Human Implementation of Upper Extremity Amputation Incorporating Agonist-Antagonist Myoneural Interface Construction.

BACKGROUND: The agonist-antagonist myoneural interface (AMI) is a mechanoneural construct in which naturally opposed innervated muscles are … -three percent of patients reported resolution of preoperative limb pain and experienced functional phantom limb perception. CONCLUSION: The incorporation of AMI construction into upper extremity amputation procedures may provide benefits similar to those observed in patients with lower extremity amputations who have undergone equivalent AMI interventions.

Neuropathic Pain after Major Limb Amputation: A Cross-Sectional Study.

BACKGROUND: Residual limb pain (RLP) and phantom pain can arise following amputation and may require additional treatment or surgery. This study aimed to determine the prevalence of neuropathic RLP following limb amputation and identify prognostic factors for the development of neuropathic RLP. METHODS: A cross-sectional study was performed of patients who underwent upper or lower extremity amputation between 1990 and 2021 with a minimum of 12 months' follow-up. The primary outcome was the prevalence of neuropathic RLP, defined as a 0 to 10 numeric rating analog scale pain score in the residu

Surviving life-threatening illness: keys to optimal nursing care.

Necrotizing fasciitis (NF) is a rare yet life-altering infection. Disfigurement and death ensue without accurate diagnosis and treatment. Educational awareness can assist nurses to advocate for differential diagnosis, immediate treatment, and plan of care. This article examines the experience of a mother with NF, application of Neuman's Systems Model (NSM) to address her healing needs, and nursing practice guided by one's personal worldview. NSM was chosen because it is holistic and concise and can be utilized from a Christian perspective.

Controlling acute pain to improve the quality of postoperative pain management: an update from the European Society of Regional Anesthesia meeting held in Maastricht (September 2016).

Improvement in postoperative pain management remains a global concern with a significant unmet need for patients. This was the focus of a 'Change Pain' session at the 35th European Society of Regional Anesthesia meeting (Maastricht, September 2016). Awareness of the size and nature of the problem is important to improve postoperative pain management strategies. Optimal treatment of acute pain should aim to avoid long-term sequelae such as the development of chronic pain disorders (e.g., phantom limb pain). Cases highlighting unsuccessful and successful strategies to manage individuals undergoi…

Postsurgical pathologies associated with intradural electrical stimulation in the central nervous system: design implications for a new clinical device.

Spinal cord stimulation has been utilized for decades in the treatment of numerous conditions such as failed back surgery and phantom limb syndromes, arachnoiditis, cancer pain, and others. The placement of the stimulating electrode array was originally subdural but, to minimize surgical complexity and reduce the risk of certain postsurgical complications, it became exclusively epidural eventually. Here we review the relevant clinical and experimental pathologic findings, including spinal cord compression, infection, hematoma formation, cerebrospinal fluid leakage, chronic fibrosis, and stimul…

Stable cortical body maps before and after arm amputation.

The adult brain's capacity for cortical reorganization remains debated. Using longitudinal neuroimaging in three adults, followed before and up to 5 years after arm amputation, we compared cortical activity elicited by movement of the hand (before amputation) versus phantom hand (after amputation) and lips (before and after amputation). We observed stable cortical representations of both hand and lips in primary sensorimotor regions. By directly quantifying activity changes across amputation, we demonstrate that amputation does not trigger large-scale cortical reorganization.

Phantom somatosensory evoked potentials following selective intraneural electrical stimulation in two amputees.

OBJECTIVE: The aim of the paper is to objectively demonstrate that amputees implanted with intraneural interfaces are truly able to feel a sensation in the phantom hand by recording "phantom" somatosensory evoked potentials from the corresponding brain areas. METHODS: We implanted four transverse intrafascicular multichannel electrodes, available with percutaneous connections to a multichannel electrical stimulator, in the median and ulnar nerves of two left trans-radial amputees. Two channels of the implants that were able to elicit sensations during intraneural nerve stimulation were chosen

Reorganization of the somatosensory cortex after amputation of the index finger.

Cortical reorganization occurs within the primary somatosensory and the primary motor cortex after amputation of the arm or forearm. Here we report on a patient showing cortical reorganization after amputation of his right index finger. Our findings indicate that the neural networks within the area of the amputated finger in the somatosensory cortex (SI) were invaded by neighbouring structures, i.e. of neural cell assemblies that subserve the thumb and middle finger of his right hand.

Development of a rodent high-energy blast injury model for investigating conditions associated with traumatic amputations.

AIMS: In recent conflicts, most injuries to the limbs are due to blasts resulting in a large number of lower limb amputations. These lead to heterotopic ossification (HO), phantom limb pain (PLP), and functional deficit. The mechanism of blast loading produces a combined fracture and amputation. Therefore, to study these conditions, in vivo models that replicate this combined effect are required. The aim of this study is to develop a preclinical model of blast-induced lower limb amputation. METHODS: Cadaveric Sprague-Dawley rats' left hindlimbs were exposed to blast waves of 7 to 13 bar burst…

Altered cortical thickness and structural covariance networks in upper limb amputees: A graph theoretical analysis.

BACKGROUND: The extensive functional and structural remodeling that occurs in the brain after amputation often results in phantom limb pain (PLP). These closely related phenomena are still not fully understood. METHODS: Using magnetic resonance imaging (MRI) and graph theoretical analysis (GTA), we explored how alterations in brain cortical thickness (CTh) and structural covariance networks (SCNs) in upper limb amputees (ULAs) relate to PLP. In all, 45 ULAs and 45 healthy controls (HCs) underwent structural MRI. Regional network properties, including nodal degree, betweenness centrality (BC),…

Targeted Peripheral Nerve Interface: Case Report with Literature Review.

Nerve transection injuries can result in painful neuromas that adversely affect patient recovery. This is especially significant following amputation surgeries in the setting of prosthetic wear and function. Targeted Muscle Reinnervation and Regenerative Peripheral Nerve Interface (RPNI) are 2 modern surgical techniques that provide neuromuscular targets for these transected nerve endings to reinnervate. These strategies have been previously shown to reduce phantom limb pain, residual limb pain, and neuroma-related pain.1,2,7,11 Two recent articles described technical adaptations of combining …
