Alcohol and the central nervous system.

Ethanol use is common to most cultures but with varying doses and to varying extents. While research has focused on the effects on the liver, alcohol exerts a range of actions on the function and structure of the nervous system. In the central nervous system (CNS) it can provoke or exacerbate neurological and psychiatric disease; its effects on the peripheral nervous system are not included in this review. Sustained alcohol intake can predispose to acute neurochemical changes which, with continued ingestion and incomplete treatment, can lead to chronic structural changes in the CNS: these incl

Digital gender violence and family honor coercion: cyberbullying and digital blackmail among Palestinian women.

BACKGROUND: Cyberbullying and digital blackmail (ibtizaz) have emerged as serious threats to psychological well-being, especially for women in conservative and honor-based societies. In Palestine, these digital abuses intersect with patriarchal norms, cultural stigma, and the broader context of political occupation. While global research addresses cyberbullying's psychological impact, there remains limited empirical insight into how these phenomena manifest in occupied Palestinian territories and affect women subject to collective codes of honor, shame, and silence. AIMS: This study aims to f

Deep Eutectic Solvents in Polymeric Drug Carriers: Insights into Release Behavior and Functional Integration.

Incorporating deep eutectic solvents (DES) into polymer-based drug delivery systems (DDS) presents a novel approach to addressing persistent pharmaceutical issues, including low solubility, restricted bioavailability, and unregulated drug release. As environmentally friendly, adjustable, and biocompatible alternatives to traditional organic solvents and ionic liquids, DES possess distinctive physicochemical characteristics-such as strong hydrogen-bonding ability, low volatility, and structural flexibility-that make them effective as functional excipients. This review consolidates a contemporar

Why cities cannot learn.

For most of the world's urban history, changes in policies and responses to hazards have proceeded on a timescale that made adaptation possible, however slow or politically contested. The Great Fire of London in 1666 produced building regulations within 2 years of the devastating blaze. Repeated flooding events along the Rhine and Danube Rivers through the late 20th century spurred zoning rules and hazard mapping that are now standard across Europe's urban planning systems. What drove these improvements was a general pathway in which disruption produced evidence, evidence generated pressure, a

Inflammation Perfect Storm: Cancer, Climate and Polarisation.

Chronic systemic inflammation is increasingly recognised as a central pathological state driving the global burden of non-communicable diseases, most notably cancer, and significantly exacerbated by environmental stressors linked to climate change. This review elucidates the intricate and synergistic relationship between chronic inflammation and cancer, further amplified by the compounding effects of climate change and contemporary sociopolitical polarisation. We examine how biological, environmental and psychosocial factors converge to worsen chronic inflammation and increase cancer risk. Add

Do Earmarks Target Low-Income and Minority Communities? Evidence from US Drinking Water.

The quality and inequality of US drinking water investments have gained attention after recent environmental disasters in Flint, Michigan, and elsewhere. We compare the formula-based targeting of subsidized loans provided under the Safe Drinking Water Act with the targeting of congressional drinking water earmarks ("pork barrel" spending). Earmarks are often critiqued for potentially privileging wealthier and more politically connected communities. We find that earmarks target Black, Hispanic, and low-income communities, partly due to targeting water systems serving large populations. Earmark

Political Rivalry During America's Founding Fathers Era and the Bullet that Ended Up in Alexander Hamilton's Upper Lumbar Spine.

On the morning of July 11, 1804, Major General Alexander Hamilton, an American statesman and one of the Founding Fathers of the United States, boarded a barge from Manhattan to the New Jersey shore. There, he met his personal and political opponent, Aaron Burr, for a duel during which Burr shot a bullet that entered Hamilton's torso, fractured a false rib, passed through internal organs, and lodged itself in one of his upper lumbar vertebrae. In this unique historical analysis of a pivotal event in American history, we provide a comprehensive discussion of the spinal injuries sustained by Alex

From inoculation to vaccination: the fight against smallpox in Siena in the 18th and 19th centuries.

Smallpox is a contagious viral disease. In the fight against smallpox, stimulation of the immune system by means of inoculation of human smallpox and subsequent vaccination constituted a very important step forward in the history of medicine. First reported in ancient Greece and in the Egypt of the Pharaohs, smallpox reappeared in the middle of the 16th century, becoming the leading endemic disease in the following century and periodically causing hundreds of thousands of deaths. In the 18th and 19th centuries, Europe was afflicted by numerous epidemics. While their consequences in large urban

The fabbrica della penicillina in postwar Italy: an institutionalist approach.

This paper focuses on the motives and long-term effects of the momentous decision to build a world-class biomedical research laboratory, the International Center for Chemical Microbiology, at the Istituto Superiore di Sanità in Rome, rather than develop domestic production of penicillin to meet the needs of a destitute postwar Italy. An institutionalist approach will provide a richer vision of the intersections of scientific and national political history in postwar Italy and the Cold War. The Center failed in its modernising mission due to an insular mentality producing an 'enclosure effect'

Cancer medicines: a private vice for public benefit?

