Intumescent white cataracts are challenging because of the high risk of intraoperative complications and poor outcomes. Several techniques have been proposed to mitigate altered lenticular dynamics; however, they require surgical expertise or additional costly devices. Here, we report the safety and outcomes of a recently described bimanual decompression technique. We retrospectively reviewed consecutive cases of intumescent cataract between January 2020 and July 2023. We assessed the rates of intraoperative complications, post-op refractive outcomes, and deviations from predicted values. Fift

Small incision lenticule extraction (SMILE) is a "flapless" keratorefractive surgery with excellent safety, efficacy, stability, and predictability for myopia correction. A recent global multicenter study also reported good refractive outcomes for hyperopic SMILE. SMILE has shown advantages including improved biomechanical strength, fewer dry eye symptoms, less corneal denervation, and fewer surgery-induced higher-order aberrations over laser in situ keratomileusis (LASIK). However, night vision complaints, including glare, halos, and starbursts, could still occur after SMILE. These symptoms h

The cornea is a crucial refractive medium in the visual system, characterized by its avascular and transparent nature under physiological conditions. Pathological factors such as ocular trauma and inflammation can induce the formation of new capillaries from the limbal vascular network, leading to corneal neovascularization (CNV). With an associated blindness rate as high as 12%, CNV has become a significant challenge in ophthalmology. Current treatment strategies for CNV primarily include drug therapy such as anti-inflammatory and antiangiogenic agents, as well as surgical procedures like las

Macular degeneration can result in legal blindness among those with progressive myopia and posterior staphyloma. Programmed photoreceptor death, apoptosis, is operative in some. Electrophysiologic and psychophysiologic techniques can permit detection of functional abnormalities before lesions become clinically apparent. Macular choroidal neovascularization occurs more often in those with moderate staphyloma than in those with advanced atrophy in the posterior pole. Indocyanine green angiography is improving our understanding of this complication, which has been correlated with an increased num

PURPOSE OF REVIEW: Corneal refractive surgery (CRS) is one of the most popular eye procedures, with more than 40 million cases performed globally. As CRS-treated patients age and develop cataract, the number of cases that require additional preoperative considerations and management will increase around the world. Thus, we provide an up-to-date, concise overview of the considerations and outcomes of cataract surgery in eyes with previous CRS, including surface ablation, laser in-situ keratomileusis (LASIK), and small-incision lenticule extraction (SMILE).
RECENT FINDINGS: Challenges associate

OBJECTIVE: Refractive errors may invalidate the good results of penetrating keratoplasty (PK). The Authors evaluate the effectiveness of excimer laser in situ keratomileusis (LASIK) in the correction of refractive error after PK.
MATERIALS AND METHODS: Four patients, a 26-year-old woman, a 54-year-old man, a 19-year-old man, and a 51-year-old woman, showed refractive errors: -11 = -4.5 x 85 ; -8, -4.5 = -11 x 95 ; and -4.5 = -4 x = 1200, with a clear graft at least 20 months after penetrating keratoplasty secondary to keratoconus. However, they underwent the LASIK procedure with a nasal-hinge

In the past two years 29 patients (ages 9 months to 15 years) received 37 epikeratophakia grafts. 91.1 percent of the original surgeries were successful. The average change in refractive error was 16.7 diopters, and the average spectacle overcorrection was +0.2 diopters. We have obtained good visual results in our very young patients were follow-up is already over 1 year. We demonstrated also some improvement in visual acuity in older children especially those with traumatic or secondary cataracts. The study demonstrates that we can add an average of +17.1 diopters with an epikeratophakia graf

We present the case of a 57-year-old woman who had combined Descemet-stripping automated endothelial keratoplasty (DSAEK) and phacoemulsification with implantation of a toric intraocular lens (IOL). Surgery was intended to treat a cataract developing in a post-penetrating keratoplasty (PKP) eye with high astigmatism and endothelial decompensation. Six months after uneventful surgery, the cornea was clear and the corrected distance visual acuity was 20/20 with a refraction of +0.25 -1.00 × 10 (from -3.00 -8.50 × 12 preoperatively). The internal topography map (OPD-Scan) showed an IOL rotation o

Sixty-five epikeratophakia procedures have been performed in 63 patients; visual acuity data have been tabulated on 31 patients with 4 to 30 months follow-up. Patients with more than a year of follow-up showed stable keratometry readings. Early patients achieved 70% of the predicted dioptric correction; more recent patients have achieved 87% with improvements in the lathing procedure, tissue handling, and surgical technique. Visual acuities improve with time. At any given time after surgery, acuities measured with a hard contact lens are better than those measured with spectacles; the decrease

INTRODUCTION: Transepithelial photorefractive keratectomy (t-PRK) is an updated maneuver based on the proven surgical procedure of conventional photorefractive keratectomy (PRK) for the correction of ametropia. Unlike conventional PRK, which is a two-step process involving the physical removal of the corneal epithelium prior to excimer laser treatment of the stroma, t-PRK uses the excimer laser to complete both activities in a single-step procedure.
METHODS: This was a prospective, randomized, controlled, contralateral, masked, single-surgeon study of adult subjects with myopia with a manifes
