epigastric tenderness on palpation, as well as moderate right upper quadrant tenderness. Serological investigations showed raised inflammatory markers and serum lipase of 13 000, confirming the diagnosis of acute pancreatitis. Liver function tests were mildly deranged with a normal bilirubin of 12 μmol/L. An abdominal ultrasound demonstrated a di

Functional studies of the parotid and pancreas glands in amyotrophic lateral sclerosis.

Functional studies of the pancreas and parotid glands are reported in 17 patients with amyotrophic lateral sclerosis (ALS). The exocrine function of the pancreas was studied by measuring amylase concentration after stimulation with the endogenous secretin-pancreozymine test (ESP). Under these conditions, the pancreatic amylase concentration in ALS patients was found to be markedly decreased by about 45% when compared with those of healthy control subjects. Different conclusions in the literature about a possible impairment of the exocrine pancreas in ALS patients induced us to study the functi

A Case of Heterozygous INS Gene Missense Variant in Neonatal Diabetes with Beta-Cell Autoantibodies: Experience in Limited Resources Setting.

INTRODUCTION: Genetic analysis is essential for diagnosing, treating, and predicting complications in neonatal diabetes mellitus (NDM) but is unavailable in some regions. Sulfonylureas are effective for NDM caused by KCNJ11 or ABCC8 mutations, which are among the most common genetic causes; therefore, they are often given before genetic testing. Unfortunately, in certain ethnicities, this mutation rarely occurs. This report presents a case of NDM with a heterozygous INS gene missense mutation and positive pancreatic autoantibodies in a single individual. CASE PRESENTATION: A three-month-old b
