Assessment of automatic rib fracture detection on chest CT using a deep learning algorithm.

OBJECTIVES: To evaluate deep neural networks for automatic rib fracture detection on thoracic CT scans and to compare its performance with that of attending-level radiologists using a large amount of datasets from multiple medical institutions. METHODS: In this retrospective study, an internal dataset of 12,208 emergency room (ER) trauma patients and an external dataset of 1613 ER trauma patients taking chest CT scans were recruited. Two cascaded deep neural networks based on an extended U-Net architecture were developed to segment ribs and detect rib fractures respectively. Model performance…

Brachial plexus injury and upper rib fracture after median sternotomy in cardiac surgery.

OBJECTIVES: Sternal retractors utilized during open-heart surgeries through median sternotomy can cause upper rib fractures which sometimes further leads to brachial plexus injury. We aimed to investigate the incidence of brachial plexus injury and upper rib fractures in open-heart surgeries and how these injuries are associated with each other. METHODS: We investigated 1050 cases during the past five years. The incidence of brachial plexus injury and upper rib fractures after median sternotomy was assessed in all patients and the patients who sustained were evaluated for the affected side, t…

Rib fracture fixation in a patient on veno-venous ECMO for severe blunt thoracic trauma.

Thoracic chest wall trauma is a common injury in patients admitted to hospital following injury and is associated with high mortality. British Orthopaedic Association Standards for Trauma and Orthopaedics guidelines recommend consideration of rib fracture fixation in patients with flail chest wall injuries with respiratory compromise or uncontrollable pain. Veno-venous extracorporeal membrane oxygenation (ECMO) can be utilised in patients with severe respiratory dysfunction and we present the case of a patient who underwent rib fracture fixation while receiving ECMO. A 32-year-old male was adm

Comparison of the use of lung ultrasound and chest radiography in the diagnosis of rib fractures: a systematic review.

INTRODUCTION: It is well-recognised that the detection of rib fractures is unreliable using chest radiograph. The aim of this systematic review was to investigate whether the use of lung ultrasound is superior in accuracy to chest radiography, in the diagnosis of rib fractures following blunt chest wall trauma. METHODS: The search filter was used for international online electronic databases including MEDLINE, EMBASE, Cochrane and ScienceDirect, with no imposed time or language limitations. Grey literature was searched. Two review authors completed study selection, data extraction and data sy…

Rib Fracture Location Should Be Evaluated When Predicting Morbidity and Mortality in Trauma Patients.

Rib fractures represent up to 55 per cent of thoracic blunt traumatic injuries and lead to significant mortality and morbidity. The aim of this study is to determine whether not only number but also the location of rib fractures can be used to risk stratify patients. This is a retrospective study of all blunt trauma patients who presented with rib fractures from January 1, 2013 to April 1, 2015 and underwent chest CT. Rib fractures were categorized by location. Primary outcome was mortality, secondary outcomes were total hospital length of stay (LOS), intensive care unit LOS, and disposition. …

Keeping an Open Mind: Cognitive Bias in the Evaluation of an Infant with Posterior-Lateral Rib Fractures.

A four-month-old former premature male is incidentally found to have posterior-lateral rib fractures during evaluation of a febrile illness. This finding led to the initiation of a workup for nonaccidental trauma. A thorough history and physical exam ultimately led to the diagnosis, which was not related to abuse. This case highlights a rare sequela of patent ductus arteriosus repair, cautions medical teams to remain aware of how cognitive bias can affect diagnostic decision-making, and emphasizes the importance of a thorough history, physical exam, and medical record review in cases of suspec…

First rib fracture and Horner's syndrome due to a motor vehicle collision: a case report.

A case of a first rib fracture and Horner's syndrome due to a motor vehicle collision is reported. Initial evaluation in a hospital emergency department and follow-up by a medical primary care physician failed to provide identification of the Horner's syndrome. Careful assessment and review of the patient's symptoms, signs and images revealed this uncommon and important neurologic case presentation. A brief discussion related to traumatic first rib fracture, Horner's syndrome and arterial dissections of the neck is provided.

Operative management of rib fractures in the setting of flail chest: a systematic review and meta-analysis.

OBJECTIVE: To perform a systematic review and meta-analysis of studies comparing operative to nonoperative therapy in adult FC patients. Outcomes were duration of mechanical ventilation (DMV), intensive care unit length of stay (ICULOS), hospital length of stay (HLOS), mortality, incidence of pneumonia, and tracheostomy. BACKGROUND: Flail chest (FC) results in paradoxical chest wall movement, altered respiratory mechanics, and frequent respiratory failure. Despite advances in ventilatory management, FC remains associated with significant morbidity and mortality. Operative fixation of the flai

"Buckle" rib fracture: an artifact following cardio-pulmonary resuscitation detected on postmortem CT.

