PURPOSE: To report a case of congenital erythropoietic porphyria that presented as acute scleritis over a bilateral scleromalacia perforans in the interpalpebral fissure.
METHODS: An 18-year-old man presented with painful red eye, a history of photophobia, and passing highly colored urine since childhood. Dermatological and biochemical evaluations were done.
RESULTS: The patient had normal vision in both eyes with bilateral scleromalacia perforans. The right eye showed painful, nodular scleritis. Dermatological examination revealed multiple, vesciculobullous cutaneous lesions with atrophy an

PURPOSE: To report a case of porphyria cutanea tarda presenting as cicatricial conjunctivitis.
DESIGN: Observational study.
METHODS: A 31-year-old man presented with bilateral inferior symblepharon, superior tarsal conjunctival scarring and concretions, and recurrent conjunctival and episcleral injection.
RESULTS: Four years after initial presentation, the patient developed hepatitis C, and 2 years later blisters on his scalp and hands. Direct immunofluorescence studies of biopsies taken from the palpebral conjunctiva of the right lower lid were negative for cicatricial pemphigoid. A twenty

The pretransplant evaluation of a patient with a rare diagnosis requires knowledge of the pathophysiology and the transplant literature. A 55-year-old man presented with hypertensive kidney failure and the clinical diagnosis of acute intermittent porphyria. Complications of acute intermittent porphyria, which is a defect of heme production, are due to the accumulation of heme intermediates often precipitated by medications. Based on animal data, cyclosporine is considered unsafe in patients with acute intermittent porphyria. As part of the pretransplant evaluation, the patient received separat

UNLABELLED: A term infant born to consanguineous parents presented at birth with hypoglycaemia, thrombocytopenia, coagulopathy and hyperbilirubinaemia associated with polycythaemia due to delayed cord clamping. Despite phototherapy and correction of polycythaemia by partial exchange transfusion, coagulopathy, hypoglycaemia and conjugated hyperbilirubinaemia persisted, suggesting hepatic failure. Metabolic work-up led to the diagnosis of tyrosinaemia type 1 on day 4. Two--(2-nitro-4-trifluoromethylbenzoyl)--1,3 cyclohexanedione (NTBC) treatment, started on day 5, resulted in progressive clinica

BACKGROUND: To know the lipid profile in patients with porphyria cutanea tarda (PCT).
PATIENTS AND METHODS: Serum lipoproteins have been studied in 64 males with PCT: 42 with chronic hepatitis C and 22 seronegatives for hepatitis B and C virus. Thirteen patients without porphyria, but with chronic hepatitis C, and 13 healthy subjects were also studied.
RESULTS: Patients with chronic hepatitis C, with or without PCT, had a decrease of total and VLDL cholesterol (TC and VLDLc) and apolipoprotein (apo) B in comparison with healthy controls and seronegative for hepatitis C virus patients with PC

HIV infection is associated with abnormal porphyrin metabolism. While HCV infection alone is thus far not associated with significant abnormal porphyrin levels, it does contribute to further elevation of porphyrin levels in patients already infected with HIV. These infections alone are not sufficient to induce the development of clinical porphyria. However, they render patients susceptible to having symptomatic porphyria upon exposure to other porphyrinogenic agents.

Two cases of familial porphyria cutanea tarda (PCT) with constrictive pericarditis are described. A 50-year-old woman and her 48-year-old younger brother were admitted because of right ventricular heart failure. Constrictive pericarditis was diagnosed by RV pressure waveform and echocardiogram. The patients were diagnosed as PCT based on clinical symptoms, histologic findings and elevated urinary excretion levels of uroporphyrin. Even to this day, over 40% of the etiology of constrictive pericarditis remains unknown. There is a possibility of overlooking porphyria cutanea tarda in constrictive

Porphyria cutanea tarda (PCT), the most common form of porphyria, is manifested as skin photosensitivity caused by excess hepatic production of uroporphyrin and heptacarboxylporphyrin. In experimental animal models, ascorbic acid modulates chemically induced uroporphyrin accumulation. The purpose of this study was to determine whether ascorbic acid is decreased in the plasma of patients with PCT. Plasma was obtained after an overnight fast from 21 PCT patients, 16 of whom were infected with hepatitis C virus (HCV), and from a separate group of 9 patients with HCV infection but not PCT. Thirtee

A case of a familial porphyria cutanea tarda (PCT-II) is reported in which the clinically overt form of PCT was provoked by factors relating to chronic lymphoid leukemia (CLL). Typical lesions of PCT developed on a 55-year-old woman after several blood transfusions and chlorambucil treatment. Besides these provoking factors, cytomegalovirus (CMV) infection was diagnosed. Erythrocyte uroporphyrinogen decarboxylase activity was about 50% of normal in the patient and in her two children. This case supports the suggestion that development of PCT in patients with hematological disorders is more tha

We report a case of porphyria cutanea tarda in which the only cutaneous abnormality and initial presenting complaint was hypertrichosis. The patient had a history of excessive alcohol consumption. Alcohol avoidance and treatment with low-dose hydroxychloroquine resulted in resolution of the hypertrichosis and normalization of biochemical abnormalities.

Porphyria cutanea tarda (PCT) is due to reduced activity of the enzyme uroporphyrinogen decarboxylase in hepatocyte. Hepatic dysfunction is found in almost all cases of PCT. Recently, high prevalence of hepatitis C virus (HCV) infection in patients with PCT was reported in Europe. Is HCV infection a trigger of PCT? Several cases of PCT associated with hepatitis C were reported in Japan. The frequency of anti-HCV increased with the severity of liver histology. These findings implicate HCV in the etiology of PCT-associated liver disease. Treatment with interferon should be considered for PCT wit

Fluorescence spectra of plasma porphyrin were measured in 6 patients with the acute intermittent porphyria (AIP), 30 patients with variegate porphyria (PV), 2 patients with hereditary coproporphyria, 2 patients with porphyria cutanea tarda, and in 6 patients with erythropoietic protoporphyria (EPP). It was found that the excitation and emission wavelengths at which maximum fluorescence is seen may help to diagnose and differentiate PV and EPP. In patients with AIP spectrum characteristic for porphyria of such a type was noted in all patients during the attack of disease and in only 33% of pati

We report a patient with sclerodermatous changes secondary to porphyria cutanea tarda, who responded to venesection treatment. The clinical characteristics of cutaneous sclerosis secondary to porphyria cutanea tarda and the degree to which it can mimic generalized morpheas are discussed.
