Office surgery for paranasal sinus recirculation.

BACKGROUND: Circular flow of mucus between adjacent openings of a single paranasal sinus is known as recirculation, and can cause persistent sinonasal symptoms. Removing the bridging tissues allows the surgical ostium to connect to the natural ostium. This procedure prevents the circular flow between the 2 ostia, and allows the mucociliary system to provide for physiologic drainage of the sinuses. Traditionally this has taken place in the operating room; however, performing this procedure in the office is possible, even when there is significant intervening bone. Here we report a series patien

Primary osteoma of the orbit with atypical facial pain: case report and literature review.

Osteoma is a benign, slowly growing tumor that mainly occurs in the bones and cavities of the middle third of the face, representing the most frequent benign tumor of the paranasal sinuses. It rarely originates primarily from the orbit. Most of these lesions develop in the fourth to fifth decades of life, and are more commonly encountered in males. In the English literature, there are so far three reported cases of primary osteoma of the orbit that originated from the sphenoid bone. Here we present another case of a primary osteoma of the orbit presenting with atypical facial pain and discuss

Sphenochoanal polyp: current diagnosis and management.

Choanal polyps arise from inflamed, edematous mucosa in the paranasal sinuses. They extend into the choana and cause nasal obstruction and mouth breathing. In most cases, these polyps arise in the maxillary sinus, but rare cases of origin in the sphenoid sinus and other nasal structures have been reported. The presence of a choanal polyp in an atypical location can lead to diagnostic confusion and exploration of the wrong sinus at surgery. The author reports the case of a 15-year-old girl who was diagnosed with a sphenochoanal polyp. The patient was successfully treated via an endoscopic surgi

Aneurysmal bone cyst in the sphenoid bone: treatment with minimally invasive surgery.

Aneurysmal bone cysts are vascular lesions that destroy and expand bone. We report a recently treated case of an aneurysmal cyst of the sphenoid bone. A 14-year-old girl presented with frontal headaches, bouts of nausea, and vomiting. Computed tomography and magnetic resonance imaging showed typical features of an aneurysmal bone cyst. Arterial embolization was undertaken before surgery. The endoscopic transnasal procedure used allowed the complete removal of the aneurysmal bone cyst. This use of minimally invasive surgery makes this case of interest to surgeons of the skull base and sinuses.

Assessing MRI interpretability of the orbit, paranasal sinuses, and nasopharynx in cochlear implant patients.

OBJECTIVES: Due to a growing focus on cost-effectiveness in healthcare, safety concerns with CI and the known limitations in image quality, there is an increasing need for well-considered indications before performing magnetic resonance imaging (MRI) in CI (cochlear implant) patients. This study aims to evaluate, for the first time, the clinical utility and limitations of MRI in CI patients for orbital, paranasal, and nasopharyngeal assessments. MATERIALS AND METHODS: CIs were positioned and fixed with bandaging around the head of a test subject at varying angular positions (90°, 120°, and 13

Some anatomical variation of paranasal sinuses using sinus endoscopic approach on "cadaver" in Isfahan, Iran.

BACKGROUND: Due to the anatomical variations of the paranasal sinuses and its great importance in sinus surgery, as this area is in very close proximity to vital structures including the optic nerve, carotid artery, and skull base, anatomical knowledge of this area is of high importance. The purpose of this study is defining a full and clear impression of paranasal sinus anatomy and its variations as a model for the human population of the country. MATERIALS AND METHODS: This study was conducted on 45 cadavers in Isfahan forensic Medicine center during 2010 to 2011. Nasal and paranasal sinuse

Endoscopic surgical treatment of nasal and paranasal sinus inverted papilloma.

Most of the papers written in the past regarding surgical treatment of nasal and sinus inverted papilloma recommend aggressive surgical treatment-usually a medical maxillectomy with external or transantral ethmoidectomy. However, since the introduction of endoscopic diagnostic and surgical techniques for the treatment of nasal and sinus disease, the treatment of inverted papilloma requires review. In Europe, Waitz and Wigand1 and Draf* routinely treat inverted papilloma endoscopically, reserving extensive surgery for papilloma involving the maxillary sinus or for recurrent disease. Fifteen cas

Pituitary abscess presenting with cranial nerve paresis. Case report and review of literature.

