A retrospective study of advancement genioplasty using a special bone plate.

This study evaluates skeletal stability and the remodeling process of the advanced genial segment when a single bone plate is used to stabilize the segment following osteotomy of the inferior border of the mandible. Thirty-nine patients with a minimum of 6 months follow-up who had advancement genioplasty stabilized with a Paulus chin plate were analyzed using cephalometrics and clinical examination. The results showed that stability of the result was excellent; pogonion maintained its immediate postsurgical horizontal position at longest follow-up. The remodeling pattern observed was similar t

Induced condylar growth in a patient with hemifacial microsomia.

Hemifacial microsomia is a congenital abnormality which results in progressive, three-dimensional facial asymmetry in the growing child. Functional appliance therapy was instituted in an 11 year, 7 month old boy with hemifacial microsomia in order to determine if symmetry could be re-established by promoting masticatory muscle function on the affected side, thereby stimulating bone growth in the affected condyle over and above what would occur without any treatment intervention. Removable functional appliance therapy, and later, unilateral Herbst appliance therapy, produced a dramatic change i

The effect of treatment with functional appliance on a pathologic growth pattern of the condyle.

Hemifacial microsomia causes asymmetry of the face frequently known to progress throughout the postnatal development if not submitted to any kind of treatment. According to the theoretic basis for treatment of hemifacial microsomia as presented by Harvold and associates, generation of the right muscle-bone interaction constitutes the necessary precondition for the bone apposition that produces facial symmetry. This theory was the basis for the treatment of three patients with abnormal condyles, one suffering from hemifacial microsomia, one patient with a unilateral condylar fracture followed b

Nonsurgical correction using miniscrew-assisted vertical control of a severe high angle with mandibular retrusion and gummy smile in an adult.

Orthodontic treatment in adult patients with a skeletal discrepancy can be challenging. In this case report, we achieved both sagittal and vertical control by combining the classic sliding mechanics straight-wire technique with miniscrew anchorage. We treated a 21-year-old Chinese woman with a severe high mandibular plane angle, a retrusive chin, and a gummy smile. Her diagnosis included a skeletal Class II skull base with a mild anterior open bite, a protrusive maxilla, and a backwardly rotated mandible. This case underscores the importance of anchorage control in both the sagittal and vertic

A Geometric Classification of Jaw Deformities.

In the United States, the most widely used classification system for jaw deformities is the one provided by the International Classification of Diseases, Clinical Modification (ICD-CM), a taxonomy scheme that is based on the World Health Organization's International Classification of Diseases (ICD). The last iteration of ICD-CM, version 10, sorts jaw deformities according to geometry, into 3 groups: anomalies of jaw size, anomalies of jaw-cranial base relationship, or unspecified. Yet these deformities can affect 6 different geometric attributes: size, position, orientation, shape, symmetry, a

Treatment of skeletal class II division 1 malocclusion with mandibular deficiency using myofunctional appliances in growing individuals.

Class II division 1 malocclusion is the most common malocclusion seen in day-to-day practice. The majority of the patients with class II division 1 malocclusions have the presence of underlying skeletal discrepancy between maxilla and mandible. The treatment of skeletal class II division 1 depends upon the age of the patient, growth potential, severity of malocclusion, and compliance of patient with treatment. Myofunctional appliance can be successfully used to treat growing patients with class II division 1 malocclusion having retrusive mandible. This article presents a discussion on treatmen

45,X/46,XX karyotype mitigates the aberrant craniofacial morphology in Turner syndrome.

The aim of this project was to study the impact on craniofacial morphology from Turner syndrome (TS) karyotype, number of intact X chromosomal p-arms, and age as well as to compare craniofacial morphology in TS with healthy females. Lateral radiographs from 108 females with TS, ranging from 5.4 to 61.6 years, were analysed. The TS females were divided into four karyotype groups: 1. monosomy (45,X), 2. mosaic (45,X/46,XX), 3. isochromosome, and 4. other, as well as according to the number of intact X chromosomal p-arms. The karyotype was found to have an impact on craniofacial growth, where the

Kocher Debre Semelaigne syndrome and associated orofacial aspects: report of a case.

Myopathy and muscular pseudohypertrophy associated with severe congenital hypothyroidism has been described as Kocher Debre Semelaigne syndrome, which is a rare disorder with only a few previously published reports. However, none of these reports have described the orofacial aspects of this syndrome. Here we report one such case, which may provide valuable information to specialists in oral healthcare regarding the associated orofacial aspects.

Observer consistency in radiographic assessment of condylar resorption.

OBJECTIVES: This study evaluates interobserver and intraobserver agreement with respect to the determination of the shape and surface of the condylar head on panoramic views. STUDY DESIGN: Detailed frames (n = 632) were made from preoperative and postoperative panoramic views of 158 consecutive patients, who underwent orthognathic surgery. The intraobserver and interobserver agreement were calculated by using Cohen's kappa. RESULTS: By using Cohen's kappa, the intraobserver agreement was determined to be reasonable or good for surface and shape of the condyle. The interobserver agreement was

[Experiences with distraction osteogenesis in therapy of severe peripheral airway obstruction in infancy and early childhood].

Children with craniofacial malformations are at special risk for the development of peripheral airway obstruction. The problems are magnified in patients with retroposition or hypoplasia of the mandible. In these cases, the base of the tongue is posteriorly displaced, hereby decreasing the airway diameter. By application of distraction osteogenesis the mandible can be lengthened to move the base of the tongue forward and open the airway. Three female patients aged between 7, 11, and 15 months suffering from peripheral airway obstruction caused by mandibular hypoplasia were treated by gradual d

A dominant-hereditary variation of the Pena-Shokeir syndrome; a case report.

Four children with the Pena-Shokeir syndrome are described. Apart from the classical manifestations of the syndrome: camptodactyly, ankyloses, facial anomalies, pulmonary hypoplasia, all four children had an extremely rigid skin. Possibly this rigidness of unknown origin led in these cases to fetal akinesia, which seems to be the explanation of the clinical manifestations of the syndrome. All children were born to one man, three in his first marriage, one in his second. Therefore this case report almost certainly presents a dominant-hereditary variation of the syndrome.

Sensory impairment following sagittal split osteotomy for correction of mandibular retrognathism.

Sagittal split osteotomy of the mandibular ramus is performed in close proximity to the inferior alveolar nerve and may result in postoperative neurosensory disturbances. Intraoperative strain on the nerve and other complications in 25 patients undergoing bilateral sagittal split osteotomy were recorded. Neurosensory testing was carried out before and after surgery. The patients reported sensory disturbances in 54% of sites 4 days postoperatively and 42% and 34% of sites at 9 weeks and 6 months, respectively. Objective assessments showed an incidence of 34% at 4 days, 20% at 9 weeks, and 8% at
