Ambiguous genitalia in the newborn.

Ambiguous genitalia in the newborn need immediate and rational management. This complex situation requires a strategy of clinical, hormonal, genetic, molecular, and radiographic investigation to determine the etiology of the intersex state and orient the therapeutic approach. Physical examination is key to diagnosis. Careful palpation to locate gonads at the genital folds or in the inguinal region provides the first element for diagnostic orientation. If gonads are absent, a diagnosis of female pseudohermaphroditism seems advisable; if gonads are palpated, a diagnosis of male pseudohermaphrodi

Molecular dynamics simulations of positively selected codons in FcγRI reveal novel biochemical binding properties.

FcγRI is a high-affinity receptor for IgG, associated with autoimmune disease pathology and determines clinical responses to antibody-based immunotherapies. FcγRI has a complex evolutionary history that is not fully understood, and to address this we explored signatures of positive selection in the receptor's functional gene, FCGR1A, using codon-based selection tests on aligned 1-1 orthologous sequences from placental mammals (n = 32). Signatures of positive selection have occurred at several locations within the gene, with two sites (H148 (M2a ω 0.997 & M8 ω = 0.993)) and (W149 (M2a ω = 0.999

Perinatal exposure to delta-9-tetrahydrocannabinol (THC) alters goal-directed behavior and dopamine functioning in wistar rats.

RATIONALE: Cannabis use during 2021pregnancy is common as many pregnant women consider cannabis as a safe way to alleviate symptoms associated with pregnancy because it is "natural". However, clinical evidence links perinatal cannabis exposure to externalizing behavior in offspring. In preclinical research, most studies focus on exposure to the psychoactive constituent of cannabis, delta-9-tetrahydrocannabinol (THC). THC is lipophilic, allowing it to cross the placental barrier and be secreted in maternal milk, thereby exposing the fetus/neonate. OBJECTIVE: We used operant procedures to measu

Association between isolated maternal hypothyroxinemia during the first trimester and adverse pregnancy outcomes in Southern Chinese women: a retrospective study of 7051 cases.

BACKGROUND: The association between isolated maternal hypothyroxinemia (IMH) and adverse pregnancy outcomes is still controversial. This study aimed to evaluate the association between IMH during the first trimester and adverse pregnancy outcomes in southern Chinese women. METHODS: This was a hospital-based, retrospective cohort study. The records of 7051 women, including 1337 IMH women and 5714 euthyroid women who had a singleton pregnancy and accepted routine prenatal service at the Third Affiliated Hospital of Sun Yat-Sen University from January 2015 to September 2018, were extracted from

Septic arthritis of the shoulder due to Ureaplasma urealyticum after emergency caesarean section: a case report.

BACKGROUND: Ureaplasma urealyticum is an intra-cellular bacterium frequently found colonizing the genital tract. Known complications include localized infections, which can result in premature deliveries. Septic arthritis due to U. urealyticum in healthy patients is exceptionally rare, although opportunistic septic arthritis in agammaglobulinemic patients have been reported. However, there are no reports of septic arthritis due to U. urealyticum following caesarean section or in the post-partum period. CASE PRESENTATION: A 38-year-old immunocompetent woman presented with severe right shoulder

Assessment of the Values of Endoglin and Soluble Fms-Like Tyrosine Kinase-1/Placental Growth Factor Ratio Among Women at Risk for Pre-eclampsia: A Cross-Sectional Study.

BACKGROUND: Preeclampsia (PE) is a common complication of pregnancy affecting pregnant women and newborns. PE causes multi-organ disorders and remains one of the main reasons for maternal morbidity and mortality. In addition, PE leads to many complications that can occur in the fetus or newborn. OBJECTIVES: This study aimed to evaluate the serum endoglin and soluble fms-like tyrosine kinase 1 (sFlt-1)/placental growth factor (PlGF) ratio in pregnant women at high risk for PE and normal pregnant women and to assess the sensitivity and specificity of endoglin and SFlt-1/PlGF ratio. Increased first-trimester CA125 levels associated with the development of preeclampsia.

BACKGROUND: Preeclampsia (PE) remains a leading cause of maternal and fetal mortality, necessitating reliable first-trimester biomarkers for early identification and pathogenesis elucidation. This study aimed to evaluate the predictive performance of first-trimester serum CA125 for the PE development and to examine their longitudinal associations with blood pressure trajectories during pregnancy. METHODS: A nested case-control study was conducted within the prospective BiCoS cohort, including 70 PE cases and 70 matched controls. Serum CA125 levels were measured at 12.26 ± 0.44 weeks of gestat

Maternal biomarkers of endothelial dysfunction and pregnancy outcomes in women with and without HIV in Botswana.

