Wildlife, people and development: veterinary contributions to wildlife health and resource management in Africa.

Human population pressures, habitat loss, environmental degradation and illegal hunting in Africa have resulted in the loss of biodiversity and near extinction of certain wildlife species. The dilemma for Africa is the balancing of conservation and development. If wildlife is not to become a relic of the past then it must have more than just aesthetic value. It must contribute materially to the well being of people who live close to the resource. In fact, appropriate management of biodiversity would lay the foundations for a more positive future for the rural people of Africa, with the key bei

A Nurse-Run Public Health Clinic for Individuals Utilizing a Community Outreach Center.

This article describes a nurse-run clinic for low-income individuals or individuals experiencing homelessness. We conducted a quality improvement project using data from 111 individuals over 194 visits. Thirty-seven percent of patients had difficulty managing their health and 22% reported being in an active health crisis. The visits resulted in patients leaving with their visit goal being met (86%), and 96% of nonemergent cases being diverted from the emergency room. The nurse-run clinic provided access to care and served as a safety net for a vulnerable population, providing support for this

The aging hemophilia patient.

Advances in the development of safe and effective treatments for hemophilia over the past century have significantly increased the life expectancy of persons with hemophilia (PwH). As a result, clinicians are now encountering age-related comorbidities that were previously uncommon in hemophilia care, including cardiovascular disease, malignancy, and renal disease. Managing this aging population of PwH requires addressing both the complications of hemophilia and the multifaceted effects of aging. Their care needs have become increasingly complex, requiring attention to broader aspects of health

Review article: patients' fears and unmet needs in inflammatory bowel disease.

The management of chronic illness is becoming increasingly patient-centred. Although patients with inflammatory bowel disease have a normal life expectancy, most individuals experience an impact of inflammatory bowel disease on their daily lives as well as on their attitudes, fears and beliefs. Although currently available therapies for ulcerative colitis and Crohn's disease are becoming increasingly effective, there are still many unmet needs to address in this patient population. Physicians and patients' spouses or significant others underestimate the type and severity of problems reported b

Age-Related Hearing Loss: Evidence-Based Strategies for Early Detection and Management.

Age-related hearing loss (ARHL) is common in older adults and has been linked to significant health and social challenges. These challenges include cognitive decline, depression, falls, and overall decreased quality of life. Despite its high prevalence, ARHL remains underdiagnosed and undertreated, partly due to its gradual onset, stigma, and lack of standardized screening and management protocols. This article provides a comprehensive overview of evidence-based strategies for the early detection and management of ARHL, with an emphasis on recent clinical practice guidelines where nurses are i

From Control to Elimination: Making the End of Dental Caries Thinkable.

BACKGROUND: Dental caries remains one of the most prevalent noncommunicable diseases worldwide despite decades of clinical and behavioural interventions. Persistently high prevalence suggests that current approaches are better organised for ongoing disease management than for preventing new disease occurrence. This commentary examines whether an elimination framework could provide a more coherent and outcome-oriented direction for oral health policy and practice. METHODOLOGY: We apply the Dahlem Framework to assess whether dental caries meets established criteria for disease eradication. We t

Self-management and independence guidelines for the care of people with spina bifida.

Self-management and independence behaviors are associated with improved health conditions common to spina bifida such as skin integrity and bowel and bladder management. While most children with spina bifida ultimately achieve basic self-care behaviors, (e.g., dressing appropriately, planning activities with peers, or cooking pre-planned meals), they often lag 2-5 years behind their typically-developing peers in these activities [1]. Valid and reliable condition-specific assessments of self-management and independence are critical to optimizing outcomes for this population. Partnerships among

Pregnancy care in solid organ transplant recipients.

Recipients of solid organ transplants who become pregnant represent an obstetrically high-risk population. Preconception planning and effective contraception tailored to the individual patient are critical in this group. Planned pregnancies improve both maternal and neonatal outcomes and provide a window of opportunity to mitigate risk and improve lifelong health. Optimal management of these pregnancies is not well defined. Common pregnancy complications after transplantation include hypertension, preterm birth, infection, and metabolic disease. Multidisciplinary preconception and prepartum ma

Post deployment care for returning combat veterans.

Since September 11, 2001, 2.4 million military personnel have deployed to Iraq and Afghanistan. To date, roughly 1.44 million have separated from the military and approximately 772,000 of these veterans have used VA health care. Combat deployments impact the physical, psychological, and social health of veterans. Given that many veterans are receiving care from non-VA providers, it is important that all community health care workers be familiar with the unique health care needs of this patient population, which include injuries associated with blast exposures (including mild traumatic brain in

Serologic survey for potential pathogens and assessment of disease risk in Australian fur seals.

The introduction of pathogens into populations of animals with no previous exposure to them and, therefore, no immunologic protection, can result in epizootics. Predicting the susceptibility of populations to infectious diseases is crucial for their conservation and management. Australian fur seals (Arctocephalus pusillus doriferus) have a relatively small population size, a restricted range, and form dense aggregations. These factors make this species vulnerable to epizootics of infectious diseases that spread by direct animal-to-animal contact. Blood samples were collected from 125 adult fem

Practical implementation of caries disease management in the era of CAMBRA: a guide for the general dentist.

Although the philosophy of caries management by risk assessment (CAMBRA) is widely accepted as an essential part of comprehensive disease management, its implementation in daily clinical practice has historically been hindered by a lack of concise guidance that bridges the gap between scholarly research and the logistics of patient care. With the aim of guiding comprehensive disease management for dental caries by integrating available evidence-based systems such as the American Dental Association Caries Classification System (ADA CCS) and caries risk assessment (CRA), this review presents cli

Effectiveness of the "Living with Cancer" peer self-management support program for persons with advanced cancer and their relatives: study protocol of a non-randomized stepped wedge study.

BACKGROUND: Persons with advanced cancer and their relatives experience physical, emotional, and psychosocial consequences of the illness. Most of the time, they must deal with these themselves. While peer self-management support programs may be helpful, there is little evidence on their value for this population. We present the research protocol of our SMART study that will evaluate the effectiveness of the "Living with Cancer" peer self-management support program, aimed at improving self-management behaviors, self-efficacy, and health-related quality of life of persons with advanced cancer a

Understanding type 2 diabetes self-management in the Marshall Islands: A mixed-methods exploration of sociocultural and structural barriers.

Type 2 diabetes mellitus (T2DM) poses a critical public health challenge in the Republic of the Marshall Islands, where prevalence rates are among the highest globally. Despite the central role of self-management in diabetes care, adherence and utilization of health services remain inadequate and are major contributors to diabetes-related morbidity and mortality. We aimed to identify factors influencing self-management and to explore the sociocultural context shaping these behaviors using an explanatory sequential mixed methods approach. From March to May 2023, we surveyed 123 randomly selecte

Vertebral Osteomyelitis and Epidural Abscess.

Severe spinal infections including vertebral osteomyelitis and spinal epidural abscess pose challenges in diagnosis and management. The evolution of epidemiologic risks for these processes includes the aging of the population, increase in spinal instrumentation procedures and health care-associated device utilization, and increased interconnection of the global community. Clinical presentation may be nonspecific, so clinicians must maintain a high index of suspicion to ensure prompt diagnosis and treatment to prevent progression to long-standing morbidity.
