BACKGROUND: Immunoglobulin G4-related disease (IgG4-RD) is a rare, systemic fibro-inflammatory condition characterized by lymphoplasmacytic infiltration with IgG4-positive plasma cells, storiform fibrosis, and elevated serum IgG4 levels. It can affect multiple organ systems, often mimicking malignancies or infections. Skull base involvement in IgG4-RD is uncommon and presents diagnostic and therapeutic challenges. Solitary plasmacytomas are localized single tumors of monoclonal plasma cells that occur in two variants: solitary plasmacytoma of bone and extraosseous plasmacytoma. Solitary bone plasmacytoma (SBP) is a rare hematological malignancy that usually occurs in the spine and rarely in the skull. An elderly male with type 2 diabetes presented with a 2-month history of otalgia and severe headaches. He was diagnosed with malignant otitis externa (MOE) and was commenced on empirical treatment with oral ciprofloxacin. Pseudomonas is the most common cause of MOE. We present a case of a 58-year-old male with type II diabetes managed with metformin and insulin, who presented to the clinic with left chronic otitis media, persistent drainage, a stenotic meatus, and a prior history of 3 canal wall-down mastoidectomies and antibiotic therapy. BACKGROUND: Otogenic skull base osteomyelitis (OSBO) is rare and potentially fatal sequelae of otitis externa. Accurate and timely diagnosis is important due to rising incidence, morbidity and costs associated with treatment.
