Bilateral Keratomalacia From Vitamin A Deficiency in Pancreatic Insufficiency.

While vitamin A deficiency is a leading cause of blindness globally, it is uncommon in the developed world. Here we describe the unique presentation of a young man in the United States with keratomalacia from vitamin A deficiency related to pancreatic insufficiency. The patient presented with bilateral blurry vision that persisted for two weeks, significant unintentional weight loss, orthostatic hypotension, and profuse diarrhea. Upon slit-lamp examination, bilateral corneal opacities were appreciated. After completing additional testing, it became clear that the patient's corneal opacities we

Corynebacterium kroppenstedtii as a pathogen of a Jones tube infection following conjunctivo-dacryocystorhinostomy.

A 35-year-old woman complained of an unpleasant odor for a few days after a change in foundation cream. The patient had previously undergone conjunctivo-dacryocystorhinostomy with a Jones tube fixed with non-absorbable suture. Slit-lamp examination revealed an orange-colored discharge in the tube. A culture test of the discharge showed Corynebacterium kroppenstedtii (1+), Aspergillus versicolor (1+), and Mycobacterium chelonae (1+). After medical treatment and suture removal, the discharge completely disappeared. This is the first reported case of a Jones tube infection following conjunctivo-d

First report of palpebral conjunctival inflammatory nodule associated with Demodex species.

Demodex mite infestation of the palpebral conjunctiva as focal inflammatory nodule has been not reported. A 46-year-old man presented with lower palpebral conjunctival nodules in his left eye. Slit-lamp examination showed focal inflammatory nodules at the inferior palpebral conjunctiva accompanied by conjunctival hyperemia and purulent secretion. Excisional biopsy of this lesion was performed. Pathological evaluations revealed inflammatory granuloma associated with Demodex species in the palpebral conjunctiva. This is the first report of Demodex-related palpebral conjunctival inflammatory nodu

Scleritis Associated with SAPHO Syndrome: A Case Report.

A 31-year-old woman developed bilateral painful red eyes. A slit-lamp examination revealed anterior diffuse scleritis. She had been diagnosed with palmoplantar pustulosis 2 years before. Further evaluation revealed hyperostosis of the sacroiliac joint and inflammation of the bilateral sternoclavicular joints and right sternocostal joint. Ultimately, she was diagnosed with SAPHO syndrome by rheumatologists after excluding other causative diseases. Scleritis associated with SAPHO syndrome is relatively uncommon. An identification of any systemic symptoms and early consultation with rheumatologis

First reported case of intraocular Gnathostoma spinigerum in Cambodia.

A live intraocular nematode was identified from a 37 year-old man presented with iritis, pain, redness, lacrimation, swelling, vision loss and intermittent blindness during many hours per day of the left eye. By using slit lamp examination, a worm was removed from iris in an ophthalmology outpatient department setting and sent to the Medical Microbiology Laboratory, Institut Pasteur du Cambodge. Gnathostoma spinigerum was identified, based on its typical morphology via microscopic examination. Based on our diagnosis, the patient was treated by oral albendazole and responded well to this therap…

[Acute macular neuroretinopathy].

A 24-year-old female patient presented with sudden onset of paracentral visual field defects of the right eye, which were noticed 2 months before the clinical examination. The slit lamp examination of the anterior and posterior segments showed unremarkable findings but three oval, well demarcated, hyporeflective areas were noticed in the right posterior pole in infrared fundus photographs. A similar small area was found in the left posterior pole. Optical coherence tomography (Cirrus OCT) showed a structural defect of the hyperreflective bands of the outer retina. The diagnosis of acute macula…

Anterior keratoconus associated with unilateral cornea guttata.

PURPOSE: To describe a 30-year-old man with bilateral but asymmetric anterior keratoconus and unilateral cornea guttata in the eye with more advanced keratoconus. METHOD: Case report. RESULTS: The patient's keratoconus was diagnosed three or four years earlier. The keratoconus was confirmed by slit-lamp examination, keratometry, and computer-assisted topographic analysis. Cornea guttata were confirmed by clinical examination and specular microscopy. CONCLUSION: This is a rare case of unilateral cornea guttata associated with asymmetric keratoconus, showing severe guttate change only in the

[Irradiation of the rabbit ciliary body by an accelerated proton beam].

