Brainstem infarction and sleep-disordered breathing in the BASIC sleep apnea study.

BACKGROUND: Association between cerebral infarction site and poststroke sleep-disordered breathing (SDB) has important implications for SDB screening and the pathophysiology of poststroke SDB. Within a large, population-based study, we assessed whether brainstem infarction location is associated with SDB presence and severity. METHODS: Cross-sectional study was conducted on ischemic stroke patients in the Brain Attack Surveillance in Corpus Christi (BASIC) project. Subjects underwent SDB screening (median 13days after stroke) with a well-validated cardiopulmonary sleep apnea-testing device (n…

Sleep self-intoxication and sleep driving as rare zolpidem-induced complex behaviour.

INTRODUCTION: The GABA(A) receptor agonist zolpidem has been used for treatment of insomnia since years, but special side effects have been reported. These side effects were called zolpidem-induced sleep-related complex behaviour. Such complex behaviour is associated with somnambulism and includes sleepwalking, sleep eating, sleep conversation and sleep driving. CASE PRESENTATION: Two cases of zolpidem-induced sleep-related complex behaviour following self-intoxication, sleep driving and amnesia are presented. In both cases, the subjects reported the voluntary intake of only one zolpidem tabl…

Associations between specific technologies and adolescent sleep quantity, sleep quality, and parasomnias.

OBJECTIVE: We tested the hypothesis that weekday bedtime use of six technologies would be significantly associated with eight sleep parameters studied relating to sleep quantity, sleep quality, and parasomnias. METHODS: In our cross-sectional study, we previously administered validated age-appropriate questionnaires (School Sleep Habits Survey, Technology Use Questionnaire). Participating adolescents (n=738; 54.5% boys) were aged 11-13 years and were from the Midlands region of the United Kingdom in 2010. RESULTS: Frequent use of all technology types was significantly inversely associated wi…

The appearance of central sleep apnea after treatment of obstructive sleep apnea.

Patients with a primary diagnosis of obstructive sleep apnea frequently demonstrate central sleep apnea that emerges during treatment with CPAP. Although a number of mechanisms for this finding have been hypothesized, the pathophysiology is not definitively known. Controversy exists as to whether the concomitant appearance of the two phenomena represents a distinct meaningful entity. Regardless, the coincidence of these diseases may have important clinical implications. Herein, we review the proposed mechanisms for obstructive sleep apnea complicated by central sleep apnea. Future research is …

The relationship between mouth opening and sleep stage-related sleep disordered breathing.

STUDY OBJECTIVES: To evaluate mouth opening during sleep and the possible correlations between mouth opening and specific patient characteristics. METHODS: A total of 55 patients consecutively referred to assess snoring and suspected obstructive sleep apnea (OSA) were included. Sensors to record mouth opening were attached to each patient's face and synchronized with a standard polysomnogram. Mouth opening data were evaluated for each sleep stage as a percentage of maximum mouth opening. The patients were divided into 2 groups: patients with REM apnea hypopnea index (AHI) > NREM AHI (REM-depe…

[Links between diabetes mellitus and sleep disorders: focusing on obstructive sleep apnea].

During the past decades obesity and diabetes have become increasingly common in modern, industrialized societies. At the same time sleep disorders, chronic sleep loss and sleep deprivation have also become more and more prevalent. There may be a positive feed back circle between the two disorders: sleep problems may affect endocrine function and metabolic conditions, while metabolic abnormalities potentially interfere with sleep regulation. Sleep-disordered breathing, obstructive sleep apnea in particular, has the strongest association with glucose metabolism. Prevalence and severity of obstru…

Association between sleep and blood pressure in midlife: the CARDIA sleep study.

BACKGROUND: Epidemiological studies have reported an association between self-reported short sleep duration and high blood pressure (BP). Our objective was to examine both cross-sectional and longitudinal associations between objectively measured sleep and BP. METHODS: This study is ancillary to the Coronary Artery Risk Development in Young Adults (CARDIA) cohort study. Blood pressure was measured in 2000 and 2001 and in 2005 and 2006. Sleep was measured twice using wrist actigraphy for 3 consecutive days between 2003 and 2005. Sleep duration and sleep maintenance (a component of sleep qualit…

Sleep disruption in patients with sleep apnea and end-stage renal disease.

OBJECTIVES: Sleep apnea (SA) is common in patients with end-stage renal disease (ESRD) and such patients are likely to suffer additional sleep disruption associated with restless legs syndrome (RLS) and periodic leg movements (PLM). Our objective was to evaluate sleep quality in ESRD patients who are newly diagnosed with SAand determine the additional contribution of PLM to sleep disruption. METHODS: Two groups of patients with SA (apnea-hypopnea index (AHI) > 15) were compared, one with ESRD (n = 12) and the other with normal renal function (n = 18), using a sleep history questionnaire, slee…

Multiple sleep latency measures in narcolepsy and behaviourally induced insufficient sleep syndrome.

BACKGROUND: Short mean latencies to the first epoch of non-rapid eye movement sleep stage 1 (NREM1) and the presence of >or= 2 sleep onset REM (SOREM) periods on multiple sleep latency test (MSLT) occur in both narcolepsy-cataplexy (NC) and behaviourally induced insufficient sleep syndrome (BIISS). It is not known whether specific MSLT findings help differentiate the two disorders. METHODS: We analyzed MSLT data including sleep latencies to and between different sleep stages of 60 age-, gender- and body mass index (BMI)-matched subjects (hypocretin-deficient NC, actigraphy-confirmed BIISS, he…

Visual dysfunction and computational sleep depth changes in obstructive sleep apnea syndrome.

The aims of this study are to clarify whether patients with obstructive sleep apnea syndrome (OSAS) have a decline in verbally or visually-based cognitive abilities and whether the possible decline is related to particular sleep depth changes. In addition, the effect of continuous positive airway pressure (CPAP) on the possible changes is investigated. Fifteen OSAS patients and 15 healthy controls joined two full-night polysomnographies, including a computational measure of deep sleep percentage (DS%) bilaterally from the frontal, central and occipital channels, and a neuropsychological assess…

Hypnotic and sleep quality-enhancing properties of kavain in sleep-disturbed rats.

The present study was performed to investigate the effects of kavain on the sleep-wake cycle in comparison with that of rilmazafone and diphenhydramine using sleep-disturbed rats. Electrodes for the electroencephalogram (EEG) and electromyogram (EMG) were implanted into Wistar rats. Total awake time, non-rapid eye movement (non-REM) sleep and rapid eye movement (REM) sleep were measured for 6 h. Kavain and rilmazafone showed a significant shortening in sleep latency, decreased awake time, and increased non-REM sleep time. On the other hand, significant shortening of the sleep latency was obser…

Sleep extension versus nap or coffee, within the context of 'sleep debt'.

Though extended night-time sleep mostly reduces the 'afternoon dip', little is known about evening benefits to alertness, or about comparisons with an afternoon nap or caffeine. Twenty healthy carefully screened adults, normal waking alertness levels, underwent four counterbalanced conditions: usual night sleep; night sleep extended<90 min (usual bed-time); up to 20 min afternoon nap; and 150 mg afternoon caffeine (versus decaffeinated coffee). Sleepiness was measured by afternoon and evening multiple sleep latency test (MSLTs), longer psychomotor vigilance test (PVT) sessions and a subjective…
