The use of a pedicle probe facilitated insertion of a unit rod into the ilium of five patients with neuromuscular scoliosis. Compared to the standard technique, the pedicle probe method greatly reduced soft-tissue dissection and also provided better perception of rod alignment and cortical perforation.

We report a case of neurofibroma plexiforme of pelvis minoris in 42-year-old woman. The first diagnosis was adnexal tumor. Between 1980 and 1997, we treated 39 patients (mean age 39 (16-66) years, 24 men) with megaprosthetic replacement of a large bone defect (> 10 cm) of the pelvis. The bone resection was necessary in 38 cases due to malignant bone and soft tissue tumors and in 1 case due to hydatid disease. Polyacetal hemipelvic replacement was performed in 29 cases, CAD hemipelvic replacement in 8 cases, and a saddle prosthesis was inserted in 2 cases. Postpartum patients rarely develop complications that require radiologic evaluation. When indicated, CT can provide a rapid and reliable examination of the major pelvic organs and enable diagnosis of a multitude of postpartum complications, including abscesses, endometritis, thrombophlebitis, as well as more serious and potentially life-threatening sequelae of toxemia, DIC, and HELLP syndrome. This pictorial essay illustrates the utility of CT in the postpartum patient and demonstrates the spectrum of complications evaluated with CT.

The cases of 40 patients with osteosarcoma of the pelvis treated between 1977 and 1994 were reviewed. The location of the tumor was ilium in 30 patients, ischium in four, pubis in one, and sacrum in five. Most (58%) of the tumors were of the chondroblastic subtype. Thirty patients had surgical excision of the tumors: 10 with hemipelvectomies and 20 with limb sparing procedures. We investigated the clinicopathological features of 62 patients with transitional cell carcinoma of the renal pelvis and ureter. Four patients had been treated for bladder cancer. Among 58 patients without precedent bladder cancer, 6 had coexistent bladder cancer and bladder cancer subsequently developed in 13. The 5-year cause-specific survival rate was 33% in cases with coexistent bladder cancer, 75% in those with subsequent bladder cancer and 62% in patients without association of bladder cancer. A rare case of primary mucinous adenocarcinoma of the renal pelvis is reported. A 76-year-old woman was admitted to our hospital because of right abdominal fullness. Physical examination revealed a melon-sized (22 cm in diameter) tumor located in the middle and lower right quadrant of the abdomen. Athletic injuries to the hip and pelvis in pediatric and adolescent athletes, although uncommon, may encompass a wide spectrum of entities. A familiarity with this spectrum and a high index of suspicion in the proper clinical setting will ensure timely diagnosis and help to facilitate implementation of a proper treatment plan thereby assuring safe return to play.

Laparoscopic surgery of the male pelvis remains a risky procedure, with around 15% of perioperative complications, as compared to 0.03% in laparoscopic surgery for gynecologic and obstetric conditions. A population-based case-control study of renal pelvis and ureter cancers (502 cases, 496 controls) conducted in three areas of the United States found cigarette smoking to be associated with a 3.1-fold increase in risk, with long-term (greater than 45 years) smokers having a 7.2-fold increased risk. The clinical course and association with other urothelial carcinomas was studied in 80 patients with carcinoma of the renal pelvis. At the time of diagnosis macroscopic haematuria was the most common symptom. Urography and retrograde pyelography gave the most accurate diagnoses. During the 13 years from 1976 to 1988, 160 patients with renal pelvic and ureteral cancer were reviewed based on a new general rule for clinical and pathological studies on renal pelvic and ureteral cancer of Japanese Urological Association. We reviewed data from 62 patients with epithelial tumors of the renal pelvis and ureter who were hospitalized and treated at Nagoya University Hospital between July, 1970 and June, 1988. Of the 62 patients, 50 were males and 12 were females, the ratio being 4.2:1. The mean age was 62.1 years, the ages ranging from 41 to 87 years. We evaluated 104 osteotomies of the pelvis after an average of 18 years: 48% of patients had hip joint pain, 15% had low back pain, 41% had restricted movement, 74% had a positive Trendelenburg sign, and 77% had limb length discrepancy greater than 1 cm. We also followed the anti-Chiari effect, the influence of the osteotomy on narrowing the birth canal, and deterioration of the opposite acetabulum. We showed that the optimum indications for Chiari osteotomy are severe instability of the hip joint and an irregular femoral head.

We report two cases in which the renal stone originated from a foreign body which was used to treat a urinary tract fistula by a gynecologist. The first patient developed a vesicovaginal fistula following an emergency Caesarean operation.
