Empowerment through agency-promoting dialogue: an explicit application of harm reduction theory to reframe HIV test counseling.

The counseling that accompanies HIV testing can be an important prevention tool for encouraging people to practice safer sex to avoid AIDS, but there is scant research about how HIV test counseling operates in practice. This article critiques the current Centers for Disease Control and Prevention (CDC) protocol for HIV test counseling for not being genuinely client centered and ignoring the unique needs of clients and offers an alternative approach that adapts and explicitly applies the tenets of harm reduction theory (HRT). Excerpts from actual HIV test counseling sessions illustrate both the

Alcohol expectancies, alcohol use, and hostility as longitudinal predictors of alcohol-related aggression.

The direct and interactive effects of alcohol expectancies for aggression, dispositional hostility, and heavy alcohol consumption on alcohol-related physical aggression were examined across the first four years of marriage in a sample of 634 newlywed couples. For husbands, alcohol aggression expectancies predicted increases in alcohol-related aggression; across husbands and wives, however, aggression expectancies were not found to interact with hostility or alcohol consumption to predict physical aggression. Consistent with previous research, hostility and alcohol consumption interacted with e

Leadership and ethics in nurse-nurse relationships.

Qualities of nursing leadership may be reflected in the patterns of relating illuminated through communications between interdependent members of a discipline and interdisciplinary professional healthcare relationships. Authority and responsibility in leading-following reside with the designated leader. However, there is power with person in situation with the ever-present possibility of conflict. The author in this column will begin a discussion of conflict in nurse-nurse relationships and offer questions for straight thinking regarding the ethics of leading-following situations with nurse-nu

Attitudinal predictors of dissociation: hostility and powerlessness.

The relationship between dissociative tendencies and attitudes relating to aggression and powerlessness was investigated in a sample of 106 Australian psychology students. Dissociation was positively related to aspects of aggression and in particular to an attitude of hostility. Respondents' perceptions of their own powerlessness did not predict proneness to dissociative experiences. The data are consistent with the view that trauma-engendered attitudes and affects may be markers of the development of dissociative tendencies.

Prospective prediction of husband marital aggression within newlywed couples.

This study prospectively examined a social interactional model of husband marital aggression. Young couples were assessed at the time of their 1st marriage with respect to marital conflict styles, alcohol consumption, hostility, gender identity, perceived power inequity, and history of family violence. Couples were reassessed at their 1-year anniversary, and information concerning marital aggression was collected. Most of the constructs were prospectively related to husband aggression, but these relationships were largely mediated through marital conflict styles and husband alcohol consumption

Surviving survivors. Coping with caring for patients who have been victimized.

Caring for survivors of victimization is very hard work, in ways that are similar to and different from other difficult patient interactions. Careful attention to basic physiologic and psychological needs of the physicians is necessary to tolerate the stress and to prevent the burnout that can accompany this work. In addition, understanding survivors' interpersonal dynamics may help physicians provide better care. Consultation with colleagues is essential to remaining healthy and effective as givers of care.

Study protocol of guided mobile-based perinatal mindfulness intervention (GMBPMI) - a randomized controlled trial.

BACKGROUND: Psychological distress is a common occurrence among women during the perinatal period. Maternal psychological distress (MPS) can also have a negative influence on neonatal outcomes such as infant health, child development or mother-child interaction. Hence, interventions to improve mental wellbeing during this period are vital. Mindfulness based intervention (MBI) has been found to be effective in reducing psychological distress. Delivery of MBI via the internet, making it accessible and inexpensive, is showing a promising positive effect in reducing psychological distress. A rando

Conceptual Foundations and Thematic Frameworks of Individual Empowerment in Health Communication: A Systematic Review.

The primary aim of this study was to systematically review and analyze conceptualizations of individual empowerment in health communication research published over the past 20 years, with the objective of identifying and comparing thematic frameworks and clarifying how different conceptual approaches are grounded, overlap, and diverge. The conducted systematic review followed the analytical procedure described in the PRISMA-Protocol and was based on a total sample of 66 research papers. A qualitative thematic analysis was conducted to examine conceptualizations of individual empowerment by ide

Evaluating the health and fitness benefits of a 6-month FES-cycling program on a recumbent trike for individuals with motor complete SCI: a pilot study.

BACKGROUND: Spinal cord injury (SCI) severely affects physical function, leading to muscle atrophy and reduced bone density. Sport-therapy, incorporating recreational and competitive activities, has shown promise in enhancing recovery for individuals with SCI. Functional Electrical Stimulation (FES)-cycling combines exercise benefits with stimulation advantages, and recent integration with mobile recumbent trikes adds further potential. This study aimed to evaluate the effects of a 6-month FES-cycling sport therapy using a recumbent trike on individuals with motor complete SCI. METHODS: Five

Using Ultrasound and Power-Assisted Devices through Lateral Incision: The OCCULT Technique for Gynecomastia-A Multicentric Large Series Study.

