Epidemiology of paragonimiasis in Cross River basin, Nigeria: prevalence and intensity of infection due to Paragonimus uterobilateralis in Yakurr local government area.

Prevalence and intensity of paragonimiasis among inhabitants of the Yakurr Local Government Area in the Cross River basin of Nigeria is reported. Diagnosis was based on a single sample detection of eggs of Paragonimus in 5 ml of sputum. 12.27% out of 880 subjects were infected. The infections were due to Paragonimus uterobilateralis. Prevalence of the infection increased with age, up to a peak prevalence of 23.75% (19 out of 108) among subjects between the ages of 17-22 y old. Thereafter, it declined progressively with increase in age to much lower values such that by the age of 53-58 y old, i…

A case report of pulmonary paragonimiasis diagnosed via the deterioration of pulmonary cavities during high-dose corticosteroid therapy for chronic eosinophilic pneumonia.

A 32-year-old Chinese woman with a cough and bloody sputum was diagnosed with chronic eosinophilic pneumonia. After starting corticosteroid therapy, chest radiography showed improvement in the bilateral infiltrative shadow. However, chest image findings showed worsening of the bilateral infiltrative shadow and pulmonary cavities during the 20 mg/day of oral prednisolone therapy. Transbronchial lung biopsy and bronchoalveolar lavage were repeated, and the patient was diagnosed with pulmonary paragonimiasis. After starting praziquantel treatment, chest radiography findings and clinical symptoms …

Occupational exposure to freshwater crustaceans as a route of paragonimiasis presenting with eosinophilic pleural effusion misdiagnosed as tuberculous pleurisy: A case report and literature review.

Paragonimiasis, a foodborne parasitic disease caused by Paragonimus, typically presents with pleural effusion and is often misdiagnosed as tuberculous pleurisy, especially in endemic regions. The present study aimed to investigate the causes of misdiagnosis of pleural effusion (PE) caused by paragonimiasis as tuberculous pleurisy and to propose preventive strategies. The clinical diagnostic and treatment course of one patient with paragonimiasis misdiagnosed as tuberculous PE (TPE) was analyzed. A literature review of English-language case reports and series on paragonimiasis with PE misdiagno…

Identification of suspected paragonimiasis-endemic foci using a questionnaire and detection of Paragonimus ova using the Ziehl-Neelsen technique in Zamboanga Region, the Philippines.

Improving paragonimiasis surveillance, which is crucial for disease control, requires adopting new tools and techniques useful in mapping endemic areas. This study aimed to (1) develop a questionnaire to identify suspected paragonimiasis-endemic foci, (2) describe the epidemiology of paragonimiasis, and (3) evaluate Ziehl-Nielsen Staining technique (ZNS) in detecting Paragonimus ova. The questionnaire, which municipal health officers filled out, was based on proposed site inclusion criteria utilized in the integrated tuberculosis (TB)-paragonimiasis surveillance and control project. Newly depl…

A Cluster of Paragonimiasis with Delayed Diagnosis Due to Difficulty Distinguishing Symptoms from Post-COVID-19 Respiratory Symptoms: A Report of Five Cases.

Paragonimiasis caused by trematodes belonging to the genus Paragonimus is often accompanied by chronic respiratory symptoms such as cough, the accumulation of sputum, hemoptysis, and chest pain. Prolonged symptoms, including respiratory symptoms, after coronavirus disease 2019 infection (COVID-19) are collectively called post-COVID-19 conditions. Paragonimiasis and COVID-19 may cause similar respiratory symptoms. We encountered five cases of paragonimiasis in patients in Japan for whom diagnoses were delayed due to the initial characterization of the respiratory symptoms as a post-COVID-19 con…

Soluble CD23 and IL-5 levels in the serum and culture supernatants of peripheral blood mononuclear cells in a girl with cutaneous paragonimiasis: case report.

We examined the levels of soluble CD23 (sCD23) and IL-5 in the serum and culture supernatants of the peripheral blood mononuclear cells of a 9-year-old girl with cutaneous paragonimiasis without respiratory symptoms. Before treatment, levels of both sCD23 and IL-5 in her serum and culture supernatants were elevated compared with those of controls. After successful treatment with praziquantel, sCD23 and IL-5 levels rapidly reduced to normal levels. These results indicate that sCD23 and IL-5 are involved in the immune-mediated pathological responses seen in paragonimiasis.

