Perilymphatic fistula in children with preexisting sensorineural hearing loss.

Progression of preexisting sensorineural hearing loss (SNHL) in infants and children has been considered an indication for fistula exploration: is this approach warranted? On exploring 36 middle ears in 26 such children, we found a perilymphatic fistula (PLF) in four ears (11%). Although there was radiographic evidence of inner-ear deformity in one half of these children, a definite fistula was found in only four of 18 radiographically abnormal ears explored (22%). Even in the four patients with a history of an "event" that could implicate a fistula such as exertion or barotrauma, a fistula wa…

Predictive factors of acute sensorineural hearing loss in adult Japanese patients for clinical application by primary care doctors: a cross-sectional study.

BACKGROUND: Several methods are used for hearing loss screening; however, their benefits are uncertain. In this study, we aimed to determine … ve factors for Ménière's disease. High body mass index and ear fullness were predictive factors for perilymph fistula. Low stress level was a predictive factor for acoustic tumours. CONCLUSIONS: Our findings can be used to distinguish between the types of acute sensorineural hearing loss. Symptoms, physical status, and lifestyle factors identified during t …

Glial cell line-derived neurotrophic factor has a dose dependent influence on noise-induced hearing loss in the guinea pig cochlea.

We examined the effectiveness of glial cell line-derived neurotrophic factor (GDNF) to attenuate cochlear damage from intense noise stress. Subjects were exposed to 115 dB SPL one octave band noise centered at 4 kHz for 5 h. They received artificial perilymph with or without GDNF into the left scala tympani at 0.5 microliter/h from 4 days before noise exposure through 8 days following noise exposure. Different concentrations of GDNF (1 ng/ml, 10 ng/ml, 100 ng/ml, and 1 microgram/ml) were applied chronically directly into the guinea pig cochlea via a microcannula and osmotic pump. Noise-induced…

Intratympanic Application of Betahistine for Menière Disease-An Animal Study on Safety and Diffusion to the Inner Ear.

HYPOTHESIS: Intratympanic (IT) application of betahistine diffuses to the inner ear after 15 minutes of exposure and exhibits no ototoxicity. BACKGROUND: Management of Menière disease involves oral treatment with betahistine; however, is mitigated by a delayed bioavailability (enteral resorption; first-pass effect). Topical IT administration might achieve higher intracochlear concentration. METHODS: Animal study on wild-type rats with different IT-betahistine concentrations: 100 mg/mL, 50 mg/mL, 10 mg/mL, 1 mg/mL, and control (saline). For diffusion (n=18) perilymph was sampled postinjection…

Endolymph movement visualized with light sheet fluorescence microscopy in an acute hydrops model.

There are a variety of techniques available to investigate endolymph dynamics, primarily seeking to understand the cause of endolymphatic hy … han 2.5 μl of fluid had been injected into scala media, with no apparent movement of fluid into the perilymphatic compartments. There was no movement of endolymph into these compartments when less than 2.5 μl was injected. The remarkable uptake of the FITC-dex into the endolymphatic duct, including an absorption into the periductal channels surrounding the …

Imaging of endolymphatic and perilymphatic fluid after intravenous administration of single-dose gadodiamide.

To visualize endolymph as bright signal after intravenous injection of single-dose gadodiamide, we shortened the inversion time of heavily T(2)-weighted 3-dimensional (3D) fluid-attenuated inversion recovery (FLAIR) to 2050 ms. In 14 patients with suspected Ménière's disease, we observed high signal of vestibular endolymph in all ears, including 6 ears without vestibular endolymphatic hydrops. We observed high signal of cochlear endolymph in 17 ears with cochlear endolymphatic hydrops but not 11 ears without cochlear endolymphatic hydrops.

Contrast enhancement of the inner ear after intravenous administration of a standard or double dose of gadolinium contrast agents.

CONCLUSION: Imaging of the endolymphatic space was possible in most cases after administering a standard dose of gadolinium (Gd) by using heavily T2-weighted 3D fluid-attenuated inversion recovery (hT(2)W-3D-FLAIR) magnetic resonance imaging (MRI). OBJECTIVES: The aim of this study was to characterize the contrast enhancement in the inner ear after intravenous injection of Gd and to compare the signal intensity after a double dose or standard dose of Gd. METHODS: We injected a standard dose of gadodiamide hydrate (DTPA-BMA) or a double dose of gadoteridol (HP-DO3A) intravenously in patients

Pathology of Ménière's disease as it relates to the sac and tack procedures.