INTRODUCTION: Gynecomastia is a common condition characterized by male breast enlargement, which can have a profound psychological impact on affected individuals. Surgical intervention is often sought to alleviate these concerns. This study evaluates the lateral port technique for gynecomastia surgery, which combines power-assisted liposuction (PAL) and ultrasound-assisted liposuction (UAL) to achieve optimal surgical outcomes with no scar on front of chest. MATERIALS AND METHODS: A retrospective analysis was conducted on 967 patients who underwent gynecomastia surgery using the Out of Sight,

Clinical effects of continuous theta burst stimulation for generalized anxiety disorder and a mechanism involving α oscillations: a randomized controlled trial.

BACKGROUND: Continuous theta burst stimulation (cTBS) is a much more rapid protocol than low-frequency repetitive transcranial magnetic stimulation (rTMS), but no clinical trial has yet investigated the efficacy and mechanisms of cTBS for the treatment of generalized anxiety disorder. The purpose of this study was to compare the clinical effects and α oscillations induced by cTBS versus 1 Hz rTMS as predictors of response, and to assess the underlying mechanisms of the therapeutic effects of cTBS in patients with generalized anxiety disorder. METHODS: We randomly allocated 120 patients with g

Catastrophic cognitions about coronavirus: the Oxford psychological investigation of coronavirus questionnaire [TOPIC-Q].

BACKGROUND: Cognitive therapies are developed on the principle that specific cognitive appraisals are key determinants in the development and maintenance of mental health disorders. It is likely that particular appraisals of the coronavirus pandemic will have explanatory power for subsequent mental health outcomes in the general public. To enable testing of this hypothesis we developed a questionnaire assessing coronavirus-related cognitions. METHODS: 12 285 participants completed online a 46-item pool of cognitions about coronavirus and six measures of different mental health problems. The s

Transactional cascades of destructive interparental conflict, children's emotional insecurity, and psychological problems across childhood and adolescence.

This study examined the transactional interplay among dimensions of destructive interparental conflict (i.e., hostility and dysphoria), children's emotional insecurity, and their psychological problems from middle childhood and adolescence. Participants were 232 families, with the first of five measurement occasions occurring when children were in first grade (M age = 7 years). Cross-lagged, autoregressive models were conducted with a multiple-method, multiple-informant measurement approach to identify developmental cascades of interparental and child cascades. Results indicated that emotional

Shell shock, trauma, and the First World War: the making of a diagnosis and its histories.

During the First World War, thousands of soldiers were treated for "shell shock," a condition which encompassed a range of physical and psychological symptoms. Shell shock has most often been located within a "genealogy of trauma," and identified as an important marker in the gradual recognition of the psychological afflictions caused by combat. In recent years, shell shock has increasingly been viewed as a powerful emblem of the suffering of war. This article, which focuses on Britain, extends scholarly analyses which question characterizations of shell shock as an early form of post-traumati

Healthcare Professionals, Post-traumatic Stress Disorder, and COVID-19: A Review of the Literature.

The recent COVID-19 pandemic impacted healthcare professionals psychologically. They were unprepared to handle such a powerful and unknown virus. Consequently, they had to face situations of extreme distress, developing vicarious traumatization and post-traumatic stress disorder (PTSD). The first one is associated with the "cost of caring" for others and affected persons who constantly are exposed to other sufferings. PTSD is a psychiatric disorder that could affect people who have experienced or witnessed a traumatic event. Post-traumatic stress disorder (PTSD) and correlated symptoms might i

Psychological stress and coeliac disease in childhood: a cohort study.

BACKGROUND: Psychological stress has previously been associated with several immunological diseases, e.g. inflammatory bowel disease. Through questionnaire data from the ABIS study (All Babies In southeast Sweden) we examined the association between psychological stress in the family and biopsy-proven coeliac disease (CD) in the child. METHODS: We used serious life event, parenting stress, and parental worries as measures of psychological stress. Data were collected when the child was 1 and 2.5 years old in some 11,000 and 8,800 children, respectively. CD was confirmed through small intestina

Independent contribution of psychological factors to fibrin turnover in subjects with sleep apnoea and/or systemic hypertension.

By promoting atherosclerosis and thrombosis, a blood-clotting diathesis could contribute to excess cardiovascular morbidity and mortality in patients with systemic hypertension and/or obstructive sleep apnoea. Since psychological states affect haemostatic activity, we wondered about the contribution of behavioural factors to a hypercoagulable state in subjects with increased risk of cardiovascular disease. To tease apart the potential additive nature of cardiovascular disease risk, we examined four patient groups - hypertensives and normotensives, with and without sleep apnoea. The procoagulan

Qualitative evaluation of the Teenage Mothers Project in Uganda: a community-based empowerment intervention for unmarried teenage mothers.

BACKGROUND: A large proportion of unmarried teenage mothers in Uganda face physical, psychological, and social problems after pregnancy and childbirth, such as obstetric complications, lack of education, and stigmatisation in their communities. The Teenage Mothers Project (TMP) in Eastern Uganda empowers unmarried teenage mothers to cope with the consequences of early pregnancy and motherhood. Since 2000, 1036 unmarried teenage mothers, their parents, and community leaders participated in economic and social empowerment interventions. The present study explored the changes resulting from the T
