First bite syndrome: incidence, risk factors, treatment, and outcomes.

OBJECTIVES/HYPOTHESIS: First bite syndrome (FBS) refers to facial pain characterized by a severe cramping or spasm in the parotid region with the first bite of each meal that diminishes over the next several bites.1, 2 It is a potential sequela of surgery involving the infratemporal fossa (ITF), parapharyngeal space (PPS), and/or deep lobe of the parotid gland. The incidence, risk factors, treatment options, and outcomes of FBS are poorly understood. We hypothesized that certain clinical and tumor variables independently predict the development of FBS. STUDY DESIGN: Retrospective cohort study…

Surgical management of the neck in patients with metastatic melanoma in parotid lymph nodes.

BACKGROUND: The role and extent of neck dissection in patients with parotid metastatic cutaneous head and neck melanoma remain unclear. The aims of this study were to determine the incidence and patterns of cervical node involvement in patients with parotid metastatic melanoma, and to determine if a limited lymphadenectomy of the clinically negative neck is appropriate. METHODS: Patients who underwent parotidectomy and neck dissection for clinically apparent parotid metastatic melanoma, irrespective of neck status, were identified from two prospectively maintained databases. RESULTS: A total

The anatomical and functional characteristics of parotid fascia.

Parotid superficial and profound fascias are considered to originate from a bifurcation of the profound fascia. Its relations with the facial nerve, the continuity with platysma, temporal and zygomatic fascias suggest it is related to a superficial fascial complex rather with a profound one. The aim of this study is to add clues which sustain the theory of parotid fascia origin from superficial fascia of the face. The study was conducted on 12 cephalic extremities, which were meticulously dissected in the "Ion Iancu" Anatomy Institute of "Grigore T. Popa" University of Medicine and Pharmacy, I

Parotid gland non-Hodgkin lymphoma in primary Sjögren syndrome.

The risk of malignant non-Hodgkin lymphoma is increased in primary Sjögren syndrome. In the literature, most studies evaluating this risk were conducted in tertiary reference university hospital. So, selection bias in series exists, in particular selection of the most severe cases in tertiary reference university care centers. Some studies had also a selection bias because patients were hospitalized (data were obtained from hospital discharge registries) and therefore the more severe cases were considered. Between October 1999 and November 2009, 109 adult patients were admitted to our departme

Modified facelift incision for partial parotidectomy versus bayonet-shaped incision: a comparison using visual analog scale.

The aim of this study is to show differences between a modified facelift incision (MFI) for partial parotidectomy versus a bayonet-shaped incision (BSI). 24 patients presenting with a parotid tumor were surgically treated with a partial parotidectomy using a MFI. We generated a "matched pair control group" regarding age, tumor size and gender, who received a BSI. A questionnaire was sent to all patients and relevant data reviewed. The cosmetic satisfaction on a VAS with a MFI was 9.74 (±0.47) compared to BSI with 7.63 (±2.44, p = 0.004). The scoring in the two subgroups "visible scar" and "peo

Aggressive metastatic parotid squamous cell carcinoma with pulmonary, cerebral, and hepatic metastases: A radiologic case report.

Parotid gland malignancies are rare and represent less than 5% of all head and neck cancers. Squamous cell carcinoma of the parotid gland is an uncommon subtype and is often associated with aggressive clinical behavior. While regional lymphatic spread is common, distant metastases to organs such as the lungs, brain, and liver are rare but indicate advanced disease and poor prognosis. We report a 60-year-old male with primary right parotid squamous cell carcinoma who initially presented with facial nerve paralysis and locally advanced infiltrative disease requiring debulking parotidectomy follo

Accessory parotid gland tumours: 24 years of clinical experience.

The accessory parotid gland (APG) is salivary tissue anterior to and anatomically separate from the parotid gland. APG is a common anatomical variation, but APG tumours are extremely rare. The authors report 6 patients with APG tumours emphasizing the diagnosis, clinical features, indications and rationales for different treatment approaches. Patients with primary tumours of the parotid gland or APG tumours who underwent surgical treatment were included. APG tumours comprised 1.23% of overall parotid tumours (6/488) and had a malignancy rate of 33.3% (2/6). There were three male and three fema

Salivary gland neoplasms in Lagos, Nigeria.

This 14 year retrospective clinico-statistical analysis of 237 salivary gland neoplasms in Lagos, Nigeria, was undertaken with a view to providing further insights into the presentation of this disease in Africans. These neoplasms constituted 10.0% of all head and neck neoplasms, and were most frequently situated at the parotid gland (32.1%), the palate (24.9%) and the submandibular gland (19.4%). While parotid squamous cell carcinoma affected more males (41.2%) than females (4.7%) (P = 0.03); parotid mucoepidermoid carcinoma affected more females (53.3%) than males (11.8%) (P = 0.0149). A Rare Case of Parotid Nodular Fasciitis in a Six-Month-Old Female.

