The etiology of retroperitoneal fibrosis (Ormond's disease) has still not been clarified. A new hypothesis postulates chronic fibrosing periaortitis. Atherosclerosis of the abdominal aorta and plaque rupture lead to the liberation of oxidized lipoproteins through the adventitia into the periaortic fatty tissue. Case histories of three male patients diagnosed as idiopathic retroperitoneal fibrosis either on autopsy or following intra-operative biopsy are reported. Two of them were in the 5th decade and one aged 22 yrs. All of them presented with features of urinary tract obstruction and renal failure. The obstruction was detected either by ultrasonography or following ascending pyelogram. One of them underwent peritoneal dialysis but later on died of sudden cardiac arrest. The authors performed intraperitoneal transposition of the right ureter in a patient who suffered from dilatation of the renal cavity-system due to retroperitoneal fibrosis. One and a half year later an angiomyolipoma was eliminated from the left kidney of the patient. Describing the associated occurrence of these 2 infrequent diseases the authors deal with the problem of etiology, diagnosis and treatment.

We have performed CT and MR on five patients with biopsy proven retroperitoneal fibrosis (RPF). Magnetic resonance (MR) accurately displayed a retroperitoneal mass of low signal intensity on T1-weighted scans and of heterogeneous medium signal intensity on T2-weighted scans. The coronal MR views demonstrated a retroperitoneal mass: the shape, signal intensity, and effects on the ureters and major vessels appear characteristic of RPF.

Serial abdominal computed tomography (CT) scans have been performed in 15 patients with biopsy-proven idiopathic retroperitoneal fibrosis. Twelve patients had abdominal CT both before diagnosis and at a variable time later. All 12 patients showed a decrease in the size of the retroperitoneal mass at follow-up, although two had not been treated with steroids. The remaining three patients who had abdominal CT at some stage after diagnosis all showed a small residual retroperitoneal mass.
