Pseudallescheria: an underdiagnosed fungus?

Pseudallescheria has been identified as one of the "clinically significant emerging mycoses" but has received little attention in the cytology literature. Recognition of this fungus is of particular importance clinically, because unlike most other fungi (including Aspergillus, with which it is most frequently confused), Pseudallescheria is not effectively treated with amphotericin B, the most frequently and often the only antifungal agent administered. Features helpful in the diagnosis of Pseudallescheria in cytologic material are presented.

Evaluation of the nutritional quality and acceptability of fingermillet-based tempe as potential weaning foods in Tanzania.

Six types of fingermillet (Eleusine coracana)-based tempe were developed by incorporating either commonbeans, groundnuts, cowpeas, mungbeans, chickpeas, sesame and/or their mixtures and fermented by Rhizopus oligosporus. The proximate and mineral composition was not changed significantly by fermentation. The protein content of tempe ranged from 13.3 to 15.7%; the total energy, 374.1 to 434.3 Kcal/100 g; the protein energy, 13.0 to 15.6%; the net dietary protein energy, 7.2 to 8.7%; and the chemical score, 61 to 71%. Tempe processing reduced the tannin and hydrogen cyanide content by 55.2 to 75…

Bilateral rhinocerebral phycomycosis.

A 51-year-old diabetic woman developed bilateral rhinoorbitocerebral phycomycosis. Successful treatment was accomplished by correction of the ketoacidosis, surgical debridement, and amphotericin B therapy. Rhinoorbitocerebral phycomycosis is a fulminant and frequently fatal disease most often seen in debilitated hosts. This survival suggests that aggressive therapy is indicated in patients with extensive fungal involvement.

Limb Mucormycosis in the Early Postoperative Period Following Kidney Transplantation: A Case Report.

Post-transplant mucormycosis is a rare but life-threatening complication with high mortality. We report a severe case of upper limb mucormycosis in a 58-year-old diabetic kidney transplant recipient. Despite standard immunosuppression, progressive skin lesions developed on the left forearm. Initial debridement failed to arrest progression, revealing extensive muscle necrosis and vascular thrombosis. Rhizopus oryzae was confirmed by histopathology and culture. Surgical management required mid-humeral amputation followed by shoulder disarticulation due to persistent fungal invasion. Antifungal t…

Approach to the neonate with ecchymoses and crusts.

Premature and systemically ill infants have a high risk of developing dermatologic infectious complications, displaying the consequences of skin barrier immaturity. Opportunistic infections are an increasing concern in neonates, with cutaneous fungal infections (Aspergillus, Rhizopus, Mucor, Fusarium) observed more commonly as pathogens. Neonates are especially susceptible due to stresses of the perinatal transition to ex-utero life, stratum corneum immaturity, and medical intervention during early life including intravenous catheters, non-sterile adhesive dressings, broad spectrum antibiotic …

Bronchial mucormycosis with progressive air trapping.

A previously healthy 70-year-old woman developed fever, cough, and exertional dyspnea. Her symptoms progressed over a 2-month period despite … nd dynamic airway obstruction with complete closure during expiration. Biopsy of the ulcer revealed Rhizopus arrhizus. Respiratory failure stabilized with the patient on conventional mechanical ventilation and receiving amphotericin B. Before surgery could be performed, Pseudomonas aeruginosa pneumonia and septic shock developed, and the patient died.

Metagenomic next-generation sequencing for pathology-suspected fungal infections at rare anatomical sites: a case series.

OBJECTIVE: Histopathology for suspected fungal infections lacks species-level identification and is prone to morphological mimics. The utility of metagenomic next-generation sequencing (mNGS) at rare anatomical sites is underexplored. METHODS: We retrospectively analyzed 10 cases with histopathology suggestive of fungal infection at rare sites (brain, cardiac valve, bone, etc.). All underwent mNGS testing on formalin-fixed paraffin-embedded samples. RESULTS: mNGS detected fungal DNA in 8/10 cases (80%), providing species-level identification (e.g., Cryptococcus, Candida, Fusarium, Rhizopus, …

Rhino-Orbital Mucormycosis in a COVID-19 Patient: A Dreadful Challenge.

