Scaphoid fracture is the most common fracture in carpal bone of the wrist and represents 11% of all upper extremity fractures. In most cases, the mechanism of injury is a fall with an outstretched hand. Scaphoid fracture detection can be made by clinical examination and standard radiography. Computed tomography (CT), bone scan and MRI improve the sensitivity of the diagnosis. Non-displaced fractures of the scaphoid are treated with cast immobilization in most cases. Operative treatment should be considered in displaced fractures or non-displaced fractures in young active patients by using inte

Background: Several operative treatments exist for scaphoid fractures, varying by approach (e.g., ercutaneous, volar, or dorsal), implant type (e.g., screw or Kirschner wire), and bone raft choice (e.g., none, nonvascularized, or vascularized). Many previous systematic eviews and meta-analyses have investigated outcomes following different surgicalÚpproaches, the use of vascularized versus nonvascularized bone graft for scaphoidßracture nonunions, and treatment for specific fracture patterns. However, given the advancements n scaphoid fracture treatment in recent years, there is a need for upd

PURPOSE: We hypothesised that intercarpal K-wire fixation of adjacent carpal bones would reduce torque and lever force within a fractured scaphoid bone.
METHODS: In eight cadaver wrists, a scaphoid osteotomy was stabilised using a locking nail, which also functioned as a sensor to measure isometric torque and lever forces between the fragments. The wrist was moved through 80% of full range of motion (ROM) to generate torque and force within the scaphoid. Testing was performed with and without loading of the wrist and K-wire stabilisation of the adjacent carpal bones.
RESULTS: Average torque …

An overview about current concepts in treating carpal injuries is presented. These injuries are more commonly seen in young, active individuals after a fall on an outstretched hand. Conventional radiographs and a thorough examination are important. The scaphoid is the most affected bone. Scaphoid fractures can be classified in accordance to OTA, AO, and other classification systems, but mostly to Herbert. It can be treated non-operatively if undisplaced, however a percutaneous internal fixation can be discussed to achieve earlier return to work and shorter time to union, but hazarding the cons

Fracture of the scaphoid is the most common injury of the carpal bones. Most scaphoid fractures heal after immobilization. When the fracture does not unite, degenerative arthritic changes and disability may occur. Twenty-four symptomatic nonunions treated surgically were reviewed. Based on presence of union, percent of normal motion, and patients' symptoms, results were rated good in 19 cases and fair in five. Nine of ten patients treated with bone grafting and styloidectomy had union of fractures with good results. All five patients with bone grafting and internal fixation had union. Excision

OBJECTIVE: Reconstruction of the scaphoid with use of NITINOL shape "memory" staples.
INDICATIONS: Unstable fractures and nonunion of the middle third of the scaphoid, which need open reduction and internal fixation from palmar. The staples can only be used, if the arms of the staples can be inserted parallel to and at a distance of 3 mm to the fracture line.
CONTRAINDICATIONS: Allergy to nickel. Cases in which the arms of the staple cannot be inserted parallel to and at best 3 mm apart from the fracture line.
SURGICAL TECHNIQUE: In fractures, open reduction of the scaphoid through a palmar

A rare injury of the wrist, scapho-capitate fracture syndrome, in a young patient is reported. Despite early recognition of the injury and surgical intervention, the scaphoid fracture did not unite and another attempt to achieve union with bone grafting and internal fixation also failed. The wrist continued to be painful and stiff. Radiographs of the wrist, 18 months after the injury, showed nonunion of the scaphoid, avascular necrosis of the scaphoid and the lunate and carpal collapse with midcarpal joint arthritis. Due to persistent and disabling symptoms arthrodesis of the wrist had to be c

BACKGROUND: Complex fractures of the carpal scaphoid with poles fragmentation, edges comminution, bone loss and non-union of fractures previously treated by screw fixation remain challenging for hand surgeons. The authors present the indications, advantages and results of scaphoid plating, underlining the importance of correct plate positioning well shaped onto the bone.
MATERIALS AND METHODS: The study includes 11 patients presenting acute fracture with distal pole fragmentation, acute fracture with comminution and non-union after prior failure of screw fixation. All patients were treated wi

Scaphoid fracture and healing are often difficult to diagnose. The scaphoid bone may heal in an abnormal position and this malunion is usually in flexion and shortening. Fractures of the mid or distal part of the bone are mainly concerned. This malunion may be the cause of an alteration of the carpal mechanics and finally of osteoarthritis revealed by pain and decrease of motion and strength. The intrascaphoid angle of Amadio helps to calculate the magnitude of the deformity and to plan the correction. This malunion has to be corrected only if symptomatic. Two other types of malunions exist: h…