BACKGROUND: Women living with HIV-1 (WLHIV) are at higher risk of having an adverse birth outcome, but the underlying mechanism(s) are unknown. We hypothesized that HIV-associated endothelial activation could adversely impact placental function and lead to impaired fetal growth or stillbirth. METHODS: We used stored samples from WLHIV and HIV-negative women who had enrolled during pregnancy in the observational Botswana Tshipidi cohort. Written informed consent was obtained from the participants. We measured plasma levels of markers of endothelial activation (soluble vascular adhesion molecul

Stillbirth in women with diabetes: a retrospective analysis of fetal autopsy reports.

INTRODUCTION: Diabetes in pregnancy is associated with an increased rate of stillbirth. There are a wide variety of factors that have been implicated including placental insufficiency, hypoxia, hyperinsulinemia and impaired cardiac function. Furthermore, there is evidence that diabetic pregnancies have an increased rate of fetal cardiomyopathy as compared to non-diabetic pregnancies. Prior studies have indicated that this association can also be an etiology for diabetic stillbirth. The purpose of this study was to determine if diabetic pregnancies have an increased risk of fetal cardiomyopathy

The Impact of Hydroxychloroquine on Primary Feto-Placental Endothelial Cells from Healthy and Early-Onset Preeclamptic Placentas.

Hydroxychloroquine (HCQ), an anti-malarial drug, is suggested as a promising candidate for the treatment of pregnancy-related disorders associated with endothelial activation, among which there is preeclampsia (PE). Arterial feto-placental endothelial cells (fpECAs) were isolated from control (CTR) and early-onset preeclamptic (EO-PE) placentas. The aim of this study was to test potential protective effects of HCQ in an in vitro model of endothelial activation as well as in cells isolated from EO-PE placentas. To mimic PE conditions, CTR fpECAs were exposed to a pro-inflammatory environment co

Characterization of postnatal brain injury and behavioural deficits in a rat model of placental insufficiency-induced fetal growth restriction.

Fetal growth restriction (FGR) is a significant pregnancy complication strongly associated with neurodevelopmental deficits in infants. Effective treatments to support brain development are currently lacking. Detailed characterization of structural and functional brain abnormalities caused by FGR can aid the development of targeted therapies. This study aimed to comprehensively characterise brain development at anatomical, cellular and functional levels in a rat model of placental-insufficiency-induced FGR, using the reduced uterine perfusion pressure (RUPP) method. Pregnant Sprague-Dawley rat

Mid-pregnancy uterine artery Dopplers and child neurodevelopment.

OBJECTIVE: Resistance to blood flow in the maternal uterine artery (UtA) has been associated with both preeclampsia (PE) and small-for-gestational age (SGA). Our objective was to assess the association of increased maternal UtA resistance with child neurodevelopmental and behavioral outcomes in low-risk pregnancies. STUDY DESIGN: Serial ultrasound exams were prospectively performed in 2334 racial and ethnically diverse, healthy patients with low-risk singleton pregnancies (July 2009 to Jan 2013) as part of the NICHD-Fetal Growth Studies. Maternal UtA Dopplers were performed between 18w0d and

A study of the association between the early pregnancy thrombosis-thyroid axis and pregnancy outcomes in patients with recurrent spontaneous abortion.

BACKGROUND: Recurrent spontaneous abortion (RSA) is a severe, yet common, pregnancy complication. Thrombophilia and thyroid dysfunction are critical factors altering placental microcirculation and embryonic development, although the synergistic predictive value of these two conditions for RSA pregnancy outcomes remains undetermined. OBJECTIVE: To investigate the factors influencing pregnancy outcomes in patients with RSA; analyze the association between early pregnancy markers (including coagulation and fibrinolysis, platelet function, and thyroid function) and pregnancy outcomes in patients

Exposure to ambient air pollution in the first 1000 days of life and alterations in the DNA methylome and telomere length in children: A systematic review.