The rationale for endolymphatic sac and tack operations is to prevent the further accumulation of endolymph, the former by draining endolymph into extralabyrinthine tissues and the latter by shunting endolymph into the perilymphatic space. While the basic concepts are reasonable enough, the probability of achieving these objectives seems remote. There appears to be adequate clinical evidence, however, to show that these procedures are of therapeutic value in selected cases. It is tempting to speculate that these surgical insults to the labyrinth, with the associated inflammatory and biochemica…

X-linked mixed deafness with congenital fixation of the stapedial footplate and perilymphatic gusher.

An X-linked syndrome characterized in males by profound mixed deafness, vestibular abnormalities, congenital fixation of the stapes and perilymphatic otorrhea on attempted stapedectomy has been documented in a large Caucasian kindred. Female heterozygotes have similar but milder audiologic abnormalities. The syndrome of congenital fixation of the stapes with perilymphatic gusher may be a relatively common form of X-linked deafness and is an important clinical entity because affected males may be significantly benefited by sound amplification.

Study of protective effect on rat cochlear spiral ganglion after blast exposure by adenovirus-mediated human β-nerve growth factor gene.

OBJECTIVE: To study whether adenovirus-mediated human β-nerve growth factor (Ad-hNGFβ) gene has any protective effect on rat cochlear spiral ganglion after blast exposure. METHODS: Deafness was induced by blast exposure (172.0 dB) in 20 healthy rats. Seven days after blast exposure, Ad-hNGFβ was infused into the perilymphatic space of 10 animals as the hNGFβ/blast group, and artificial perilymph fluid (APF) was infused into the perilymphatic space of 10 animals as the APF/blast control group. An additional control group consisted of 10 healthy rats which received Ad-hNGFβ target gene with no …

Traumatic perilymphatic fistula with the luxation of the stapes into the vestibule.

Traumatic perilymphatic fistula due to luxation of the stapes into the vestibule caused by an earpick is an extremely rare situation. In order to treat such an injury, it is necessary to evaluate the extent of the damage and the actual condition of the middle and inner ear. However, it is difficult to obtain such precise information about the condition of the stapes prior to performing surgery. We report on a case of a traumatic perilymphatic fistula with luxation of the stapes into the vestibule that was diagnosed using multislice CT (MSCT). MSCT clearly demonstrated the presence of air in th…

[Spontaneous perforation of the membrane of the round window--a major cause of sudden deafness?].

In 1983, 45 out of 63 clinically treated patients with sudden hearing loss underwent an exploratory tympanotomy. In 27 cases (60%) a perforation of the round window membrane was found, whereas in 18 cases (40%) the round window membrane was intact and there was no perilymph fistula. From 1982 to the end of 1983 30 perforations of the round window membrane were found which are reported here, most of them being spontaneous perforations without any preceding trauma. Audiometrically, hearing losses were found ranging from medium to high degree. These included all types of hard hearing, such as los…

Perilymphatic fistula and its interest to audiologists.

Forty-one patients who had sudden hearing loss associated with spontaneous perilymphatic fistllas were reviewed in this investigation. A history of physical exertion or an upper respiratory infection prior to the onset of hearing loss strongly suggests the presence of spontaneous perilymphatic fistula. However, absence of such a history does not necessarily preclude the possibility of membrane rupture. A great variation in audiologic and vestibular findings was observed. Tinnitus was present for most patients and vertigo or disequilibrium was reported by some. Findings support the need for sur…

Chaotic Dynamics Enhance the Sensitivity of Inner Ear Hair Cells.

Hair cells of the auditory and vestibular systems are capable of detecting sounds that induce sub-nanometer vibrations of the hair bundle, below the stochastic noise levels of the surrounding fluid. Furthermore, the auditory system exhibits a highly rapid response time, in the sub-millisecond regime. We propose that chaotic dynamics enhance the sensitivity and temporal resolution of the hair bundle response, and we provide experimental and theoretical evidence for this effect. We use the Kolmogorov entropy to measure the degree of chaos in the system and the transfer entropy to quantify the am