The viability of the bone is compromised in two main situations at the wrist: Kienböck's disease and scaphoid nonunion with avascular necrosis. Plain radiography and CT allow an accurate anatomical approach of morphological changes associated with avascular necrosis of the lunate and the scaphoid fracture with complications. CT is readily available to detect nondisplaced fractures. However, early forms of necrosis can be misdiagnosed and evaluation of bone vitality is impossible. MRI is the best imaging modality to detect avascular necrosis and the intravenous injection of gadolinium improves

Radiographs of the scaphoid after injury are difficult to interpret, and bone scintigraphy is widely used to increase the accuracy of diagnosis, though many fractures suspected on scintigraphy cannot be confirmed radiologically. We have reviewed the clinical consequences, after one year, of managing suspected scaphoid fractures according to the bone-scan results. We studied 160 patients, 35 of whom had initially positive radiographs and were treated in a cast for 12 weeks. The other 125 had bone scintigraphy and were managed according to the result. After a minimum of one year 119 patients wer

OBJECTIVE: We planned this study to evaluate the role of bone scintigraphy in patients with suspected carpal fracture and normal or suspicious radiographs following carpal injury.
METHODS: Three-phase bone scintigraphy using Tc-99m-MDP was performed on 32 patients with negative radiographs but clinically suspected fracture at two weeks after the trauma. Focally increased radiopharmaceutical uptake was interpreted as a fracture. The final diagnosis was established with clinical follow-up.
RESULTS: Twelve (38%) patients had a normal scan excluding fracture. Twelve patients had a single fractur

Backgound The scaphoid is the most commonly fractured carpal bone and often presents as a diagnostic challenge. Fractures can often go unnoticed on initial radiographic evaluation and clinical presentation can vary significantly among patients. Awareness of high-risk cohorts assists practitioners in making the appropriate clinical diagnosis of acute scaphoid fracture. Materials and Methods The National Trauma Data Bank is the world's largest trauma data repository. Utilizing the 2016 public use file, we isolated scaphoid fractures by anatomic fracture location by utilizing International Clas

AIM: In the United Kingdom, diagnostic management of patients presenting to emergency department with a scaphoid injury varies. Follow-up plain radiographs, after an initial inconclusive X-ray, are common practice. We optimised the diagnostic pathway for these patients by focusing on the most appropriate diagnostic modality and on minimising the time to follow-up diagnostics.
MATERIALS AND METHODS: A baseline audit in the period 2008-2009 involving a total of 184 patients was conducted, and after the introduction of new local guidelines for scaphoid injury diagnostics, a follow-up audit invol

Arthroscopic bone grafting (ABG) in difficult scaphoid delayed union and nonunion allows thorough assessment and comprehensive management for scaphoid fracture and its sequelae. It provides a favorable biological environment for bony healing and produces minimal trauma to the soft tissues, aiding in rehabilitation. With adequate training and experience, high union rates and satisfactory clinical outcomes can be achieved. Poor blood supply of the scaphoid is not a contraindication to bone grafting; union rates over 80% have been reported, comparable to other existing surgical methods. This arti

Two cases of acute scaphoid fractures of the middle third with palmar comminution were treated with cancellous bone transplantation and Herbert screw fixation. Despite 6 weeks of cast immobilization, secondary loss of reduction resulted in primary grade dislocation in one patient. In the other patient scaphoid dislocation led to dorsiflexed intercalated segment instability and the need for screw removal due to secondary joint irritation. As a consequence the authors recommend the use of cortical bone grafting of the iliac crest in cases where palmar defects occur after reduction of the scaphoi

UNLABELLED: Not all proximal pole scaphoid nonunions with avascular necrosis require vascularized bone graft or a formal open incision. If the distal scaphoid is well perfused and the proximal pole can be secured rigidly after percutaneous bone grafting, then nonunion repair and healing can proceed. We describe minimally invasive techniques that permit fracture site débridement, establishment of distal perfusion by central axis reaming, arthroscopic assessment of perfusion, percutaneous bone grafting, and rigid internal fixation. As surgeons develop new minimally invasive techniques, it is sti

The aim was to evaluate if motion between the scaphoid bone fragments is related to the position of the fracture line and if a scaphoid nonunion results in the uncoupling of the proximal and distal carpal row during wrist motion. The influence of dorsal intercalated segment instability on interfragmentary motion was also analysed. In this study, 12 patients were included with unilateral scaphoid nonunion. Four-dimensional computerized tomography was used to analyse flexion-extension and radioulnar deviation motion of both wrists. We found that an increased instability of the scaphoid fragments…
