Effectiveness of family-provider communication interventions on family's psychological health, communication quality, and health care utilization in the ICU: A systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: This review aims to assess evidence regarding the effectiveness of family-provider communication interventions. METHODS: We searched six databases (PubMed, PsycINFO, Web of Science, EMBASE, CINAHL and the Cochrane Library). Two researchers independently extracted data and assessed bias. Data were analyzed using Stata software and Review Manager. RESULTS: Nineteen studies were included. Communication interventions had a moderate effect on family needs (SMD = -0.43, 95 % CI: -0.81 to -0.06, P = 0.025), small effects on 6-month psychological distress (Overall: SMD = -0.14, 95 % CI: -

Life history and meanings: end of life in an intensive and semi-intensive care unit.

This study addressed the end-of-life experience of a patient admitted to an intensive and semi-intensive care unit based on the accounts of a family member who accompanied her therapeutic journey. The study sought to understand how healthcare teams consider patients' life histories and how this influences care strategies. We also sought to identify the interaction between healthcare teams and patient family members when patients are no longer able to communicate. Immersion in an intensive and semi-intensive care unit enabled an ethnographic approach involving participant observation, in-depth

Frequency and determinants of words, gestures, and attitudes experienced as disrespectful during childbirth and postpartum: A national population-based study.

INTRODUCTION: Disrespectful maternity care is receiving increasing attention but remains poorly assessed in high-income countries. The aim was to assess the frequency and determinants of professionals' words, gestures and attitudes experienced as disrespectful by women during childbirth and postpartum stays. MATERIAL AND METHODS: This national quantitative observational study used the population-based Enquête Nationale Périnatale (ENP) 2021 database, which provides a representative sample of women who gave birth in metropolitan France and includes a 2-month postpartum follow-up (n = 7394). Intersections of personal vs. collective trauma during the COVID-19 pandemic: the hijacking of the human imagination.

This paper explores how the deadly shadow of COVID-19 passing over the Earth constitutes a collective trauma that frequently opens up or 'triggers' un-remembered personal trauma, and it provides clinical examples of these intersections. The paper further explores how the human imagination, which we normally utilize to make meaning out of traumatic experience, can be hijacked by fear - leading to avoidance of suffering and to illusory formulations and alternative realities such as conspiracy theories. Alternatively, the imagination can be employed in more realistic and creative ways - leading t

The Shared Subjective Frames of Interdisciplinary Practitioners Involved in Function-Focused Care in a Nursing Home: Q-Methodology.

BACKGROUND: An interdisciplinary team-based approach in nursing homes has been suggested in the literature as a strategy for delaying functional decline in residents. Function-focused care is a philosophy-based approach in which interdisciplinary practitioners assess functional capacity and help older adults to optimize and maintain their remaining abilities. PURPOSE: This study explored and described the shared subjective frames of interdisciplinary practitioners as regards function-focused care for nursing home residents. METHODS: Q-methodology was used to analyze the subjectivity of each

"You don't want anyone who hasn't been through anything telling you what to do, because how do they know?": Qualitative analysis of case managers in a hospital-based violence intervention program.

STATEMENT OF PURPOSE: Intentional violent injury is a leading cause of disability and death among young adults in the United States. Hospital-based violence intervention programs (HVIPs), which strive to prevent re-injury through intensive case management, have emerged as a successful and cost-effective strategy to address this issue. Despite the importance of strong therapeutic relationships between clients and their case managers, specific case manager behaviors and attributes that drive the formation of these relationships have not been elucidated. METHODS: A qualitative analysis with a mo

What bereaved parents want health care providers to know when their babies are stillborn: a community-based participatory study.

BACKGROUND: Bereaved parents experience higher rates of depressive and post-traumatic stress symptoms after the stillbirth of a baby than after live-birth. Yet, these effects remain underreported in the literature and, consequently, insufficiently addressed in health provider education and practice. We conducted a participatory based study to explore the experiences of grieving parents during their interaction with health care providers during and after the stillbirth of a baby. METHODS: This community-based participatory study utilized four focus groups comprised of twenty-seven bereaved par

Continuous support for women during childbirth.

BACKGROUND: Historically, women have generally been attended and supported by other women during labour. However, in hospitals worldwide, continuous support during labour has often become the exception rather than the routine. OBJECTIVES: The primary objective was to assess the effects, on women and their babies, of continuous, one-to-one intrapartum support compared with usual care, in any setting. Secondary objectives were to determine whether the effects of continuous support are influenced by:1. Routine practices and policies in the birth environment that may affect a woman's autonomy, fr

Responding to the existentials of non-life-threatening chronic conditions.

