Covering small defects on the weight bearing surfaces of the foot: the free temporal fasciocutaneous flap.

Although defects in the weight bearing area of the heel can be covered by local flaps, radiodermatitis is a contraindication to these flaps. Thin free flaps, as grafted fascial or muscles flaps and thin fasciocutaneous flaps, are usually the option of choice in these particular defects. These reconstructions are prone to shearing strains resulting in ulceration, hypertrophic scars and hyperkeratosis. The authors present a retrospective study of the reconstruction of six small heel defects with the fasciocutaneous temporal free flap performed between 1996 and 2001. The mean size of the defect w…

Mechanism-Inspired Biomaterials and Regenerative Therapies for Radiation-Induced Skin Injury.

Radiation-induced skin injury (RISI) represents the most common complication of cancer radiotherapy, severely impairing patient health and treatment adherence. Conventional therapies show limited benefits due to the complex pathological mechanisms of RISI, creating an urgent demand for more effective approaches. Emerging biomaterials and regenerative therapies offer new opportunities for intervention. This review first outlines the key mechanisms of RISI and then highlights the mechanism-driven biomaterial design, with particular focus on the rational selection of active ingredients and advanc

Nanotechnology for the Treatment of Radiation Dermatitis: Advances and Translational Perspectives.

Radiation dermatitis is the most common complication of radiotherapy, but conventional treatments only alleviate some symptoms, with limited efficacy and safety, thus necessitating the exploration of novel therapeutic strategies. This review synthesizes current understanding of radiation dermatitis pathogenesis, and highlights recent advances enabled by nanomaterial-based strategies. We categorize nanomaterials according to their mechanisms of action and pathological stage-specific application in radiation dermatitis. Fullerene alcohol, a representative direct antioxidant material-has demonstr

Amelioration of Acute Radiation Dermatitis in Breast Cancer Patients by a Bioadhesive Barrier-Forming Gel (Episil): A Single-Center, Open, Parallel, Randomized, Phase I/II Controlled Trial.

Episil is a bioadhesive barrier-forming liquid gel that can relieve mucositis caused by radiotherapy (RT) and effectively relieve pain. The purpose of this trial is to compare the efficacy and safety of Episil in improving acute radiation dermatitis (ARD) in patients with breast cancer. This study included patients who met the criteria for postoperative RT for breast cancer. The primary end point was the grade of RD during treatment. A total of 102 patients were included in this study. The patients were grouped in a 2:1 ratio using the randomized number table method: 67 patients received Episi

An Experimental Model of Proton-Beam-Induced Radiation Dermatitis In Vivo.

Radiation dermatitis (RD) is one of the most common side effects of radiation therapy. However, to date, there is a lack of both specific treatments for RD and validated experimental animal models with the use of various sources of ionizing radiation (IR) applied in clinical practice. The aim of this study was to develop and validate a model of acute RD induced using proton radiation in mice. Acute RD (Grade 2-4) was obtained with doses of 30, 40, and 50 Gy, either with or without depilation. The developed model of RD was characterized by typical histological changes in the skin after irradiat

Impact of a mobile application (reminder app) on acute toxicity during radiotherapy of head-and-neck cancer - results of a randomized phase III trial (RAREST-02).

BACKGROUND: Radiotherapy of head-and-neck cancer (SCCHN) is often associated with acute toxicity. In a previous trial, daily reminders by staff members to perform skin care resulted in less dermatitis. This randomized trial investigated whether a mobile application can replace these reminders. METHODS: Patients were stratified according to tumor site, treatment and center. Fifty-three patients were eligible for per-protocol-set (25 with, 28 without app). Primary endpoint was grade ≥ 2 dermatitis until 60 Gy. Secondary endpoints included dermatitis grade ≥ 2 until end of radiotherapy (EOT), de

Radiation recall myositis with pazopanib in a patient with soft tissue sarcoma.

