Application of the Intracranial Arterial Pulsatility Index for Determination of Prognosis after Lacunar Infarct.

BACKGROUND: We aimed to investigate the usefulness of intracranial arterial pulsatility index to assess the prognosis of lacunar infarcts. MATERIALS AND METHODS: Forty-nine patients with confirmed acute lacunar infarct were enrolled in the study. A transcranial color-coded sonography was performed to assess the pulsatility index of bilateral middle cerebral, posterior cerebral, vertebral, and proximal internal carotid arteries. Patients' clinical status was assessed using a modified Rankin scale. Spearman correlation was used for reporting the relation between quantitative data. Statistical s

Stepwise Incisionless Percutaneous Flow-Control Treatment of a High-Flow Scalp Arteriovenous Fistula with a Large Superficial Venous Pouch: A Case Report.

OBJECTIVE: Scalp arteriovenous fistulas (AVFs) with multiple tortuous superficial feeders and a large venous pouch can require meticulous flow control during treatment. Open surgical excision may be associated with blood loss, wound-healing problems, and cosmetic concerns, whereas percutaneous liquid embolization of a high-flow superficial shunt carries a risk of nontarget migration and scalp necrosis. We report an incisionless, stepwise percutaneous flow-control strategy for such a lesion. CASE PRESENTATION: A man in his 50s presented with a progressively enlarging, pulsatile right temporal

Superconducting nanowire single-photon sensing of cerebral blood flow.

Significance: The ability of diffuse correlation spectroscopy (DCS) to measure cerebral blood flow (CBF) in humans is hindered by the low si … e SNSPD-DCS as compared to typical DCS. At this separation, the SNSPD-DCS is able to detect a clean pulsatile flow signal at 20 Hz in all subjects. With the SNSPD-DCS, we also performed measurements at 35 mm, showing a lower scalp sensitivity of 31 ± 6 % with respect to the 48 ± 8 % scalp sensitivity at 25 mm for both the 850 and 1064 nm systems. Furthermore, …

The spectrum of complications following left ventricular assist device placement.

INTRODUCTION: Left ventricular assist device (LVAD) support is associated with many complications, but relatively few studies have examined the full spectrum of complications beyond infectious and bleeding events. METHODS: We conducted a retrospective review of patients receiving either a pulsatile-flow Heartmate XVE (HM1; Thoratec Corp., Pleasanton, CA, USA) or continuous-flow Heartmate II (HM2; Thoratec Corp.) LVAD at our institution (June 2000 to March 2012). Frequency and date of onset of nonbleeding, noninfectious complications were examined. RESULTS: One hundred eighty-two LVADs were i

[High flow priapism following a straddle-injury-induced arteriocavernosal fistula: a case report].

A 26-year-old man with high flow priapism after blunt perineal trauma, is described herein. Patient evaluation included intracavernal blood-gasometry, cavernography, color flow Doppler sonography. The blood-gasometry showed pH 7.413, pO2 77.9 mmHg, pCO2 41.0 mmHg, HCO2- 26.1 mmol/L, BE 2.0 mmol/L. By direct cavernosography, pooling of contrast agent was seen at the root of the penis. Color flow Doppler sonography revealed pulsatile, turbulent flow within left corpus cavernosum. Our case was diagnosed as high flow priapism from these findings. Detumescence was not achieved by an alpha-adrenergi

Importance of ductus venosus Doppler assessment for fetal outcome in cases of intrauterine growth restriction.

OBJECTIVE: The measurement and evaluation of ductus venosus (DV) blood flow velocity waveform in high-risk pregnancies has been studied intensively in recent years in order to find a more intermediate signal of fetal compromise. Our objective was to study the fetal outcome of pregnancies with intrauterine growth retardation (IUGR) and normal pulsatility of DV compared to an IUGR group with increased DV pulsatility. METHODS: The outcome of 42 fetuses before 32 weeks of gestational age without chromosomal or structural aberrations was analyzed. All fetuses showed IUGR <5th percentile based on p

Preliminary experimental study about the feasibility of combining pulsatile cardiopulmonary support system and a membrane oxygenator.

A cardiopulmonary support (CPS) device that incorporated a pneumatic ventricular assist device (VAD) and a membrane oxygenator was developed for the support of patients with profound heart and/or respiratory failure. This device has an advantage of being both a pulsatile assist device and membrane oxygenator. A "triple flow" regulator was included in this system to control the blood flow through the oxygenator. The purpose of this study was to clarify the efficacy of this system in supporting an animal model with combined cardiac and respiratory failure. In vitro tests showed 3.7 L/min of pump

MR imaging of a false carotid aneurysm.

Two months after a gunshot wound to the head of a 12-year-old boy, MR imaging outlined an asymptomatic false aneurysm of the carotid artery. The scan's ability to image in multiple planes with noninvasive visualization of pulsatile vessel flow suggests its use for follow-up examinations of traumatic injuries to the head or neck.

Phasic hemodynamics and reverse blood flows in the aortic isthmus and pulmonary arteries of preterm lambs with pulmonary vascular dysfunction.

