Marfan syndrome is an autosomal dominant connective tissue disorder that affects multiple systems, including the skeletal, ligamentous, oculofacial, pulmonary, abdominal, neurological, and cardiovascular systems. Cardiovascular complications, which involve the aorta and aortic valve, contribute most significantly to patient morbidity and mortality. A literature review was conducted on pathophysiology of the disease and recommendations for early diagnosis and treatment. Diagnosis largely relies on clinical features and a thorough history. Echocardiogram is used for monitoring aortic abnormaliti

Two women, aged 57 and 55 years, with metastatic breast cancer were admitted for uncontrolled pain due to bone metastases. Despite the fact that progressive disease was evident, a change in antitumour therapy had not been recommended. The pain control was optimised in both patients. In one patient, palliative chemotherapy was installed, combined with trastuzumab because of HER2/neu overexpression. She was still alive after one and a half year of treatment. The other patient could not adjust mentally to the fact that her palliative therapy was changed to antitumour therapy; she died one month l

We present a case report of refractory multifactorial shock initially managed as septic shock, requiring multidisciplinary collaboration when the diagnostic pathway was limited by acute kidney injury; the primary learning objective is to highlight clinical decision-making in shock when standard contrast imaging is contraindicated. Echocardiography revealed right ventricular failure with right ventricular thrombus, and pulmonary embolism was strongly suspected but could not be confirmed due to the inability to obtain contrast imaging. Persistent shock prompted a transition from a septic shock-d

Mandibular bone loss can cause functional and aesthetic challenges. Idiopathic osteolysis, characterized by spontaneous and progressive bone resorption, may affect the craniofacial skeleton and often presents without pain or inflammation, leading to delayed diagnosis. This report presents a case of a 33-year-old female patient with mandibular bone loss exhibiting tooth mobility, pain, and difficulty chewing. Subsequently, the patient underwent mandibular reconstruction to restore anatomical integrity and improve functional capacity and psychosocial well-being. The case highlights the nonspecif

Guillain-Barré syndrome (GBS) is a rare complication in children with acute lymphoblastic leukemia (ALL). This case report explores the presentation and management of GBS in a 16-year-old male with a history of ALL, who developed GBS during maintenance therapy. The patient exhibited progressive symmetrical weakness, sensory loss, and autonomic dysfunction. Diagnostic workup, including nerve conduction studies and lumbar puncture, confirmed the diagnosis of GBS. Differentiating GBS from vincristine-induced neuropathy, a common challenge in this population, was crucial for appropriate management

Hydatidosis, caused by the larval stage of Echinococcus granulosus, is a zoonotic disease typically affecting the liver and lungs. Cerebral localizations are rare, especially in the brainstem. We present a case of a 9-year-old boy with a brainstem hydatid cyst. The patient exhibited progressive walking difficulties and limb impairment. MRI revealed a brainstem mass consistent with a hydatid cyst. Due to the lesion's size and location, surgical intervention was necessary. The cyst was decompressed and removed without complications, followed by albendazole treatment. Postoperative recovery was u

This study presents the clinical features and disease progression of a 39-year-old male patient diagnosed with Huntington's disease (HD). The diagnosis was confirmed by direct genetic testing, using DNA obtained from a blood sample that revealed expanded cytosine-adenine-guanine (CAG) repeats in the huntingtin gene (HD gene). The patient exhibited motor symptoms, including chorea, muscle rigidity, coordination difficulties, and speech and swallowing impairments. Cognitive symptoms comprised impaired judgment, planning difficulties, slowed thinking, memory lapses, and attention problems. The pa

Syringomyelia is a neurological condition characterized by the presence of fluid-filled cavities within the spinal cord, resulting in progressive neurological deficits. Secondary holocord syringomyelia, a rare manifestation in the entire spinal cord, is associated with spinal hemangioblastomas. We report the case of a 29-year-old female who presented with neck and bilateral upper limb pain and numbness. She was diagnosed with secondary holocord syringomyelia associated with a spinal hemangioblastoma and underwent conservative management. Magnetic resonance imaging plays a critical role in diag

A 62-year-old man presented with worsening dyspnoea, haemoptysis and reduced exercise tolerance. He was found to be hypoxaemic with bilateral basal opacification on chest imaging, but inflammatory markers, respiratory virus PCR and sputum culture demonstrated no signs of infection. The patient reported having initially mild, yet progressive, symptoms since he started vaping 14 months previously. He was treated with oxygen therapy, supportive care and cessation of vaping. Chest imaging at discharge showed marked improvement of previous bilateral opacification and the patient returned to baselin

