BACKGROUND: Reverse total shoulder arthroplasty (RTSA) performed in the setting of massive proximal humeral bone loss often requires special reconstructive techniques. Restoration of the proximal part of the humerus with an allograft provides a number of theoretical benefits, including implant support, restoration of humeral length, deltoid tensioning, and an opportunity to repair the posterior aspect of the cuff to improve strength in external rotation and repair of the subscapularis to improve stability. However, reverse allograft-prosthesis composites (APCs) are costly, are technically dema

BACKGROUND: The role of cementless surface replacement arthroplasty (CSRA) in young individuals is currently unclear. The aim of this study was to evaluate CSRA long-term results for glenohumeral arthritis in young patients.
METHODS: Between 1990 and 2003, 54 CSRAs were performed on 49 patients (25 men, 24 women) aged younger than 50 years. Mean age was 38.9 years (range, 22-50 years). Three patients (4 shoulders) died over time and 8 were lost to follow-up, leaving 38 patients (42 shoulders) with a mean follow-up of 14.5 years (range, 10-25 years). There were 17 total shoulder replacements w

BACKGROUND: Primary hemiarthroplasty of the shoulder is used to treat complex proximal humeral fractures, although the reported functional results following this method of treatment have varied widely. The aim of this study was to prospectively assess the prosthetic survival and functional outcomes in a large series of patients treated with shoulder hemiarthroplasty for a proximal humeral fracture. By determining the factors that affected the outcome, we also aimed to produce models that could be used clinically to estimate the functional outcome at one year following surgery.
METHODS: A thir

Rotator cuff tear arthropathy (RCTA) is a complex condition characterized by rotator cuff insufficiency, altered shoulder kinematics, and progressive joint degeneration. In advanced stages, it results in pain, loss of active elevation, and significant functional impairment. Reverse shoulder arthroplasty (RSA) is now the best surgical option for advanced RCTA because it restores shoulder elevation by changing how the joint works and making the rotator cuff less important. The original RSA concept focused on medialization and distalization of the center of rotation to increase the deltoid moment

BACKGROUND: Modifications in reverse shoulder arthroplasty (RSA) have been made with the intent of maximizing motion, although there is little objective evidence outlining their benefit. This study investigated the RSA component combinations that impart the greatest effect on impingement-free glenohumeral motion.
METHODS: A previously validated virtual shoulder model was implanted with RSA components that varied by humeral implant type (inset/onset), glenosphere diameter (30, 36, and 42 mm), glenosphere placement (inferior/neutral), glenosphere center-of-rotation offset (0, 5, and 10 mm), hum

BACKGROUND: Although prior studies have raised concern regarding the longevity of anatomic total shoulder arthroplasty (aTSA) in young and active patients, it remains an effective treatment option for this population commonly presenting with multifactorial etiologies of shoulder arthritis. The purpose of this study was to perform a systematic review evaluating aTSA indications, outcomes, and implant survivorship in young and active patients aged 50 years or less.
METHODS: A systematic review of the literature from 1980 to 2024 evaluating outcomes following aTSA in patients less than 50 years

BACKGROUND: Instability is the most common complication after reverse total shoulder arthroplasty (rTSA). In the native glenohumeral joint, in addition to full dislocations, more subtle forms of instability exist. However, the incidence of more subtle forms of instability, the factors associated with instability, and the effect of instability on validated outcome scores after rTSA remain poorly understood.
QUESTIONS/PURPOSES: (1) After rTSA, what is the risk of instability, including more subtle forms of subjective instability? (2) What are the factors associated with instability? (3) Are mor

BACKGROUND: The incidence of medial calcar resorption has been shown to be common after uncemented total shoulder arthroplasty (TSA). With etiologies including stress shielding, debris-induced osteolysis, and infection, the clinical impact of medial calcar resorption has not been specifically examined. The purpose of this study was to determine whether resorption is associated with inferior outcomes or higher rates of radiographic loosening in TSA patients.
METHODS: We conducted a retrospective review of TSA patients with minimum 2-year clinical follow-up. Patient-reported and functional outc

Substantial bone loss at the time of complex primary and revision shoulder or elbow arthroplasty is challenging. Large bone defects compromise component support and important muscle-tendon units. Megaprosthesis, osteoarticular allografts, vascularized bone transfers, fusions, and allograft prosthetic composites (APCs) have all been described for reconstruction in these difficult situations. APCs increase bone stock and provide a better opportunity for soft-tissue reconstruction; however, they are prone to other complications, including graft resorption, nonunion, and fracture. BACKGROUND: Glenohumeral osteoarthritis can, in the most severe cases, require surgery with insertion of a shoulder arthroplasty. A design with a stem in the humeral bone canal is currently regarded as the standard treatment option in patients who have an intact rotator cuff function, but complications related to the stem including humeral fractures can have devastating consequences. By using a stemless humeral component, stem-related complications can be reduced. The aim of this study is to compare the Comprehensive Nano stemless total shoulder arthroplasty (intervention group) with the Compr

After a major upper limb amputation, the use of myoelectric prosthesis as assistive devices is possible. However, these prostheses remain quite difficult to control for grasping and manipulation of daily life objects. The aim of the present observational case study is to document the kinematics of grasping in a group of 10 below-elbow amputated patients fitted with a myoelectric prosthesis in order to describe and better understand their compensatory strategies. They performed a grasping to lift task toward 3 objects (a mug, a cylinder and a cone) placed at two distances within the reaching ar

BACKGROUND: Posterior glenoid wear and persistent static posterior subluxation of the humeral head (SPSH) are associated with early failure of anatomic total shoulder arthroplasty (aTSA). In patients with Walch B2 or B3 glenoid types, posterior augmented glenoid (PAG) implants have been reported to successfully correct SPSH on short-term follow-up. However, the medium-term and long-term clinical and radiological outcomes of PAG remain unclear.
METHODS: We prospectively followed 10 consecutive patients (mean age, 67.9 ± 8.9 years; 5 females) with primary glenohumeral osteoarthritis and B2 or B

INTRODUCTION: Little information is available on the clinical and radiographic outcome of intraoperative, non- to minimally displaced humeral fractures that occur during implantation of a stemmed, reverse shoulder prosthesis but are only recognized on routine postoperative radiographs. The goal of this study is to report the clinical and radiographic outcome for this rarely reported fracture type.
MATERIALS AND METHODS: 39 conservatively treated non- to minimally displaced intraoperative periprosthetic humeral fractures after stemmed RTSA were detected from our radiographic database between 1…
