Telavancin versus standard therapy for treatment of complicated skin and soft-tissue infections due to gram-positive bacteria.

BACKGROUND: Telavancin, a novel lipoglycopeptide, exerts concentration-dependent, rapid bactericidal activity on account of its multiple mechanisms of action. Telavancin is highly active against gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-intermediate, and vancomycin-resistant strains. METHODS: We conducted a randomized, double-blind, controlled, phase-2 clinical trial. Patients > or = 18 years of age with a diagnosis of complicated skin and soft-tissue infection caused by suspected or confirmed gram-positive organisms were randomized to re

From Debridement to Skin Grafting: Where Fish-Skin Xenografting Fits in Staged Reconstruction of Necrotizing Soft Tissue Infections.

BackgroundNecrotizing soft tissue infections (NSTIs) pose substantial reconstructive challenges, particularly in patients with significant comorbidities. Biologic fish-skin xenografts, which are decellularized and rich in polyunsaturated fatty acids, have anti-inflammatory and antimicrobial properties and have shown promise in complex wound management. However, published data remain limited.MethodsA retrospective review of nine patients with NSTIs treated between January 2024 and March 2025 at a single institution was conducted. All underwent serial debridements until wounds were free of gross…

Surgical fecal diversion vs non-diversion in perineal necrotizing soft tissue infections: A systematic review and meta-analysis.

BACKGROUND: Perineal necrotizing soft tissue infections (P-NSTI) are aggressive and potentially fatal conditions. Surgical fecal diversion (SFD) is often used to reduce contamination, but whether non-diversion provides comparable outcomes remains uncertain. We hypothesized that non-diversion does not confer worse outcomes than SFD. AIM: To evaluate clinical outcomes of non-diversion compared with SFD in adults with P-NSTI. METHODS: Systematic review and meta-analysis following PRISMA and Cochrane standards. PubMed, EMBASE, Scopus, Web of Science and Cochrane Library were searched without dat…

Molecular Typing and Drug Resistance of Methicillin-Resistant Staphylococcus aureus in Skin and Soft Tissue Infections in Shaoxing.

Staphylococcus aureus, commonly colonizing the mucous membranes and skin of humans, is a prevalent pathogen responsible for Skin and Soft Tissue Infections (SSTIs), with a notably high prevalence of methicillin-resistant Staphylococcus aureus (MRSA). The antimicrobial resistance profiles of MRSA vary across different regions, with differences in population structure and epidemiological characteristics over time and geography. Molecular typing is frequently employed to investigate the population structure and transmission features among strains. The purpose of this study is to investigate the m…

Under the Skin: A Case Series of Clostridium sordellii Necrotizing Soft Tissue Infections in Patients Who Inject Drugs.

Clostridium sordellii has been well-associated as a cause of obstetric infections but is a less commonly recognized organism of necrotizing skin and soft tissue infections (NSTI). C. sordelli infections are rare with only 14 cases reported in the literature to date. These infections are often associated with profound septic shock and the mortality rate remains high. We report four patients who presented with C. sordellii NSTI over a period of two years at an academic medical center. Notably, injection drug use was the main risk factor for infection in these patients. Appropriate management of …

Effectiveness of oritavancin for management of skin and soft tissue infections in the emergency department: A case series.

Skin and soft tissue infections, such as cellulitis, are commonly diagnosed in the emergency department and these patients are often admitted to the hospital for intravenous antibiotic therapy. Oritavancin is a novel antibiotic approved for the treatment of skin and soft tissue infections that is administered as a one-time infusion. While oritavancin has demonstrated comparable efficacy with multi-dose parenteral antibiotics in clinical trials and has been proposed as an alternative to admission for emergency department patients, there is a paucity of available real world effectiveness data. I…

Clostridial Gas Gangrene - A Rare but Deadly Infection: Case series and Comparison to Other Necrotizing Soft Tissue Infections.

OBJECTIVE: Clostridial gas gangrene (GG) or clostridial myonecrosis is a very rare but life-threatening necrotizing soft tissue infection (NSTI) caused by anaerobic, spore-forming, and gas-producing clostridium subspecies. It is the most rapidly spreading and lethal infection in humans, also affecting muscle tissue. The high mortality, of up to 100%, in clostridial GG is mediated by potent bacterial exotoxins. Necrotizing fasciitis (NF) is an important differential diagnosis, most often caused by group A streptococci, primarily not affecting musculature but the subcutaneous tissue and fascia.

Diagnostic Value of Laboratory Parameters for Distinguishing Between Herpes Zoster and Bacterial Superficial Skin and Soft Tissue Infections.

Clinical differentiation between herpes zoster and bacterial superficial skin and soft tissue infections of the face can be difficult. In addition, diagnosis can be complicated by bacterial superinfection of lesional herpes zoster. The aim of this study was to determine whether inflammatory parameters, such as C-reactive protein (CRP) and blood counts, might be reliable biomarkers to distinguish between skin and soft tissue infections and herpes zoster when the face is infected. The study data (multivariate analysis and area under the curve) identified CRP (0.880) and leukocytes (0.730) togeth…

The clinical value of metagenomic next-generation sequencing in the microbiological diagnosis of skin and soft tissue infections.

BACKGROUND: Metagenomic next-generation sequencing (mNGS), with its comprehensiveness, is widely applied in microbiological diagnosis. Etiological diagnosis is of paramount clinical importance in patients with skin and soft tissue infections (SSTIs). However, the clinical application of mNGS in SSTIs is relatively less studied. MATERIALS AND METHODS: From April 1, 2017 to December 31, 2019, 96 SSTI cases were collected. The positive rates of pathogens detected by mNGS and culture were compared by analyzing tissue samples, pus, swabs, and/or interstitial fluids obtained from the infected parts…