Buckle rib fractures are incomplete fractures involving the inner cortex alone, and are rarely detected on routine chest X-ray or at autopsy. The characteristics of these fractures have not been well evaluated in situ although they are commonly observed on postmortem CT images especially following CPR. The postmortem CT findings in 42 cases showing buckle rib fractures caused by CPR were reviewed. The cause of death in all cases was non-traumatic. The shape, number, location, and distribution of these buckle rib fractures and their relationship to other types of rib fractures were evaluated us…

Rib fracture patterns predict thoracic chest wall and abdominal solid organ injury.

Blunt trauma patients with rib fractures were studied to determine whether the number of rib fractures or their patterns were more predictive of abdominal solid organ injury and/or other thoracic trauma. Rib fractures were characterized as upper zone (ribs 1 to 4), midzone (ribs 5 to 8), and lower zone (ribs 9 to 12). Findings of sternal and scapular fractures, pulmonary contusions, and solid organ injures (liver, spleen, kidney) were characterized by the total number and predominant zone of ribs fractured. There were 296 men and 14 women. There were 38 patients with scapular fracture and 19 p…

Refractory empyema caused by an intraoperative rib fracture sustained during esophagectomy: report of a case.

A 57-year-old woman underwent esophagectomy with three-field lymphadenectomy for lower esophageal cancer, followed by gastric roll reconstruction through the posterior mediastinum. A laparotomy and right thoracotomy with partial resection of the right fifth rib were performed. A purulent discharge requiring drainage developed postoperatively and continued for 3 months despite anastomotic integrity and the absence of a bronchopleural fistula. An empyema developed 3 months after drain removal, and a thoracostomy tube was reinserted. A displaced fragment of the right fifth rib was identified with

Assessing the true risk of abdominal solid organ injury in hospitalized rib fracture patients.

BACKGROUND: Despite the lack of evidence, traditional trauma teaching has suggested that low rib fractures increase the risk of abdominal solid organ injury (ASOI). This study was designed to assess if in fact this is true, and to try and define other factors that increased the risk of ASOI in rib fracture patients. METHODS: The charts of 476 hospitalized rib fracture trauma patients were reviewed. Data were collected for age; sex; Injury Severity Score (ISS); rib fracture location; and the presence or absence of injuries to the abdominal organs, head, neck, face, thorax, great vessel, heart,…

[Scintigraphic diagnosis of sport injuries: multiple fractures of ribs by golf-players (author's transl)].

Three untrained golf-players suffered from pain in the dorsal region of the left thorax after some training hours. Three to four weeks after the pains had begun serial fractures of ribs were diagnosed by 99mTc-MDP bone-scan-examinations. All fractures were localized near the costal angulus (paravertebral) of these ribs. The fractures are causal by manual tensions. The difficulties in timing the bone-scan and in identifying the morphologic substrate are demonstrated. It is recommended, that golf-players suffering from thoracic pain ought to be examined by bone scan.

Mixed Reality for Localisation of Rib Fractures Before Surgical Stabilization: A Pilot Study of an Innovative Tool.

BackgroundAccurate localisation of rib fractures is crucial for successful surgical stabilisation of rib fractures (SSRF). Mixed reality (MR) technology can overlay anatomical imaging onto a patient in real-time, which may facilitate the visualisation of rib fractures, allowing accurate skin surface markings for incision. This pilot study assessed the feasibility of using mixed reality (MR) for rib fracture localisation in SSRF.MethodsThis study involved a pre-clinical phase with two healthy patients and a clinical phase with six patients undergoing SSRF in a single tertiary trauma centre. CT …

Video-Assisted Thoracoscopic Surgery (VATS)-Aided Simultaneous Fixation of Scapular Body and Rib Fractures: A Case Report.

Scapular fractures are rare injuries, whereas rib fractures are more common. Scapular fractures are frequently associated with rib fractures, which may lead to significant respiratory compromise. While surgical fixation has demonstrated benefits for both scapular and rib fractures, simultaneous fixation using a single surgical approach is not widely practiced. A 54-year-old male sustained a right scapular body fracture (AO 14B1) and multiple right rib fractures following a crush injury. Due to severe respiratory pain and prolonged mechanical ventilation, surgical fixation was performed on post…

Rib fractures after chest compressions for cardiac arrest: retrospective analysis of the AfterROSC1 and AfterROSC2 multicenter databases.

PURPOSE: External chest compressions for resuscitation after out-of-hospital cardiac arrest (OHCA) can cause rib fractures, which are best diagnosed by computed tomography (CT). We assessed the prevalence, management, and associations with outcomes of CT-documented rib fractures in patients with OHCA. METHODS: We retrospectively analyzed data collected prospectively at five AfterROSC Network centers in 2020-2023. We included consecutive patients with return of spontaneous circulation and coma after non-traumatic OHCA who underwent CT within 6 h after admission. Rib fractures and other chest-w…
