Speech pathology assessment of dysphagia post endotracheal extubation: A service-model evaluation.

BACKGROUND: As postextubation dysphagia (PED) is correlated with pneumonia, feeding-tube placement, and in-hospital mortality, early identification is paramount. Endotracheal intubation duration of ≥48 h is independently predictive for PED. Therefore, a blanket intensive care unit (ICU) referral protocol was implemented to conduct PED assessment of patients intubated for ≥48 h. OBJECTIVES: The objective of this study was to review outcomes of an established blanket referral model for PED assessment in patients intubated for ≥48 h. Outcomes of the model were examined over a 2-year period (June

[Efficacy of rehabilitation in oropharyngeal dysphagia].

BACKGROUND: efficacy of rehabilitation in oropharyngeal dysphagia. In our country the practice of speech-language pathology in oropharyngeal dysphagia has increased significantly and, at this moment, deserves attention since practice needs to be based on scientific evidence. Therapeutic techniques and the outcome of rehabilitation in oropharyngeal dysphagia have been studied since the 70s, reaching its high point during the 80s and 90s. Few studies have investigated the efficacy of therapy in the rehabilitation of oropharyngeal dysphagia, the vast majority have tried to prove the effects of th…

The core components of clinical planning for Comprehensive, High-dose Aphasia Treatment (CHAT): A task analysis.

BACKGROUND: Intensive comprehensive aphasia programmes (ICAPs) deliver intensive aphasia rehabilitation via a cohort approach, aligning with the World Health Organization's (WHO) International Classification for Functioning, Disability and Health (ICF). ICAPs are an effective treatment approach for aphasia rehabilitation, and their implementation within healthcare settings is currently being investigated. However, there are challenges associated with selecting and tailoring evidence-based treatments for delivery within ICAPs and supportive processes for selecting and tailoring therapy are requ

Fear of public speaking-the role of the SLP.

Although there are some individuals who have a true phobia of speaking in a public forum, public speaking is usually near the top of any list of activities that most individuals dislike, fear, or avoid. Such nonphobic speakers are the subjects of this article. They comprise individuals who, for various reasons, are (or fear they will be) inadequate speakers. This article is based on extensive clinical experience with such individuals. It describes some basic causes for these fears; considers evaluative and treatment procedures; and provides a rationale for the involvement of the speech-languag

Embedded-explicit emergent literacy intervention II: goal selection and implementation in the early childhood classroom.

This article, the second in a two-part series, provides guidance to speech-language pathologists (SLPs) for implementing the explicit component of the embedded-explicit emergent literacy intervention model for at-risk preschool and kindergarten children. The explicit component refers to the provision of regular structured therapeutic interactions that intentionally target critical emergent literacy goals. This article describes fundamental principles of explicit literacy instruction, identifies literacy domains targeted as part of explicit literacy instruction, and presents examples of how ear

Histologic tau lesions and magnetic resonance imaging biomarkers differ across two progressive supranuclear palsy variants.

Progressive supranuclear palsy is a neurodegenerative disease characterized by the deposition of four-repeat tau in neuronal and glial lesions in the brainstem, cerebellar, subcortical and cortical brain regions. There are varying clinical presentations of progressive supranuclear palsy with different neuroimaging signatures, presumed to be due to different topographical distributions and burden of tau. The classic Richardson syndrome presentation is considered a subcortical variant, whilst progressive supranuclear palsy with predominant speech and language impairment is considered a cortical

Fetal alcohol syndrome. Hearing, speech, language, and vestibular disorders.

Fetal alcohol syndrome (FAS) refers to a pattern of anomalies that include craniofacial, CNS, growth, and various sensory anomalies. We have observed that FAS is associated with four kinds of hearing disorders: (1) developmentally delayed auditory function, (2) sensorineural hearing loss, (3) intermittent conductive hearing loss owing to recurrent serous otitis media, and (4) central hearing loss. As is the case with other syndromes associated with craniofacial anomalies and hearing impairments, speech and language pathologies also are common in FAS patients. Although auditory and vestibular s

Is the perception of dysphonia severity language-dependent? A comparison of French and Italian voice assessments.

In this cross-language study, six Italian and six French voice experts evaluated perceptually the speech of 27 Italian and 40 French patients with dysphonia to determine if there were differences based on native language. French and Italian voice specialists agreed substantially in their evaluations of the overall grade of dysphonia and moderately concerning roughness and breathiness. No statistically significant effects were found related to the language of the speakers with the exception of breathiness, a finding that was interpreted as being due to different voice pathologies in the patient
