Uncovering the Mechanisms of Failure of Proton Pump Inhibitors in GERD: The Role of Esophageal Acid Sensitivity.

PURPOSE: For unclear reasons, up to 50% of subjects with gastroesophageal reflux disease have an inadequate symptomatic response to treatment with a proton pump inhibitor. Our primary objective was to examine esophageal acid in subjects who experienced heartburn following a standard meal that failed to respond to a proton pump inhibitor. METHODS: We analyzed data from a previous randomized crossover trial of proton pump inhibitors in 27 subjects with gastroesophageal reflux disease and baseline esophageal pH < 4 for more than 10% of a 24-h recording. Esophageal pH and heartburn severity were

Evaluating the risk of osteoporosis-related adverse events with proton pump inhibitors: a pharmacovigilance study.

BACKGROUND: Proton pump inhibitors (PPIs) are the most effective antacids and are widely used in the treatment of acid-related diseases. However, the impact of PPIs on bone remains controversial. This study aimed to explore the association between PPIs and osteoporosis-related adverse events in the real world. MATERIALS AND METHODS: Data from the United States Food and Drug Administration Adverse Event Reporting System from the first quarter of 2004 to the third quarter of 2024 were included in this study. Four pharmacovigilance analyses, reporting odds ratio (ROR), proportional reporting rat

Mucosal integrity of the larynx and hypopharynx and the response to proton pump inhibitors in patients with laryngopharyngeal reflux.

PURPOSE: To evaluate whether impairment of integrity of laryngeal and hypopharyngeal mucosa in laryngopharyngeal reflux (LPR) could be related to response to proton pump inhibitors (PPIs). METHODS: Chronic hoarseness with Reflux Finding Score (RFS) > = 7 was evaluated using the Reflux Disease Questionnaire (RDQ) and Reflux Symptoms Index (RSI). Upper endoscopy was performed and samples of the laryngeal posterior commissure were collected to evaluate ex vivo laryngeal mucosal integrity by transepithelial resistance (TER) using Ussing chamber. In vivo, hypopharyngeal integrity was measured usin

The Association Between Proton Pump Inhibitor Use and Gastroenteropancreatic Neuroendocrine Tumors: A Systematic Review.

PURPOSE: This systematic review evaluated the site-specific association between long-term use of proton pump inhibitors (PPIs) and neuroendocrine tumors (NETs), with a primary focus on gastric and duodenal NETs. METHODS: Eight non-randomized studies involving about 1273 cases from the USA, Mexico, and Italy were included and reviewed. Definitions of PPI exposure and comparator groups varied across studies, and methodological quality was assessed with the ROBINS-I tool. Due to heterogeneity in design and outcomes, results were narratively synthesized. RESULTS: The findings were site-specific.

Link between pharyngeal acid reflux episodes and the effectiveness of proton pump inhibitor therapy.

BACKGROUND: Diagnosing laryngopharyngeal reflux (LPR) is challenging due to overlapping symptoms. While proton pump inhibitors (PPIs) are commonly prescribed, reliable predictors of their responsiveness are unclear. Reflux monitoring technologies like dual potential of hydrogen (pH) sensors and multichannel intraluminal impedance-pH (MII-pH) could improve diagnosis. Research suggests that a composite pH parameter, defined by ≥ 2 pharyngeal acid reflux (PAR) episodes and/or excessive esophageal acid reflux (EAR), predicts PPI efficacy. The criteria for PAR episodes, a pharyngeal pH drop of ≥ 2

Proton-Pump Inhibitors and Cardiovascular Adverse Events: A Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Some observational studies suggest a cardiovascular risk from proton-pump inhibitor (PPI) treatment, but observational data may be subject to bias. We conducted a meta-analysis of randomized controlled trial data on cardiovascular events during PPI treatment. METHODS: Manufacturers of PPIs provided data from their PPI clinical trial programs. We included randomized trials with at least 100 subjects, treatment duration >30 days, and a non-PPI comparator (active or placebo). We obtained person-time of exposure per trial, to calculate summary incidence rate ratios (primary analysis

Association between exposure to proton pump inhibitors and delirium: a descriptive and disproportionality analysis of VigiBase.

OBJECTIVES: Proton pump inhibitor (PPI) exposure can lead to hyponatraemia, which is a common cause of delirium. An association between PPI exposure and delirium without hyponatraemia has been suggested in the literature. We aimed to describe the association between reports of delirium and PPI exposure and to assess the association between PPI and delirium with and without hyponatraemia. DESIGN: A descriptive and disproportionality analysis of claims data. SETTING: World pharmacovigilance database VigiBase between 1 January 1991 and 9 February 2022. PRIMARY AND SECONDARY OUTCOME MEASURES: I

Impact of STAT6 Variants on the Response to Proton Pump Inhibitors and Comorbidities in Patients with Eosinophilic Esophagitis.