Nodular fasciitis (NF) is a rare benign self-limiting lesion that is often mistaken for malignancy due to its progressive nature. Reported cases of nodular fasciitis in the parotid gland are uncommon, and its incidence is variable among different age groups. Histopathological and immunohistochemical studies are helpful in distinguishing these kinds of lesions. We report a case of a six-month-old baby with a two-month history of progressive rapid-growing mass in the left parotid region. Clinical examination showed some mild facial nerve weakness with no other significant findings locally or sys…

Extracapsular dissection in the parapharyngeal space: benefits and potential pitfalls.

The aim of this study was to investigate the benefits and potential pitfalls of transcervical extracapsular dissection in the treatment of parotid gland tumours in the parapharyngeal space. We retrospectively evaluated the records of all patients with parapharyngeal parotid gland lesions treated between 2000 and 2015 by transcervical extracapsular dissection. Patients having revision operations and patients whose records were not complete were excluded, leaving 49 patients in the study. We found acceptable oncological and functional outcomes throughout. Special attention should be paid in case

Salivary duct cyst of accessory parotid gland in contact with the temporomandibular joint.

A 60-year-old woman presented with a painless swelling in the right preauricular region. Physical examination revealed a soft, nontender, oval-shaped lump in this location. Computed tomography indicated an anterolateral cystic lesion that was attached to the condyle of the mandible and to the accessory parotid gland. Differential diagnoses included ganglion cyst, synovial cyst of the temporomandibular joint, and accessory parotid gland cyst. Surgical removal of the cyst was performed with the patient under general anesthesia. The specimen was 18 × 20 × 18 mm in size; it contained fluid that wa

Surgery for Primary Malignant Parotid Neoplasms.

The successful treatment of salivary gland malignancies originating in the parotid gland begins with rigorous and thorough surgical management coupled with the directed and appropriate potential adjuvant use of radiation therapy and chemotherapy. The anatomic complexity of the region in relation to the facial nerve and adjoining neurovascular and musculoskeletal structures requires sound surgical planning and decision making based on preoperative and intraoperative findings. The clinical presentation of parotid malignancy is summarized, as well as the further evaluations that are performed, in

Synovial sarcoma of the parotid gland: a case report and review of the literature.

Parotid gland tumours are very heterogeneous, being benign in 80% of cases, and generally arising from epithelial cells. Nevertheless, a small group of non-epithelial tumours representing just 5% of all salivary gland neoplasms has also been reported, the most common of these being haemangioma, especially in children. However, lymphomas, neuromas, neurofibromas, lipomas and sarcomas can also be found. Synovial cell sarcoma is a high grade histological variety of sarcoma and is generally located near large joints and bursae of the lower extremities, such as knee, tendon sheaths and bursal struc

Massive crystal-storing histiocytosis associated with low-grade malignant B-cell lymphoma of MALT-type of the parotid gland.

Massive crystal deposition is unusual in lymphoproliferative disorders. In this report, a mucosa-associated lymphoid tissue (MALT) low-grade B-cell lymphoma of the parotid gland containing large numbers of crystal-storing histiocytes is described. The patient, an 81-yr-old female, presented with a history of long-standing left parotid gland enlargement. FNA cytology of the tumor showed a lymphoplasmacytic infiltrate and sheets of large benign histiocytes with abundant eosinophilic intracytoplasmic inclusions. Paraffinsection immunohistochemistry performed on the cell block demonstrated that th

[Tissue reactions with administration of piezoelectric shock waves in lithotripsy of salivary calculi].

Before clinical application of an extracorporeal piezoelectric lithotripter to treat sialolithiasis, tissue reaction during shockwave application was examined in vitro and in experiments with animals. Application of shockwaves to human tissue in vitro showed neither macroscopic nor microscopic effects. In animal experiments, the acute experiment (16 rabbits, Chinchilla-Bastard) revealed minor bleeding in the parenchyma of the parotid gland, while the chronic experiment (14 rabbits, Chinichilla-Bastard) revealed no morphologic tissue damage to the parotid region of the rabbit, as a result of pi…

Metastasis to the ciliary body and iris from a parotid carcinoma.

Adenoid cystic carcinoma (ACC) of the parotid gland is a relatively rare neoplasm, accounting for 10-15 % of all salivary gland tumors. Metastasis to the uveal region, particularly to the ciliary body and iris, is extremely uncommon, with the first case reported in 2011. This case report describes a 32-year-old woman with a history of ACC of the parotid gland. Despite radical surgery, radiotherapy, and facial nerve reconstruction, the patient developed metastasis to the ciliary body and iris one year after treatment. Ultrasonography revealed a solid iridociliary mass, and radiotherapy was admi