Diagnosis of a major life-threatening illness can prompt a range of existential responses that typically include strong emotions and a reassessment of priorities. What has not attracted attention are the existential responses to being told about less major but permanent medical conditions. A conceptual frame is developed for identifying and speaking about the existential dynamics of permanent change. It introduces two key concepts, finitude and transcendence, and uses them to explain the common sequence of responses first from being told about a chronic condition through to when its presence h

The Role of Psychopathic Traits in the Development of the Therapeutic Alliance Among Sexual Offenders.

There is a growing body of research demonstrating that the therapeutic alliance (TA) affects outcomes among specialized forensic populations, including sexual offenders. Despite this consensus, researchers continue to question whether higher levels of psychopathic traits are conducive to the formation of a therapeutic relationship for high-risk sexual offenders. Thus, the current study adds to the literature by examining the relationship between the TA and levels of psychopathy among a sample of incarcerated sexual offenders participating in sexual offender treatment. Overall, we found no sign

Therapeutic bond judgments: Congruence and incongruence.

OBJECTIVE: The present study had 2 aims: (a) to implement West and Kenny's (2011) Truth-and-Bias model to simultaneously assess the temporal congruence and directional discrepancy between clients' and therapists' ratings of the bond facet of the therapeutic alliance, as they cofluctuate from session to session; and (b) to examine whether symptom severity and a personality disorder (PD) diagnosis moderate congruence and/or discrepancy. METHOD: Participants included 213 clients treated by 49 therapists. At pretreatment, clients were assessed for a PD diagnosis and completed symptom measures. Sy

Client attachment and therapist feelings in the treatment of bulimia nervosa.

The relationship between client attachment and therapist postsession feelings was investigated in a randomized clinical trial of psychoanalytic psychotherapy (PPT) and cognitive behavior therapy (CBT) for bulimia nervosa. Therapists completed feeling word checklists after each session, and client attachment was assessed with the Adult Attachment Interview. A factor analysis identified 3 feeling factors, Happy/Enthusiastic, Overwhelmed/Moved, and Indifferent/Bored. Positive feelings were predominant, but PPT therapists reported more negative and fewer positive feelings than CBT therapists did.

Sexual Offenders' Perceptions of the Client-Therapist Relationship: The Role of Risk.

The therapeutic alliance has been shown to be integral to treatment outcomes even in forensic settings. There is still a relative dearth of research examining factors related to the formation of the therapeutic alliance in sex offender treatment specifically. Using a sample of 202 incarcerated male sexual offenders participating in sex offender treatment, this study examined whether perceptions of the client-therapist relationship from the perspective of male sexual offenders varied by risk for sexual and general recidivism. Overall, we found a significant negative relationship between risk fo

Contact with primary care: the experience of people with intellectual disabilities.

BACKGROUND: People with intellectual disabilities experience disparities in their health and health care. Annual health checks are intended to counter such disparity by improving access to primary health care. However, little is known about their experience of having a health check or other types of contact with primary care services. MATERIALS AND METHODS: The findings of two studies which used focus groups were combined. 102 people with intellectual disabilities participated in the focus groups. RESULTS: Participants' experiences of primary care services generally, and health checks in par

Spirituality in palliative home care: a framework for the clinician.

PURPOSE: Spiritual care at the end of life remains poorly understood despite its promotion by the World Health Organisation. The purpose of this paper was to develop a consensus-based framework of the main elements of spiritual care in palliative home care. METHODS: Expert meeting using the nominal group technique, followed by a two-stage web-based Delphi process, was used. Experts from three stakeholder groups (physicians, professional spiritual care givers and researchers) representing two countries (Belgium and the Netherlands) participated in this study. RESULTS: Fourteen elements of spi

When they hear what we say: ethical challenges in presenting research findings to the Huntington disease community.

Sharing findings with the Huntington disease (HD) community and other genetic disease communities is challenging because of the sensitivity involved in effectively communicating findings to participants. We describe our experiences of presenting multi-disciplinary research findings to the HD community, and discuss the need to: (1) balance potential benefits and harms for participants, researchers, and others; (2) demonstrate respect for participants' needs, expectations, and priorities; and (3) ensure transparency and respect for autonomy.

A dialogue among various cultures and its manifestation in stuttering therapy.

UNLABELLED: This paper presents a historical retrospective of the evolution of the clinical aspects of stuttering, and refers to social, cultural, political, and economic variables that might have exerted an influence on this evolution, particularly in relation to the authors' experience in Israel. The retrospective commences in the early decades of the 20th century, when speech and language pathology did not exist as a profession, and presents the major approaches to therapy in children and adults employed by clinicians throughout the century. We focused on the impact of the change that occur