Time-domain representations of the fetal aortopulmonary circulation were carried out in lamb fetuses to study hemodynamic consequences of congenital diaphragmatic hernia (CDH) and the effects of endothelin-receptor antagonist tezosentan (3 mg/45 min). From the isthmic aortic and left pulmonary artery (PA) flows (Q) and isthmic aortic, PA, and left auricle pressures (P) on day 135 in 10 controls and 7 CDH fetuses (28 ewes), discrete-triggered P and Q waveforms were modelized as Pt and Qt functions to obtain basic hemodynamic profiles, pulsatile waves [P, Q, and entry impedance (Ze)], and P and

Transcranial Doppler ultrasound in evaluating cerebral blood flow abnormalities in major depressive disorder.

Previous research has shown that blood flow abnormalities affect major depressive disorder (MDD) from multiple perspectives. Therefore, this study aims to investigate the relationship between middle cerebral artery (MCA) blood flow velocity parameters and clinical symptom scores (Hamilton Depression Rating Scale [HAMD] and Montgomery-Åsberg Depression Rating Scale [MADRS]) in patients with MDD. We compared the MCA blood flow velocity parameters, including peak systolic velocity (MCA-PSV), end-diastolic velocity (MCA-EDV), and mean velocity (MCA-MV), between 50 MDD patients and 50 control subje

Evaluation of clitoral blood flow by color Doppler ultrasonography.

The feasibility of reliably measuring clitoral blood flow using standard color Doppler ultrasonography was evaluated by two independent assessors in a sample of 40 pre- and post-menopausal women. High positive correlations with no significant mean differences between examiners were found for three major standard blood flow measures including maximum velocity, resistance, and pulsatility indices. With further methodological refinements, ultrasonographic evaluation of clitoral blood flow will be adaptable for clinical and research use and is likely to become the standard physiological measure of

Cerebral vasoreactivity in HeartMate 3 patients.

BACKGROUND: While rates of stroke have declined with the HeartMate3 (HM3) continuous- flow (CF) left ventricular assist device (LVAD), the impact of non-pulsatile flow and artificial pulse physiology on cerebrovascular function is not known. We hypothesized that improved hemodynamics and artificial pulse physiology of HM3 patients would augment cerebrovascular metabolic reactivity (CVR) compared with HeartMate II (HMII) CF-LVAD and heart failure (HF) patients. METHODS: Mean, peak systolic and diastolic flow velocities (MFV, PSV, MinFV, respectively) and cerebral pulsatility index were determi…

The relationship between maternal uterine artery doppler ultrasound during the second and third trimesters and infant cognitive development at one year of age: Findings from a South African prospective cohort study.

INTRODUCTION: Increased resistance to uterine artery blood flow is an index for poor pregnancy outcomes. This study aimed to examine the association between maternal uterine artery pulsatility index and cognitive development in infants aged one year, and whether placental dysfunction moderates this relationship. METHODS: This was a prospective cohort study in an economically deprived community in South Africa. 1297 pregnant women with singleton gestations and their term infants were assessed. uterine artery pulsatility index, assessed by Doppler ultrasound in the second and third trimesters o

Physiologic and hematologic concerns of rotary blood pumps: what needs to be improved?

Over the past few decades, advances in ventricular assist device (VAD) technology have provided a promising therapeutic strategy to treat heart failure patients. Despite the improved performance and encouraging clinical outcomes of the new generation of VADs based on rotary blood pumps (RBPs), their physiologic and hematologic effects are controversial. Currently, clinically available RBPs run at constant speed, which results in limited control over cardiac workload and introduces blood flow with reduced pulsatility into the circulation. In this review, we first provide an update on the new ch

Changes in maternal posterior and anterior cerebral artery flow velocity during pregnancy and postpartum--a longitudinal study.

BACKGROUND: To evaluate the normal range of blood flow velocity in the maternal anterior (ACA) and posterior cerebral arteries (PCA) along the normal pregnancy and postpartum period. METHODS: Transcranial Doppler ultrasound was used to measure the systolic, diastolic, and mean blood velocities in the ACA and PCA during normal gestation. The resistance and pulsatility indices were calculated. Data were analyzed using multilevel modeling, incorporating random effects models, to construct mean and percentile curves. RESULTS: We performed 355 measurements on 59 patients, which showed that systol

An In Vitro Study of Embolus Migration in an Anatomical Cerebrovascular Model.

Acute ischemic stroke (AIS) remains a driver of mortality and long-term impairment, with clinical outcomes depending strongly on the final lodging site of emboli within the cerebrovasculature. However, the physical mechanisms governing embolus migration and partitioning remain incompletely understood. Further, due to the paucity of experimental data in realistic anatomical models under pulsatile flow conditions, computational modeling of embolus migration in AIS remains largely unvalidated. The study objective is to acquire experimental benchmark data of embolus migration in an in vitro anatom…

Strategies to predict and decrease flow rate pulsation in a rotary peristaltic pump with novel tube design.

Peristaltic pumps are commonly used in medical applications like cardiopulmonary bypass and drug infusion due to their minimal contact with bio-fluids and precise flow control. However, these pumps generate inherent flow pulsations, which can cause backflow or strain on the fluid, potentially leading to issues like hemolysis in blood. This study investigated the mechanics of pulsation in the output mass flow rate of peristaltic pumps and explored strategies for reduction. Using an optimized tube geometry, 3D fluid-structure interaction simulations were conducted under realistic conditions. Tra
