A case of priapism in a patient receiving home parenteral nutrition is reported. To our knowledge this is the first such case to occur in a patient receiving home parenteral nutrition. The literature is reviewed and the possible aetiology discussed.

The painful pathological condition of the penis, priapism, is usually associated with serious destructive changes which lead to impotence if treatment is with-held or delayed. A case of juvenile priapism is presented following cortisone and penicillin treatment for a traumatic lesion of the oesophagus. Increased coagulability from such treatment is discussed as causative factor. The importance of early surgical intervention is stressed and the methods of choice presented.

A man of 35 years, who had had three attacks of subarachnoid hemorrhage in the previous 3 years, was admitted to hospital with complaints of headache and priapism. After assessing the data currently available on the multiple modalities used to manage priapism secondary to sickle cell disease we adopted a specific therapeutic regimen for the treatment of this problem. Early non-surgical measures, including hydration, alkalization and analgesics, rapid red cell infusion to double the hematocrit and late shunting procedures in conjunction with exchange transfusion, offer the soundest approach on pathophysiological grounds. There is currently no evidence to support early surgical intervention as the best approach to conserve potency in this problem.

Clinical data and therapy of all male home dialysis patients of two centres were compared in order to establish predisposing factors to priapism. One centre (Frankfurt) had nine priapisms in 96 patients, whereas the other centre (Montpellier) had none in 59 patients. The only difference found were higher haematocrits in Frankfurt than in Montpellier. The highest haematocrits were found in the priapism patients. Two cases of priapism treated by corpus-saphenous by-pass are presented. Priapism responded to treatment successfully in both cases. Satisfactory erection and sexual intercourse followed 6 weeks after operation in one case. The second patient failed to have further erections. It was thought that this was due to the length of time (6 weeks) between onset of priapism and operation.

Two children with priapism are presented; one case was secondary to trauma and the other was idiopathic in nature. Both boys were effectively treated by bilateral corporosaphenous shunts. The mechanism of normal erection and the pathophysiology of priapism are discussed.

In a review of the literature on shunt operations for priapism, 100 case reports were found. The over-all result in terms of potency is 61 per cent. In general there is little difference in outcome between the cavernosus-saphenous vein and cavernosus-spongiosum shunts. If the former surgical procedure is to be performed, a unilateral operation appears to yield better results than a bilateral one. Various causes of priapism are noted in relation to results of surgery, and the complications of shunt procedures are discussed.
