OBJECTIVE: To assess the efficacy of biologic drugs, beyond tumor necrosis factor- (TNF-) α inhibitors, in the management of noninfectious refractory scleritis, either idiopathic or associated with systemic immune-mediated disorders. PURPOSE: To determine the population-based incidence and disease associations of episcleritis and scleritis in a midwestern U.S.
DESIGN: Population-based retrospective cohort study.
METHODS: All residents of Olmsted County, Minnesota from 2006 to 2015 were identified by using the Rochester Epidemiology Project. A total of 223 episcleritis cases and 77 scleritis cases were identified. PURPOSE: Vision-threatening ocular inflammation can be a devastating complication of granulomatosis with polyangiitis (GPA). Here we performed a retrospective observational study to describe the safety and efficacy of treating scleritis and uveitis with either cyclophosphamide or rituximab in GPA.

METHODS: A retrospective review of uveitis and scleritis cases at Tilganga Institute of Ophthalmology in 2014.

Necrotizing scleritis is uncommon in dogs and presumed to be immune-mediated. Its clinical pattern and histopathology are similar to ocular lesions observed in humans suffering from granulomatosis with polyangiitis (GPA), formerly named Wegener's granulomatosis, where the pathogenesis revolves around anti-neutrophil antibodies (e.g., anti-myeloperoxidase). Senile scleral plaque is an age-related, commonly asymptomatic hyaline degeneration of the sclera. We report a case of calcific scleral plaque associated with anterior scleritis in an elderly patient with rheumatoid arthritis. Response to anti-inflammatory treatment was favorable.

PURPOSE: To describe a case of necrotizing scleritis associated with relapsing polychondritis (RP), which was treated with infliximab.
METHOD: Observational case report.
RESULTS: A 60-year-old woman presented with a red eye. She had a past medical history of nose deformity, hearing problem, and tinnitus. Ophthalmological examination disclosed necrotizing scleritis and peripheral ulcerative keratitis. RP was diagnosed. Necrotising scleritis is a severe form of anterior scleritis which is known to be associated with connective tissue disease but has not previously been reported in association with limited scleroderma. It is often a difficult condition to treat and without adequate early intervention leads to significant morbidity including visual loss. We report three cases of necrotising scleritis, two occurring in the context of previously unreported associations and a third case to compare presentation of the condition and highlight difficulties in management.