Non-adenomatosus lesions of the pituitary represent a small part of the intrasellar processes and they have heterogeneous presentation. Making a precise diagnosis is of great importance, as it may lead to more efficient management. A 65-year-old man was admitted to the hospital because of headache and right cranial nerve III palsy. Basic laboratory work-up was normal whereas endocrinological assessment revealed hypopituitarism without diabetes insipidus. Plain radiography showed an enlarged sella and frontal and paranasal sinusitis. Computed tomography (CT) and magnetic resonance imaging (MRI)

Low-frequency pulsed ultrasound in the nasal cavity and paranasal sinuses: a feasibility and distribution study.

BACKGROUND: Bacterial biofilms have been implicated in refractory rhinosinusitis. Biofilms have been shown to respond to treatment with low-frequency ultrasound (LFU) therapy in vitro, and exposure to LFU has shown efficacy in wound repair and topical drug delivery in other fields. This preliminary study was designed to evaluate the safety and feasibility of LFU for use in the nasal cavity and paranasal sinuses. METHODS: This was an experimental observational study. Six cadaver heads were used to deliver a mixture of Renografin and methylene blue solvent to the paranasal sinuses via LFU both

[Possibility and limits of the endonasal approach in sinonasal inverted papilloma cases--the technique and own experience. Preliminary report].

The aim of this article is to present the technique of endonasal operation due to sinonasal inverted papilloma with it's limitations. Material consists of 21 patients operated with microscope and endoscopes. The tumor is resected in few pieces, but I try (especially in the beginning) to keep the resected tumor as much as possible in one part--which makes easier to assess the tumor borders. Mostly it is possible to leave at place the inferior turbinate and only the upper part of it must be resected with the tumor. Involvement of the frontal sinus is the contraindications for a purely endonasal

Isolated sphenoid sinus disease: differential diagnosis and management.

PURPOSE OF REVIEW: To review the current literature on isolated disease of sphenoid sinus, and to describe the various approaches to the sphenoid sinus. RECENT FINDINGS: Due to its posterior location and subtle presenting symptoms, sphenoid lesions, in the past, have often been missed and were reported as rare occurrences. However, with the availability of current diagnostic modalities such as endoscopy and imaging techniques such as computed tomography (CT) and magnetic resonance imaging (MRI), diseases of the sphenoid sinus are now more frequently diagnosed. Since the introduction of endosc

Isolated sphenoid sinus changes--history, CT and endoscopic finding.

This study reviews the records of 21 patients with isolated sphenoid sinus disease who were treated by rigid endoscopic sphenoidotomy at the Nose and Sinus Unit, Department of Otolaryngology of Beilinson Medical Center, Israel. Diagnosis was made on the basis of history, rigid nasal endoscopy and computed tomography (CT) scan. The most frequent symptom was headache; no instances of 'pathognomonic' headache were found. Sphenoidotomy was performed through the area of the natural ostium. The pathological finding was infection in 11 patients, cyst in four patients, polyps in three patients, mucoco

Development of posttraumatic frontal brain abscess in association with an orbital roof fracture and odontogenic abscess: A case report.

BACKGROUND: Brain abscess is a potentially fatal condition. Orbital fractures caused by penetrating injury may be associated with intracranial infection. Such complication associated with blunt trauma, orbital roof fractures, and odontogenic abscesses is exceedingly rare. CASE DESCRIPTION: We report the case of a 40-year-old transgender female with a frontal abscess presenting several weeks following a motor vehicle crash from which she suffered multiple facial fractures and an odontogenic abscess. On computed tomography scan, the patient had multiple right-sided facial fractures, including a

Tumors of the paranasal sinuses: a therapeutic challenge.

Cancer involving the ethmoid and sphenoid frontal sinus complex can be successfully eradicated by a combined transcranial and transfacial surgical dissection. Survival rates of 44 to 58 percent with a 3 percent hospital mortality rate in patients whose previous surgery or radiotherapy was largely unsuccessful suggest that this cosmetically acceptable surgical endeavor should be used more often by the head and neck surgeon in treating paranasal sinus cancer. Utilizing the principles of antibiotic prophylaxis, strict attention to principles of tumor removal and surgical technique, and the talent

Orbitomaxillary mucormycosis (zygomycosis) and the surgical approach to treatment: perspectives from a maxillofacial surgeon.

Rhinocerebral or rhino-orbitocerebral (mucormycosis) zygomycosis (ROCZ) usually occurs among patients with poorly controlled diabetes mellitus (especially those with ketoacidosis), solid malignancies, iron overload or extensive burns, in patients undergoing treatment with glucocorticosteroid agents, or in patients with neutropenia related to haematologic malignancies. The disease process starts with inhalation of the fungus into the paranasal sinuses. The fungus may spread to invade the palate, sphenoid sinus, cavernous sinus, orbits or cranially to invade the brain. Pain and swelling precede
