Maxillary sinus volume in patients with canted occlusal plane: A cone-beam computed tomography study.

OBJECTIVES: To compare the maxillary sinus volume between both sides in adult patients with upper occlusal canting (>2 mm cant) and a control group (≤2 mm cant) using cone-beam computed tomography scans. MATERIALS AND METHODS: This retrospective study included a total of 84 scans (42 scans per group) according to predetermined selection criteria. OnDemand 3-D™ software was used for volumetric and linear measurements of the maxillary sinus. Maxillary occlusal canting was determined at the level of the maxillary first molars. It was defined as the vertical difference between right and left side

Complication and Salvage of Sinus Floor Elevation in the Maxillary Sinus With Asymptomatic and Noncalcified Fungus Colonization: A Case Report.

The present study aimed to report (1) the sequela of sinus floor elevation (SFE) in the posterior maxilla with severe sinus membrane thickening and an undiagnosed fungal colonization but a patent ostium and (2) a treatment course without implant removal. A 73-year-old woman underwent dental implant placement in the left posterior maxillary area. Although the patient was asymptomatic, severe sinus membrane thickening with Haller cells was observed on a radiographic examination, but the ostium was patent. After SFE and simultaneous implant placement, the patient developed acute sinusitis and was

The survival rate of transcrestal sinus floor elevation combined with short implants: a systematic review and meta-analysis of observational studies.

BACKGROUND: Currently, insufficient bone volume always occurs in the posterior maxilla which makes implantation difficult. Short implants combined with transcrestal sinus floor elevation (TSFE) may be an option to address insufficient bone volume. PURPOSE: The clinical performance of short implants combined with TSFE was compared with that of conventional implants combined with TSFE according to the survival rate. METHOD: In this systematic review and meta-analysis, we followed the Meta-Analysis of Observational Studies in Epidemiology (MOOSE) guidelines. Articles were identified through Pub

Volume stability of the augmented sinus using a collagenated bovine bone mineral grafted in case of a perforated Schneiderian membrane: An experimental study in rabbits.

OBJECTIVES: To determine the volume stability of a sinus augmented with a collagenated bovine bone mineral (CBBM) in case of an intact or perforated Schneiderian membrane (SM). MATERIALS AND METHODS: A bilateral sinus augmentation procedure was performed in eight rabbits. The SM was intentionally perforated in one side (SMP group), while it remained intact in contra-lateral side (control group) and the same amount of CBBM was then grafted. At 12 weeks, the animals were euthanized for radiographic and histomorphometric analyses. RESULTS: The augmented volume did not differ significantly betwe

An Atraumatic Approach to Internal Sinus Lifting: The Motorized Expansion Drill Technique.

This case series demonstrates internal maxillary sinus elevation and placement of 37 posterior implants in 34 private practice patients using sequential motorized expander burs exclusively to infracture the sinus border and embed bone graft material (allograft or xenograft) for vertical augmentation. With this technique, manual osteotome/malleting was completely avoided, improving surgical control and the patient experience. Sinus borders were lifted by a mean of 3.92 mm in ridges that had an average residual height of 7.52 mm. All implants achieved high insertion torque values (≥90 Ncm), obta

A New Approach for Lateral Sinus Floor Elevation.

The lateral sinus lift procedure has been extensively investigated and described as a reliable surgical solution aimed at facilitating implant placement and rehabilitation when the posterior upper maxilla is atrophic. The standard technique consists in a lateral antrostomy, the careful raising of the sinus membrane, and following apposition of a bone substitute between the membrane and the sinus floor. The present technical note illustrates a new conservative technique enabling lateral sinus lift without the use of bone substitutes and with predictable outcomes.

Calvarial graft resorption index in the reconstruction of the maxillary sinus in patients with atrophic maxillas: a prospective clinical study.

The objective of this study was to assess the resorption index of particulate calvarial grafts in maxillary sinuses of patients undergoing total reconstruction of an atrophic maxilla with residual alveolar bone that was less than, or equal to, 3mm thick. Twenty-one maxillary sinus floor elevations were carried out using particulate calvarial grafts in 11 individuals with totally edentulous maxillas. All patients had computed tomography (CT) before (T0), and 48hours (T1) and six months after surgery (T2). For each CT scan, linear measurements were taken of sections of the anterior, medial, and

Fabrication of a Surgical Guide for Lateral Sinus Augmentation.

Cone-beam computed tomography (CBCT) scans are used in planning implant treatment to help determine volume of available bone and identify anatomical structures. This enhances planning, particularly when inadequate bone is present in the posterior maxilla due to proximity of the inferior aspect of the maxillary sinus to the crestal bone. To place dental implants into this area, sinus augmentation is often needed to provide sufficient bone volume to support implants and allow prosthetic loading with predictable long-term results. Use of surgical guides for implant placement and sinus augmentatio

Schneider membrane thickness classification evaluated by cone-beam computed tomography and its importance in the predictability of perforation. Retrospective analysis of 200 patients.

Perforation of the Schneiderian membrane is a common complication of sinus augmentation. The aim of this retrospective study was to assess the associations between the lateral wall technique and the ridge approach and the risk of perforation, together with the relations between the thickness of the membrane and the risk of perforation. We studied 200 patients (103 men and 97 women) who had cone-beam computed tomography (CT) before a sinus lift with either a lateral wall technique (n=100) or the rigid approach (n=100) for insertion of prosthetic implants. Two-hundred cone-beam CT images were st

Piezoelectric surgery vs rotary instruments for lateral maxillary sinus floor elevation: a systematic review and meta-analysis of intra- and postoperative complications.

PURPOSE: Lateral maxillary sinus floor elevation (LMSFE) is a predictable preprosthetic surgical procedure that is used to overcome the limitations of the atrophied alveolar ridge for the placement of oral implants. Techniques using piezoelectric devices (PEDs) and conventional rotary instruments have been described for LMSFE in the literature, with little information regarding their efficiency in terms of membrane perforation, operating time, and implant outcomes. The aim of this systematic review was to examine the intra- and postoperative events associated with the use of PEDs and conventio

A guide to recognizing maxillary sinus pathology and for deciding on further preoperative assessment prior to maxillary sinus augmentation.

Dentists contemplating sinus augmentation frequently take a cone beam computed tomography (CBCT) scan as part of the work-up. Maxillary sinus pathology is common, and such dentists will be faced with pathologic sinus findings. Not all sinus pathology needs to be treated. The purpose of this article is twofold: to describe the most common sinus findings as seen on a CBCT scan and a way to recognize them, as well as to provide an algorithm for decision making on when further evaluation of the sinus is necessary.
