Transsacral Canal Plasty for Decompression of Lumbar Spinal Stenosis in a Patient With Epidural Lipomatosis: A Case Report.

CASE: A 58-year-old man presented with lumbar spinal stenosis due to epidural lipomatosis. He underwent transsacral canal plasty (TSCP), in a manner similar to epidural adhesiolysis, which can be performed under local anesthesia. His leg pain improved dramatically in the year after surgery. Furthermore, magnetic resonance images during this time show neural decompression by reduction of the amount of epidural fat. CONCLUSION: TSCP was found not only to relieve pain but also to achieve neural decompression in this patient with spinal canal stenosis caused by lumbar epidural lipomatosis. This i…

Investigation of in vivo three-dimensional changes of the spinal canal after corrective surgeries of the idiopathic scoliosis.

OBJECTIVE: To determine the three-dimensional (3D) changes of the spinal canal length (SCL) after corrective surgeries and their association with the radiographic and clinical outcomes of idiopathic scoliosis patients. The length of the spinal cord has been demonstrated to be strongly correlated with the SCL. Understanding the changes in SCL could help determine the morphologic changes in the spinal cord to prevent spinal cord injury. METHODS: Twenty-seven scoliotic patients' 3D spinal canal were investigated using computed tomography images. The SCL between the upper and lower end vertebrae …

Hypertrophy of the ligamentum flavum in lumbar spinal canal stenosis is associated with abnormal accumulation of specific lipids.

Ligamentum flavum hypertrophy (HLF) is the most important component of lumbar spinal canal stenosis (LSCS). Analysis of hypertrophied ligamentum flavum (HLF) samples from patients with LSCS can be an important que. The current study analyzed the surgical samples of HLF samples in patients with LCSC using quantitative and qualitative high performance-liquid chromatography and mass spectrometry. We collected ligamentum flavum (LF) tissue from twelve patients with LSCS and from four patients with lumbar disk herniation (LDH). We defined LF from LSCS patients as HLF and that from LDH patients as n…

Entrapment of a metal foreign body in the cervical spinal canal during surgical procedure: A case report.

RATIONALE: Retention of foreign objects in spinal canal usually results from penetrating spinal trauma or failed internal instruments. However, entrapment of a foreign body in cervical spinal canal during surgery is rare, and whether such an object may cause neurological complications remains unknown in literature. PATIENT CONCERNS: A 50-year-old man underwent C5 corpectomy and instrumentation surgery due to cervical myelopathy. During the surgery, the cutting edge of a Kerrison rongeur was broken and the metal tip was retained behind C4 vertebra. DIAGNOSIS: Retention of foreign body in the …

Expression of the JAK/STAT signaling pathway in the ligamentum flavum of patients with lumbar spinal canal stenosis.

BACKGROUND: Ligamentum flavum (LF) hypertrophy is an important cause of lumbar spinal canal stenosis (LSS), one of the most common spinal disorders in the elderly. Although many cytokines are reported to be associated with LF hypertrophy, the intracellular signaling system is rarely discussed. The purpose of this study was to identify the JAK/STAT signaling pathway and to examine the role of the JAK/STAT systems in the hypertrophied LF. METHODS: The LF of 10 LSS patients was analyzed and the expression of JAK1, STAT3, phosphorylated (p)-STAT3, and actin was examined by Western blot analysis. …

Decreased elastic fibers and increased proteoglycans in the ligamentum flavum of patients with lumbar spinal canal stenosis.

Elastic fibers and proteoglycans are major components of the extracellular matrix and their changes have been reported in some pathological conditions. Further, recent studies have indicated that some glycosaminoglycans and proteoglycans inhibit elastic fiber assembly. The purpose of this study was to investigate changes of the elastic fibers and proteoglycans in the ligamentum flavum and analyze their relationships to thickening of the ligamentum flavum from lumbar spinal canal stenosis (LSCS). Ligamentum flavum samples were collected from 20 patients with LSCS (thickened flavum group) and 10…

Comparison between MRI and myelography in lumbar spinal canal stenosis for the decision of levels of decompression surgery.

STUDY DESIGN: A retrospective case-control study. OBJECTIVE: To investigate differences between number of levels of decompression surgery for lumbar spinal canal stenosis (LSCS) as decided by magnetic resonance imaging (MRI) and myelography with computed tomographic myelography (CTM), and to investigate intraobserver reliability and interobserver reproducibility of MRI and myelography with CTM. SUMMARY OF BACKGROUND DATA: Although the decision to carry out decompression surgery for LSCS largely depends on the severity of stenosis on imaging along with clinical findings, no universally accept…

Morphometric determinants of the sagittal dimensions of the cervical spinal canal in achondroplasia: an analysis of the reliability of the Torg ratio.

This study was undertaken to evaluate the accuracy of the Torg ratio for predicting the spinal canal stenosis in achondroplasia, by trying to establish its correlation with the corresponding effective sagittal canal diameter (ESCD) and dural sac ratio (DSR) on the MRI of the cervical spine. Lateral radiographs and the sagittal and axial MRI study of the cervical spine from C3 to C7 level were carried out in 18 asymptomatic achondroplasia subjects. A total of 90 levels were evaluated on the lateral radiographs and on the mid sagittal T2 MR images. On statistic analysis, the highest correlation …

Evidence for rapid fluid flow from the subarachnoid space into the spinal cord central canal in the rat.

Horseradish peroxidase (HRP) was used as a CSF tracer in Sprague-Dawley rats. One group of rats received an injection of HRP in the cisterna magna and a second group was injected in the thoracic spinal subarachnoid space. The animals were sacrificed 0, 10 or 30 min after HRP injection by rapid perfusion with paraformaldehyde and glutaraldehyde. In both groups, there was rapid HRP labeling of brain and spinal cord perivascular spaces. HRP was present in the central canal in a pattern that was not consistent with flow from the fourth ventricle: in both groups there were segments of unlabeled cen

Development of a mathematical model of the cervical spinal canal and its use in detecting early pathologic changes.

Detection of subtle osseous changes on plain film roentgenograms of the cervical spine is essential in expediting the evaluation of suspected intraspinal tumors. It is our experience that the earliest osseous changes in such cases involve thinning or erosion of the adjacent pedicles and/or lamina and a subsequent increase in the oblique diameter of the cervical spinal canal. A statistical model is established which accurately describes the normal cervical vertebral column in terms of its spinal canal size (sagittal and oblique diameters) as well as pedicle and lamina thickness. Data was obtain…