Cancer medicines have become one of the most dominant global medical technologies. They generate huge profits for the biopharmaceutical industry as well as fuel the research and advocacy activities of public funders, patient organisations, clinical and scientific communities and entire federal political ecosystems. The mismatch between the price, affordability and value of many cancer medicines and global need has generated significant policy debate, yet we see little change in behaviours from any of the major actors from public research funders through to regulatory authorities. In this polic

Neurotuberculosis: A Clinical Review.

In September 2018, the United Nations General Assembly held the first ever meeting to discuss the global epidemic of tuberculosis (TB) and adopted a political declaration titled "United to end tuberculosis: an urgent global response to a global epidemic." The timing of the meeting was prescient but overdue since Mycobacterium tuberculosis surpassed the human immunodeficiency virus as the world's leading infectious killer in 2014. Infection of the central nervous system by Mycobacterium tuberculosis, herein referred to as neurotuberculosis, is the most feared and dangerous form of tuberculosis,

Telehealth: the backbone of healthcare financing.

Malaysia, like many other countries faces major challenges in meeting increasing demands with limited resources. Changes in demography, life-style diseases, increasing consumer expectations, new medical technologies and rapid economic growth all fuel demand for more healthcare services. There are problems related to the distribution and delivery of healthcare services, and there is inadequate integration of healthcare delivery and continuity of care is a major concern. Resources tend to be concentrated in the very expensive hospital sector, although services would be cost-effectively and conve

New developments in contraceptive drugs for use by women.

Since the oral contraceptive pill was first marketed in 1959, advances in contraceptive drugs for women have been limited to variations on the theme of oestrogen in combination with progestogen or progestogen alone. Alterations to the pill regimen, new progestogens and new delivery systems have increased choice but while these advances significantly improve acceptability, side effects and risks remain essentially unchanged. New ideas are in short supply. Antiprogesterones offer a new approach with a variety of dose regimens but their development has been seriously hampered by the politics asso

Post-traumatic Stress Disorder in Prisoners' Offspring: A Systematic Review and Meta-analysis.

BACKGROUND: Parental incarceration can produce serious effects on the offspring's mental health. The presence of Post-Traumatic Stress Disorder (PTSD) in prisoners' offspring is understudied and the few literature data showed heterogeneous evidence, with some studies suggesting that about 25% of prisoners' offspring have PTSD and other reporting much lower prevalence rates around 2-3%. There is no systematic review and meta-analysis about PTSD in prisoners' offspring. OBJECTIVES: The present systematic review and meta-analysis aimed to provide a first quantitative synthesis of the prevalence

Structural determinants of HIV inequities in South Africa: Policy analysis of the national strategic plan for HIV 2023-2028.

BACKGROUND: South Africa continues to experience the world's highest HIV burden despite major progress in antiretroviral therapy (ART) scale-up. Persistent disparities across gender, geography, and socioeconomic status reveal that structural and political inequities shape vulnerability, access, and outcomes. This paper examines inequalities in HIV outcomes and evaluates how social, economic, and political structures influence policy effectiveness despite expanded ART coverage. METHODS: A document-based policy analysis was conducted using Walt and Gilson's Policy Triangle framework. National s

Opportunities and Challenges for Promoting Sustainable Diets within "Food is Medicine" Programs in the United States: A Qualitative Research Study.

BACKGROUND: The United States population is experiencing concurrent, interrelated challenges of diet-related chronic diseases, food and nutrition insecurity, and climate change. Food is Medicine (FIM) has been described as a range of food-based programs that aim to prevent, manage, and treat diet-related chronic diseases in partnership with health care systems. Research is lacking in how these programs could promote plant-rich dietary patterns and other sustainability practices to support both human and planetary health. OBJECTIVES: This study explored how FIM experts in the United States def

Developing an Accreditation Framework for Postgraduate Medical Speciality Education in Low-Resource Settings: A Qualitative Study from Sudan.

INTRODUCTION: Postgraduate medical education (PGME) is essential for producing competent specialists and strengthening health systems. Although Sudan has an established undergraduate accreditation system through the Sudan Medical Council (SMC), PGME accreditation under the Sudan Medical Specialization Board (SMSB) remains fragmented and inconsistently aligned with international standards. This study assessed the status of PGME accreditation in Sudan, examined the SMSB experience, and developed a framework to strengthen accreditation practices. METHODS: A qualitative case study design was empl

From Policy to Practice: Brazil's Pathway to Eliminating Vertical Transmission of HIV as a Public Health Milestone.

INTRODUCTION: The elimination of vertical transmission of HIV is a global health priority endorsed by the Pan American Health Organization (PAHO) and the World Health Organization (WHO), aligned with the Sustainable Development Goals (SDGs). Brazil has made substantial progress towards this goal through sustained investment in universal health policies and a rights-based approach to healthcare. DISCUSSION: Brazil's national HIV response is anchored in the Unified Health System (Sistema Único de Saúde-SUS), a universal, decentralized and participatory health system that ensures access to preve