Rabbit ciliary body has been irradiated by accelerated proton beam. 24 animals have received on 20 and 40% of the total surface of ciliary body 45 and 60 Gy. We could confirm the very good precision of proton beam irradiation. No complication was noticed on slit lamp examination. The light microscope examination revealed haemorrhages, lymphocytes, plasma cells and histiocytes infiltration, late fibrosis and severe capillary alterations. The electron microscope examination showed a frank difference of radiosensitivity of the epithelial layers and a presumed partial rupture of the blood-aqueous …

Pauciarticular juvenile rheumatoid arthritis presenting in an adult.

Type I pauciarticular juvenile rheumatoid arthritis (PA-JRA) is characterized by a female predominance, chronic iridocyclitis and a positive antinuclear antibody (ANA) test. In contrast, patients with type II PA-JRA are usually male, have a negative ANA test, but are HLA-B27 positive. We report a patient with longstanding iridocyclitis who presented at age 20 with oligoarthritis, whom we believe has Type I PA-JRA. Slit lamp examination should be considered in patients with oligoarthritis; the detection of asymptomatic chronic iridocyclitis may aid in the diagnosis. Untreated, this condition ma…

Complications of contact lens wear after radial keratotomy in an animal model.

In 12 rabbits radial keratotomy was performed on one eye with no treatment to the contralateral eye. Cellulose acetate butyrate contact lenses were fitted bilaterally for extended wear. The eyes were monitored weakly for three weeks with a standardized slit-lamp grading method. Corneal neovascularization occurred earlier and progressed further in eyes that had radial keratotomy than in the control eyes (P less than .0025). These results suggested that patients who require contact lenses after radial keratotomy may be at higher risk for complications such as corneal neovascularization.

Kayser-Fleischer-like rings in patients without Wilson's disease.

Three patients, one with cryptogenic cirrhosis, one with active chronic hepatitis and one with neonatal hepatitis, were found to have corneal pigmentation rings indistinguishable from early Kayser-Fleischer rings on slit lamp examination. They did not have the clinical features of Wilson's disease and their serum copper and ceruloplasmin concentrations were normal. Urinary copper excretion rates and hepatic concentrations were only slightly raised but were below the range found in symptomatic Wilson's disease. It is concluded that the Kayser-Fleischer ring would no longer be considered as path…

Deep learning model for extensive smartphone-based diagnosis and triage of cataracts and multiple corneal diseases.

AIM: To develop an artificial intelligence (AI) algorithm that diagnoses cataracts/corneal diseases from multiple conditions using smartphone images. METHODS: This study included 6442 images that were captured using a slit-lamp microscope (6106 images) and smartphone (336 images). An AI algorithm was developed based on slit-lamp images to differentiate 36 major diseases (cataracts and corneal diseases) into 9 categories. To validate the AI model, smartphone images were used for the testing dataset. We evaluated AI performance that included sensitivity, specificity and receiver operating chara

Case Report: Focal Choroidal Excavation: Conversion from Conforming to Nonconforming in a Pregnant Female.

SIGNIFICANCE: Although rarely seen, clinicians should closely monitor patients, especially pregnant patients with focal choroidal excavation (FCE), as it can convert between subtypes and has been linked to pachychoroidal disease and potential for vision loss. PURPOSE: This study aimed to report a case of the conversion of conforming FCE to nonconforming FCE with spontaneous resolution in a pregnant female. CASE REPORT: A 35-year-old Indian woman presented with a slightly decreased vision in the right eye. The patient was 3 months pregnant at this visit. Her ocular history included stable con

Corneal decompensation due to spontaneous absorption of lens and anterior dislocation of lens capsule: A case report.

RATIONALE: Spontaneous absorption of lenses or cataracts is rare. However, lens capsule attachment to the endothelium combined with corneal decompensation can still occur. PATIENT CONCERNS: An 81-year-old male presented with left eye pain and bulbar conjunctival injection for 6 months. Diffuse corneal edema and inferior bullous lesions were observed by slit-lamp microscopy. Following examination with swept-source optical coherence tomography, we could clearly identify a membrane structure adherent to the corneal endothelium, as well as a lens not in situ. In vivo confocal microscopy found dec

A novel method in correcting aberropia using pin hole soft contact lenses: A case report.

Aberropia leads to decrease in image quality due to higher order aberrations (HOA). Correction of these HOA poses a great challenge to clinicians. A case report is presented here where a 30 year old male visited with complaints of cloudy vision, diplopia in reduced illumination along with inferior vertical elongation of light during night time in the right eye since 6 years. The unaided visual acuity was found to be 0.0 Log MAR units and near acuity was N6. The patient did not accept any correction in subjective refraction. The aberrometry findings revealed vertical trefoil and vertical coma o