Endemicity of Paragonimus and paragonimiasis in Sub-Saharan Africa: A systematic review and mapping reveals stability of transmission in endemic foci for a multi-host parasite system.

Paragonimiasis is caused by zoonotic trematodes of Paragonimus spp., found in Asia, the Americas and Africa, particularly in tropical regions. These parasites have a complex, multi-host life cycle, with mammalian definitive hosts and larval stages cycling through two intermediate hosts (snails and freshwater decapod crustaceans). In Africa, paragonimiasis is particularly neglected, and remains the only human parasitic disease without a fully characterised life cycle. However paragonimiasis has potentially significant impacts on public health in Africa, and prevalence has likely been underestim…

IgG4 elevation dilemma at the crossroads of helminth infection and IgG4-related disease: insights from a case report of pediatric paragonimiasis with persistent pericardial effusion.

BACKGROUND: Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is a systemic fibroinflammatory disorder defined by elevated serum IgG4, tiss … m when histopathology is unavailable. CASE DESCRIPTION: A 10-year-old boy with confirmed pulmonary paragonimiasis developed refractory pericardial effusion and markedly elevated IgG4 [11 g/L, 8.1× upper limit of normal (ULN)]. Despite five courses of praziquantel (75 mg/kg/day), effusions persisted for 8 weeks. Subsequent glucocorticoids (methylprednisolone 2 m …

Paragonimus westermani found in the tip of a little finger.

Paragonimiasis is the infestation of lung flukes of the trematode genus Paragonimus. Because the symptoms and radiologic findings of paragonimiasis mimic those of tuberculosis, some patients with paragonimiasis are initially treated for tuberculosis. Although Paragonimus may also reach ectopic sites such as the peritoneum or brain, infection in the skin is rare. To our knowledge, paragonimiasis has not been found in the tip of a finger. We report a case of 39-year-old woman who was belatedly diagnosed as having paragonimiasis with a parasitic migration to the tip of the left little finger afte…

Reversing the poor prognosis of primary amyloid light‑chain amyloidosis with cardiac involvement.

We used multi-row detector computed tomography (MDCT) to identify the distinguishing characteristics of hepatic paragonimiasis and small hepatocellular carcinoma lesions. We analyzed a cohort of 60 patients, of which 26 had hepatic paragonimiasis and 34 with a small (≤ 3cm) hepatocellular carcinoma. MDCT detected 65 lesions that were retrospectively reviewed and analyzed based on their imaging features. Both groups showed distinct MDCT imaging features that could contribute to an accurate diagnosis. In the paragonimiasis group, 75% (21/28) lesions were located in the hepatic subcapsular region…

Paragonimus westermani: pathogenesis and clinical features of infection.

PATHOGENESIS: Although the pathogenesis of human infection of P. westermani is not fully elucidated, experimental infections of cats or dogs could explain the early phase of paragonimiasis. As the larvae of P. westermani penetrate the intestinal wall and localize in the peritoneal cavity there appears to be a considerable migration inside the abdominal cavity before they direct toward the chest cavity through the diaphragm. Approximately 20 days following experimental infection with metacercariae by oral route first pathological changes can be detected in the pleural cavity with turbid or haem…

A curious case of hemoptysis.

Paragonimiasis is a crustacean-borne parasitic zoonosis caused by lung fluke or Paragonimus. Several endemic foci of paragonimiasis have been discovered in Northeastern India. Chronic pulmonary infection with Paragonimus is characterized by hemoptysis, which is usually mistaken for Tuberculosis (TB). Paragonimiasis is acquired by eating raw, undercooked, or pickled crabs or crayfish containing encysted metacercariae of Paragonimus. Infection can usually be confirmed by finding Paragonimus eggs in sputum or bronchoalveolar lavage. We present a rare case of paragonimiasis masquerading as hypereo…

Paragonimus westermani infection in lung: A confounding diagnostic entity.

Paragonimiasis is a food-borne parasitic zoonosis caused by the genus Paragonimus. Fresh water snails, crabs, and crayfish are the first and second intermediate hosts, respectively. Humans acquire this infection by ingesting uncooked/undercooked crustaceans. Laboratory diagnosis of Paragonimiasis is done by demonstration of ova in the sputum/feces/pleural fluid or by serology. A case of pulmonary Paragonimiasis is presented herewith; the patient having been diagnosed with pulmonary tuberculosis earlier. The aim of this presentation is to highlight this entity so that it is considered in the di…
