The response of plasma aldosterone and cortisol concentrations to acute volume depletion was studied in 18 chronically anephric subjects and four recently nephrectomized subjects. Volume-depleting hemodialysis and hemodialysis without volume depletion produced insignificant changes in plasma aldosterone concentrations in chronically anephric subjects. Failure of volume depletion to increase plasma aldosterone concentrations in these subjects could not be attributed to reductions in plasma potassium concentrations and was in marked contrast to the effect on plasma cortisol concentrations, which

We aimed to compare different fluids indicated in volume replacement in multiple trauma patients, enlightening the indications, mechanisms of action and side effects. An extensive review of references (indexed journals) between 1997 and 2008 was performed. There is not yet a consensus about which fluids should be used in trauma patients. The systematic reviews available did not show a benefit of colloid solutions over crystalloid fluids. Crystalloids intensify physiological internal dilution, furthered by water migration from interstitial and intracellular spaces into intravascular space due t

BACKGROUND: Crystalloids are commonly used as priming solutions during cardiopulmonary bypass (CPB). Consequently, hemodilution is a regular occurrence at the start of a CPB. This study describes the time-course variations of hemodynamic parameters, plasma volume (PV) and fluid exchange following crystalloid hemodilution at start of normothermic CPB.
METHODS: Forty-five anesthetized piglets were given 60-min normothermic CPB. Ringer's solution was used as priming solution and maintenance fluid. Fluid input/losses, PV, colloid osmotic pressures (plasma/interstitium), hematocrit, and s-proteins…

PURPOSE: Fluid overload is considered a main risk factor for exercise-associated hyponatremia (EAH). The aim of this study was to investigate the incidence of EAH in ultra-runners at the 100 km ultra-run in Biel, Switzerland.
METHODS: Pre- and postrace, body mass, urinary specific gravity, hemoglobin, hematocrit, plasma [Na+], and plasma volume were determined.
RESULTS: Of the 145 finishers, seven runners (4.8%) developed asymptomatic EAH. While running, the athletes consumed a total of (median and interquartile ranges) 6.9 (5.1-8.8) L over the 100 km distance, equal to 0.58 (0.41-0.79) L/h.…

Total body water (TBW) undergoes substantial physiological expansion during pregnancy, reflecting coordinated cardiovascular, renal, and end … used to assess body-water distribution in pregnant populations. We review the mechanisms underlying plasma volume expansion, interstitial fluid accumulation, and tissue hydration, and discuss their implications for fetal growth, hypertensive disorders of pregnancy, and gestational diabetes mellitus. We also examine the principles, strengths, and limitations of …

Endothelial glycocalyx degradation induced by fluid overload adds to the concern of a detrimental effect of uncontrolled fluid resuscitation and the risk of unnecessary fluid infusion. As a consequence, the use of new tools for monitoring response to fluids appears promising. From that perspective, the monitoring of plasma concentration of glycocalyx degradation markers could be useful.

Emergency fluid resuscitation of hypovolemic patients begins with an accurate assessment of the degree of volume depletion as well as identification of the cause and associated abnormalities. On the basis of this information, the proper resuscitative fluid can be chosen and administered by the appropriate route, as guided by the urgency of the situation. Patients with severe volume depletion and those in shock require intravenous fluids. In other situations, oral rehydration is often appropriate. Specific fluids then can be tailored to the individual patient's needs and adjusted as warranted b

In hypercapnic chronic respiratory failure red cell volume correlates with the arterial oxygen tension during the quiescent phase of the disease. During an acute exacerbation measurement of the red cell volume can give a reasonable index of the arterial oxygen tension existing in the previous quiescent state. This level of arterial oxygen tension appears to determine the tolerance of the individual to treatment with oxygen.

OBJECTIVE: To compare a solution of 3% dextran-60 (D60) in Ringer's lactate (RL) with RL alone as maintenance fluids for abdominal aortic su … intravascular albumin decreased from 0.7 g/kg (1.4 to 0.7 g/kg) in both groups, corresponding to a plasma volume (PV) loss of 13 mg/kg without fluid infusions. A total intravascular dextran of 0.5 g/kg resulted in a PV expansion at 1 hr of 4.4 mL/kg above preoperative level, in sharp contrast to 7.0 mL/kg decrease in PV with RL. Of the intraoperative 3% D60 an …