Proton pump inhibitors (PPIs) are the first-line drug for eosinophilic esophagitis (EoE), although it is estimated that there is a lack of histological remission in 50% of patients. This research aimed to identify pharmacogenetic biomarkers predictive of PPI effectiveness and to study their association with disease features. Peak eosinophil count (PEC) and the endoscopic reference score (EREFS) were determined before and after an eight-week PPI course in 28 EoE patients. The impact of the signal transducer and activator of transcription 6 (STAT6), CYP2C19, CYP3A4, CYP3A5, and ABCB1 genetic var

Targeting ATP12A, a Nongastric Proton Pump α Subunit, for Idiopathic Pulmonary Fibrosis Treatment.

Idiopathic pulmonary fibrosis (IPF) is a pathological condition of unknown etiology that results from injury to the lung and an ensuing fibrotic response that leads to the thickening of the alveolar walls and obliteration of the alveolar space. The pathogenesis is not clear, and there are currently no effective therapies for IPF. Small airway disease and mucus accumulation are prominent features in IPF lungs, similar to cystic fibrosis lung disease. The ATP12A gene encodes the α-subunit of the nongastric H+, K+-ATPase, which functions to acidify the airway surface fluid and impairs mucociliary

Omeprazole was safely reused in a rhabdomyolysis patient associated with proton pump inhibitors: A case report.

Proton pump inhibitors (PPIs) are commonly used in the clinical treatment of abnormal gastric acid secretion and gastric acid related diseases. There are disputes about blood purification and PPIs reuse in patients with PPIs-induced rhabdomyolysis. Herein we reported an 84-year-old woman with a 10-year history of coronary heart disease and gastric acid. After 18 days of omeprazole therapy, the blood myoglobin of the patient rose progressively. Laboratory examination confirmed rhabdomyolysis, and PPIs-induced rhabdomyolysis was considered. Atorvastatin was initially discontinued. Additionally,

Vitamin B12 deficiency and use of proton pump inhibitors: a systematic review and meta-analysis.

BACKGROUND: Proton pump inhibitors (PPI) may impact the absorption of vitamin B12. We performed a systematic review to ascertain if PPI use increases risk of vitamin B12 deficiency. METHODS: Electronic databases (PubMed, Embase, Scopus) were searched on first of September 2022. We selected studies that compared the frequency of vitamin B12 deficiency in PPI users and non-users. Pooled Odds Ratio (OR) was calculated for the occurrence of vitamin B12 deficiency in PPI users compared to non-users. The risk of bias was assessed using the Newcastle Ottawa scale. RESULTS: Twenty-five studies were

Oral Disintegrating Tablets of Proton Pump Inhibitors for Chronic Gastroesophageal Reflux Disease: An Update.

Oral disintegrating tablets (ODT) offer an attractive choice for Gastroesophageal Reflux Disease (GERD) patients suffering from dysphagia. In chronic condition, GERD patient suffers from severe erosive esophagitis. Thus patients feel difficulty and pain during swallowing, which results in patient in-compliance toward medication of tablets or capsules- especially in geriatrics and pediatric patients. These symptoms of GERD patients have attracted the formulation scientists in improving the formulation methodology for such patients. Orally disintegrating tablets could increase the therapeutic im

Investigation into the impact of proton pump inhibitors on sertraline transport across the blood-brain barrier.

As a widely used antidepressant that works by inhibiting the reuptake of serotonin, sertraline exerts an antidepressant effect depending on its concentration in the brain, which might be limited by the blood-brain barrier (BBB). It is highly possible to combine proton pump inhibitors (PPIs) with sertraline in clinical trials. Nevertheless, the role played by PPIs in regulating the transport of sertraline across the BBB remains unclear. Here, the impact of PPIs on the distribution of sertraline in the brain and the mechanisms involved were investigated. A mouse brain distribution study showed t

Effect of Proton Pump Inhibitor Treatment in "PPI Non-responsive" Patients with Eosinophilic Esophagitis.

BACKGROUND AND AIMS: Some eosinophilic esophagitis (EoE) patients can have a decline in eosinophil count after proton pump inhibitor (PPI) treatment without achieving histologic response, but little is known about this group. We aimed to determine the effect of PPIs on reducing esophageal eosinophilia in patients deemed non-responsive to PPI therapy. METHODS: We analyzed prospectively collected cohort data from newly diagnosed adults with EoE who were histologic non-responders (≥15 eos/hpf) to PPI-only therapy. Symptoms, endoscopic histologic features were assessed before and after PPI. Pre-