Coronavirus disease 2019 (COVID-19) is a pandemic disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-COV-2). COVID-19 infections may be associated with a wide range of bacterial and fungal co-infections. Recent studies are reporting invasive fungal infection associated with severe COVID-19. Herein, we report a case of COVID-19 rhino-orbital mucormycosis infection caused by Rhizopus sps in a 32 year old diabetic patient who was successfully managed with early aggressive debridement of infected tissue endoscopically with extended ethmoidectomy by modified Denker's approach a…

Solid-State Fermentation Reduces Phytic Acid Level, Improves the Profile of Myo-Inositol Phosphates and Enhances the Availability of Selected Minerals in Flaxseed Oil Cake.

Flaxseed oil cake was subjected to fermentation with Rhizopus oligosporus (DSM 1964 and ATCC 64063), and the phytate (InsP6) content, myo-inositol phosphate profile and in vitro bioavailability of essential minerals were studied. Flaxseed oil cake had a phytate mass fraction of 13.9 mg/g. A 96-hour fermentation of flaxseed oil cake by R. oligosporus DSM 1964 and R. oligosporus ATCC 64063 decreased the InsP6 content by 48 and 33%, respectively. The strains had different phytate-degrading activities: fermentation of flaxseed oil cake with R. oligosporus DSM 1964 was more advantageous, yielding I…

Mucormycosis. Adjunctive therapy with hydrogen peroxide.

Mucormycosis is a devastating fungal disease affecting mainly diabetic and immunosuppressed patients and frequently causing death. Mucor rhizopus, the opportunistic fungus, has been controlled via radical extirpation and intravenous Amphotericin B. Hyperbaric oxygen also has been used. The authors present two interesting patients: 1) A diabetic female with rhinoorbital mucormycosis post total maxillectomy with recurrent mucormycosis and 2) A diabetic female with acute myelogenous leukemia and sphenoid sinus mucormycosis post functional endoscopic sinus surgery with residual mucormycosis. Both …

Rhino-Orbital Cerebral Mucormycosis in a Patient With Diabetic Ketoacidosis: A Case Report and Literature Review.

BACKGROUND: Rhino-orbital cerebral mucormycosis (ROCM) is a rare, invasive, and fatal fungal disease. Due to the lack of specific clinical manifestations and adequate auxiliary examinations, patients are easily misdiagnosed in the early stage. Early diagnosis and timely therapy are essential for successful treatment. CASE REPORT: We report a 68-year-old man with diabetic ketoacidosis, presented with orbital apex syndrome (OAS), fever, and pansinusitis, which progressively worsened to death only 4 days after admission. It was finally confirmed as a fungal Rhizopus arrhizus infection by metagen…

Evaluation of cold plasma for decontamination of molds and mycotoxins in rice grain.

Few reports are on the application of cold plasma (CP) for mold and mycotoxin control in grain. The aim of this study was to evaluate the viability of CP for mycotoxin and mold control in rice grain. Rice grains artificially contaminated with molds or mycotoxins were subjected to CP treatment. The microbial activities of Aspergillus niger, Rhizopus oryzae, Penicillium verrucosum and Fusarium graminearum were significantly inhibited by CP treatment. The maximal reduction of DON and OTA were 61.25 % and 55.64 %, respectively. The electrical conductivity and malondialdehyde content in rice grain …

An atypical case report of extensive mucormycotic osteomyelitis of maxilla as a consequence of post-COVID complication.

Saprophytic molds such as Mucor, Rhizopus, and Absidia cause mucormycosis, a fungal infection. These saprophytic fungi are common in the environment and have a strong proclivity for invading major blood arteries, causing tissue ischemia, necrosis, and infarction. They have been linked to immunocompromised individuals with a history of diabetic ketoacidosis, corticosteroid medication, HIV infection, malignant lymphomas, and patients currently receiving and recovering from COVID-19 treatment. The foregoing is the case of a 78-year-old COVID-19 recovered male who presented with a primary complain…

Identification and susceptibility of Rhizomucor spp. isolated from patients with cutaneous zygomycosis in China.

Zygomycosis is a relatively uncommon mycosis with a morbidity that is increasing worldwide. Cutaneous zygomycosis, one of the clinical manifestations of the disease, has also emerged in recent decades. The major reported etiologic agents in China include Rhizomucor spp., Rhizopus spp., Mucor spp., and Lichtheimia spp. (formerly Absidia spp.). This study examined 11 clinical isolates of Rhizomucor that belong to three species (R. variabilis, R. regularior, and R. chlamydosporus). They were identified by both morphological and molecular methods and were found to have a high degree of correlation…