INTRODUCTION: Radiation recall reaction is an acute inflammatory reaction confined to previously irradiated areas that are most commonly triggered by the administration of anti-cancer agents after radiotherapy. Radiation recall myositis is a relatively rare form of radiation recall reaction. CASE REPORT: Here we report a 29-year-old female patient who suffered from metastatic monophasic synovial sarcoma. 8.5 months after post-operative radiotherapy of the right thigh region, the patient suffered pain, edema, redness, and increased temperature locally on the right thigh. Physical exam showed r

Ceftriaxone-induced radiation recall dermatitis.

BACKGROUND: Radiation recall dermatitis (RRD) is an acute inflammatory skin reaction occurring in a skin area previously exposed to radiotherapy and triggered by subsequent intake of a drug, most commonly a chemotherapeutic agent. RRD secondary to antibiotics has also been reported but is a rare phenomenon overall and there are no reports of RRD in association with ceftriaxone exposure. METHODS: We report on a 59-year-old patient who had received radiotherapy to the neck bilaterally and who developed RRD 6 months later after a single dose of intramuscular ceftriaxone. RESULTS: The patient's

Prophylaxis of Radiation-Induced Dermatitis in Patients With Breast Cancer Using Herbal Creams: A Prospective Randomized Controlled Trial.

Radiation-induced toxicity is a major limiting factor for prescribing radiation dose in cancer radiotherapy. Skin reaction to radiation is one of the primary concerns, which could affect quality of life of the patients both physically and mentally. Reviews of the literature show limited number of effective reagents for its prophylaxis. In this study, we attempted to determine whether prophylactic treatment of the 3 different herbal creams containing Centella asiatica, Cucumis sativus, and Thunbergia laurifolia extracts as well as a commercial moisturizing cream could reduce acute skin reaction

Surface dose and acute skin reactions in external beam breast radiotherapy.

The biologically relevant depth for acute skin reactions in radiotherapy is 70 µm. The dose at this depth is difficult to measure or calculate and can be quite different than the dose at a depth of as little as 1 mm. For breast radiotherapy with medial and lateral tangential beams, the skin dose depends on both the contribution from the entrance beam and the exit beam. The skin dose has been estimated in a breast model hemi-ellipse accounting for field size, beam energy, obliquity, lack of backscatter, fractionation, size and shape of the hemi-ellipse. The dose has been held constant along the

Fluoroscopy-induced chronic radiation dermatitis: a report of three cases.

We report these cases to emphasize the importance of recognizing fluoroscopy as a cause of radiation dermatitis. The diagnosis of fluoroscopy-induced chronic radiation dermatitis should be raised when patients present with morpheaform, telangiectatic, or ulcerative skin findings in the characteristic locations.

The use of sodium hyaluronate for the treatment of radiation recall dermatitis.

Hyaluronic acid (HA) is a high-molecular weight polysaccharide, a glycosaminoglycan that enhances fibroblast movement and metabolism during wound healing, increasing collagen fibers forming granulation tissue. We describe a 42-year-old female, who developed radiation recall dermatitis following an allogeneic stem cell transplant from an unrelated donor (MUD-PBSCT). Her skin was treated with a topical sodium hyaluronate gel twice daily (RadiaplexRX, MPM Medical). There was marked improvement after 5 days of therapy without discontinuation of any of her therapeutic agents. At a time when the hea

[Sunlight-induced granulomatous reaction to permanent lip liner].

The demand for permanent make up and especially permanent lip liner has increased in recent years in Germany. Information about complications is not available in the medical literature, because tattooing is a trade whose results and problems are not monitored and documented, as is the case for medical procedures. A female patient with permanent lip liner experienced no complications for one year. Then, after intensive sun exposure she developed solid, linear papules along the outer margin of her lips. Histology revealed a granulomatous tissue reaction with a marked lymphohistiocytic infiltrate

Radiation recall: a well recognized but neglected phenomenon.

INTRODUCTION: Radiation recall is an inflammatory skin reaction at a previously irradiated field subsequent to the administration of a variety of pharmacologic agents. Although skin has been the major site of radiation recall toxicity, instances involving other organ have been reported. MATERIALS AND METHODS: Data for this review were identified by searches of Medline and Cancerlit. The search terms "radiation", "recall", and "toxicity" were used. References identified from within retrieved articles were also used. There was no limitation on year of publication and no abstract forms were incl