This is a case presentation of a female patient who underwent a Girdlestone arthroplasty at age 10 years and a total hip arthroplasty at age 21. Despite early postoperative rehabilitation, the patient experienced increasing pain, progressive gait deviations, and functional limitations during the year after surgery. This course of care was initiated 1 year after surgery and focused on motor retraining to address pain and gait deviations. This case demonstrates that positive outcomes can be achieved after longstanding musculoskeletal dysfunction is corrected, but that prolonged rehabilitation ma

We describe the case of a 45-year-old male with pulmonary paracoccidioidomycosis and spontaneous pneumothorax. The patient presented to the hospital with sudden and intense chest pain accompanied by dyspnea and had a six-month history of dry cough, weight loss, and progressive dyspnea on exertion. Chest X-ray showed a small right pneumothorax, bilateral nonhomogeneous opacities, and emphysematous areas in the lung base. Chest computed tomography showed consolidation in both lungs, with architectural distortion, nodules, interlobular septal thickening, and emphysema, in addition to the right pn

There are indications for the use of cytotoxic drugs in some dermatologic diseases. These drugs may be lifesaving for patients with diseases such as pemphigus, lymphomas, and vasculitis or they may merely offer an improved quality of life for patients who have diseases such as psoriasis, progressive systemic sclerosis, or sarcoidosis. In either set of circumstances, in the properly monitored patient cytotoxic drugs may be used safely and effectively, offering very few problems for the patient or the therapist. The dermatologist who is interested in offering his/her patients the best medical ca

A 40-year-old man had a severe headache of sudden onset and progressive palsy of the third cranial nerve from a minor subarachnoid hemorrhage. Despite assessment by two primary care physicians the possibility of serious neurologic disease was not raised and the urgency of the situation not appreciated. Two weeks after the onset of the headache the patient was urgently admitted to hospital at the request of a neuro-ophthalmologist. Cerebral angiography revealed an aneurysm of the posterior communicating artery, which was surgically occluded with a clip. The patient had a satisfactory recovery,

BACKGROUND: Friedreich's ataxia (FA) is a rare progressive and multi systemic neurodegenerative disorder characterized by loss of coordination, typically resulting in loss of ambulation. Cardiomyopathy, diabetes mellitus, and scoliosis are common and serious manifestations of the disease. This project explores the FA patient pathways, healthcare use and costs, and compares people's care experiences attending specialist ataxia centres (SAC) with care in non-specialist settings in three European countries.
METHODS: We collected data in the UK, Germany and Italy using a patient survey, to gather

A 47-year-old woman with a history of a pulsatile headache, photophobia, dizziness and blurred vision was diagnosed with a massive expansive meningioma and proposed for surgical excision. During surgery, the patient began to show progressive haemodynamic instability with extreme hypotension and severe arrhythmia that only responded to epinephrine. With the continuity of haemodynamic instability, ST segment elevation and great amount of blood loss, the surgery was postponed. The follow-up brain CT scan showed evidence of posterior reversible encephalopathy syndrome and cardiac catheterisation d

A woman with metastatic melanoma was treated with immunotherapy induction with ipilimumab and nivolumab and radiotherapy to liver metastases. The patient deteriorated shortly thereafter, becoming febrile and hypotensive and requiring admission to the intensie care unit (ICU) for inotrope support. Failure to respond to antibiotics and a negative septic screen prompted further investigation, which ultimately led to a diagnosis of haemophagocytic lymphohistiocytosis (HLH). The patient improved on high dose steroids and was discharged home. Months later, in the context of progressive melanoma, the

BACKGROUND: Subacromial impingement syndrome (SIS) is a painful, and often long lasting, shoulder condition affecting patient function and quality of life. In a recent study, we observed major strength impairments in shoulder external rotation and abduction (~30%) in a population of patients with pronounced and long-lasting SIS. However, the current rehabilitation of such strength impairments may be inadequate, with novel rehabilitation programmes including exercise therapy only improving external rotation strength by 4-13%. As these previous studies are the basis of current practice, this sug

Aim of the study was to examine the course of schizophrenia patients within 2 years after discharge. Within a multicenter study of the German Competence Network on Schizophrenia, patients suffering from a schizophrenia spectrum disorder were examined regarding their psychopathological improvement, tolerability, and the treatment regime applied during hospitalization and a 2-year follow-up period. Response, remission, the level of everyday functioning, and relapse were furthermore evaluated during the follow-up period using established definitions for these outcome domains. The psychopharmacolo
