The rotator cuff is the primary dynamic stabiliser of the glenohumeral joint and is placed under significant stress during overhead and contact sports. Mechanisms of injury include repetitive microtrauma, usually seen in the athlete involved in overhand sports, and macrotrauma associated with contact sports. Rotator cuff injury and dysfunction in the overhand athlete may be classified based on aetiology as primary impingement, primary tensile overload, and secondary impingement and tensile overload resulting from glenohumeral instability. A thorough history and physical examination are paramou

Modern arthroscopic tools and techniques have allowed surgeons to operate on a wide variety of injuries using procedures less invasive than traditional open methods. For shoulder surgery in general, and rotator cuff repairs specifically, methods now yield a similar footprint as open procedures with several advantages, including reduced tissue trauma, postoperative pain, swelling, and concern about the deltoid attachment, which should lead to good outcomes.

UNLABELLED: Synovial chondromatosis is characterized by benign synovial proliferation that leads to chondral or osteochondral foci formation. In this case report, a right-handed female suffered from progressively worsening pain and limited mobility of forward elevation, abduction and external rotation in her right shoulder. A shoulder arthroscopy was conducted, during which, thickened bursal synovium and several loose bodies were observed, associated with bursal side tear of rotator cuff. A thorough synovectomy, subacromial debridement and acromioplasty were conducted. The pathological finding

The term "biceps brachii" is a Latin phrase meaning "two-headed (muscle) of the arm." As its name suggests, this muscle has two separate origins. The short head of biceps is extraarticular in location, originates from the coracoid process of the scapula, having a common tendon with the coracobrachialis muscle. The long head of biceps tendon (LBT) has a much more complex course, having an intracapsular and an extracapsular portion. The LBT originates from the supraglenoid tubercle, and in part, from the glenoid labrum; the main labral attachments vary arising from the posterior, the anterior of

We have undertaken an in vivo assessment of the tissue metabolism and cellular activity in torn tendons of the rotator cuff. Cellular oxygen consumption was measured in 13 patients undergoing mini-open repair of small, medium, large and massive full-thickness tears. Measurements were also taken from three control patients who were undergoing open stabilisation of the shoulder with grossly normal tendons. The level of oxygen and nitrous oxide was measured amperometrically using silver needle microelectrodes at the apex of the tear and 1.5 cm from its edge. With nitrous oxide indicating the degr

The authors report a series of 38 patients who had been examined by MRI and then operated for a rotator cuff syndrome. The correlation between the description of the cuff lesions after MRI and the surgical observations were excellent for 37 patients. In one case MRI showed a false image of tear of the supra spinatus m. on its anterior edge. This was due to a bad knowledge of the anatomy of the muscle and tendon and to a poor orientation of the frontal cut plane. This study was completed with MRI and anatomic study of 12 non embalmed cadaveric shoulders. The results showed that MRI was very sen

Rotator cuff repair outcomes remain inconsistent, with high retear rates, attributed to multiple risk factors including narrow acromiohumeral distance, elevated critical shoulder angle, larger tear dimensions, fatty infiltration, increased retraction of the torn tendons, and advanced patient age (>63-70 years). Since its introduction, the Rotator Cuff Healing Index, which has been externally validated, is a useful tool for systematic risk stratification that can help guide clinical decision-making, though it should complement rather than supersede surgical judgment.

BACKGROUND: The suture-bridge (SB) technique is widely used for rotator cuff repair. However, re-tear rates remain high, suggesting that further optimization of repair constructs is necessary. The triple-row (TR) technique, which incorporates an additional middle row of fixation, has been proposed to enhance tendon stabilization, but its mechanical performance remains unclear. This study aimed to compare the mechanical properties of SB and TR repair techniques for rotator cuff tears.
METHODS: Based on the middle-row stitch technique, TR repair was classified as double-simple (TR-DS), double-h

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised

Bimanual tasks are necessary for daily activities but become difficult to complete with a musculoskeletal disorder. Rotator cuff tears (RCT) are a common musculoskeletal disorder. It is unclear how an unaffected contralateral limb compensates when performing bimanual tasks. This work used a computational model to examine the effects of a unilateral RCT on performance of loaded bimanual tasks. A bilateral upper limb model was developed in OpenSim (v3.3), then modified to represent 4 RCT severities ranging from no RCT to massive RCT. A shared load (13.3 N or 44.5 N) was added to the models to re

In terms of rotator cuff repair, there is a goal for complete repair and healing, as rotator cuff integrity correlates with clinical and functional results. Retear has been shown to have a significant influence on progression toward osteoarthritis, and patients with an intact supraspinatus show superior abduction and flexion strength. However, in cases where complete repair may not be possible and/or cost limitations may prohibit augmentation, partial repair can provide a respectable outcome. Furthermore, regardless of healing status, partial repair may provide satisfactory relief for patients