BACKGROUND: Exposure to air pollution during the first 1000 days of life (from conception to the 2nd year of life) might be of particular relevance for long-term child health. Changes in molecular markers such as DNA methylation and telomere length could underlie the association between air pollution exposure and pollution-related diseases as well as serve as biomarkers for past exposure. The objective of this systematic review was to assess the association between air pollution exposure during pregnancy and the first two years of life and changes in DNA methylation or telomere length in child

Influence of a combination of two tetrachlorobiphenyl congeners (PCB 47; PCB 77) on thyroid status, choline acetyltransferase (ChAT) activity, and short- and long-term memory in 30-day-old Sprague-Dawley rats.

The important role of thyroid hormones in growth and development, maintenance of body temperature, digestion, cardiac function, and normal brain development can be disrupted by environmental contaminants like polychlorinated biphenyls (PCB). Polychlorinated biphenyls are environmental contaminants that are widespread, persistent, lipophilic, and bioaccumulate through food webs, concentrating in adipose tissue. Placental and lactational PCB exposure of offspring causes metabolic and endocrine disruptions including hypothyroxinemia, spatial learning and memory deficits, neurochemical and neurobe

Use of tissue-specific promoters in the regulation of aromatase cytochrome P450 gene expression in human testicular and ovarian sex cord tumors, as well as in normal fetal and adult gonads.

We have previously demonstrated that the tissue-specific regulation of human aromatase cytochrome P450 (P450arom) gene expression is, in part, the consequence of the use of tissue-specific promoters. Promoter I.1 (PI.1) and PI.2-specific transcripts are expressed in the placenta, whereas promoter II (PII) appears to be the only active promoter in the corpus luteum. Testicular and ovarian sex cord tumors with annular tubules (SCTATs) associated with gynecomastia in prepubertal boys and isosexual precocity in girls with Peutz-Jeghers syndrome (P-JS) have been previously reported. In the present

The impact of histological chorioamnionitis and funisitis on breathing effort of premature infants receiving physiological based cord clamping: a retrospective analysis.

RATIONALE: In premature infants, physiological-based cord clamping (PBCC; clamping after lung aeration), may allow a smoother cardiopulmonary transition than time-based cord clamping (TBCC; clamping after 30-60 s). However, in infants affected by antenatal inflammation of the fetal membranes (histological chorioamnionitis; HCA) and umbilical vessels (funisitis/funiculitis/fetal response; FUN), PBCC could prolong exposure to inflammatory mediators that suppress breathing. We therefore investigated how HCA ± FUN affects breathing effort of PBCC infants. METHODS: Infants born <30 weeks' gestatio

Biallelic ATG9B Variants Define a Novel Autophagy-Related Neurodevelopmental Disorder with Cerebellar Ataxia.

BACKGROUND/OBJECTIVES: Autophagy is a highly conserved eukaryotic cellular process whose dysfunction results in human pathologies including cancer and neurodegenerative disease. First identified in yeast, ATG genes are central players in autophagy. Mutations in core autophagy genes ATG5 and ATG7 have been previously reported to cause rare genetic disorders with autosomal recessive inheritance. METHODS: Here we report, for the first time, variants in human ATG9B gene as causative factors for a rare neurodevelopmental disease with autosomal recessive inheritance. Three distinct mutations were d

Single-nucleotide polymorphisms in dizygotic twin ovine fetuses are associated with discordant responses to antenatal steroid therapy.

BACKGROUND: Antenatal steroid (ANS) therapy is given to women at risk of preterm delivery to accelerate fetal lung maturation. However, the benefit of ANS therapy is variable and how maternal and fetal factors contribute to this observed variability is unknown. We aimed to test the degree of concordance in preterm lung function, and correlate this with genomic, transcriptomic, and pharmacokinetic variables in preterm dizygotic twin ovine fetuses. METHODS: Thirty-one date-mated ewes carrying twin fetuses at 123 ± 1 days' gestation received maternal intramuscular injections of either (i) 1 × 0.

Delivery of small-for-gestational-age neonate and association with early-onset impaired maternal endothelial function.

OBJECTIVE: Women who have delivered a small-for-gestational-age (SGA) infant are at an increased risk of developing cardiovascular disease (CVD) in later life. Endothelial dysfunction is a subclinical sign of early CVD. It is unknown whether women who have recently had a pregnancy complicated by SGA, in the absence of other maternal and fetal diseases, have subclinical endothelial dysfunction. Our aim was to assess maternal endothelial function 6 months after a pregnancy complicated by SGA. METHODS: This was a case-control study conducted in a tertiary referral hospital in London, UK, over a
