Linking spinal circuit reorganization to recovery after thoracic spinal cord injury.

Rats exhibit significant recovery of locomotor function following incomplete spinal cord injuries, albeit with altered gait expression and reduced speed and stepping frequency. These changes likely result from and give insight into the reorganization within spared and injured spinal circuitry. Previously, we developed computational models of the mouse spinal locomotor circuitry controlling speed-dependent gait expression (Danner et al., 2017; Zhang et al., 2022). Here, we adapted these models to the rat and used the adapted model to explore potential circuit-level changes underlying altered ga…

Charcot Spinal Arthropathy Secondary to Spinal Cord Injury - A Case Report.

INTRODUCTION: Pressure ulcers are common complications in immobilized patients, typically occurring over bony prominences. However, in rare instances, particularly in high-risk individuals with hematologic disorders such as sickle cell disease, ulcers may develop at unusual anatomical sites due to external medical devices. This case highlights a rare presentation of a vulvar pressure ulcer caused by prolonged application of a Thomas splint in a patient with homozygous sickle cell disease (HbSS). CASE REPORT: A 24-year-old woman with HbSS was admitted with a closed distal femoral shaft fractur

Split cord malformation concomitant with spinal teratoma without open spinal dysraphism.

PURPOSE: Split cord malformation (SCM) presenting concomitant with spinal teratoma without any open spinal dysraphism has rarely been reported in the literature. We aimed to make a systematic review and qualitative analysis of the literature about the topic and present the first case of SCM concomitant with spinal teratoma harboring papillary thyroid carcinoma (PTC) component. METHODS: Two big search tools (Pubmed/MEDLINE) and Scopus were used. The search strategy was compatible to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). An exemplary case of ours was also

[Spinal infection with spinal cord/cauda equina affection requires emergency handling].

Seventeen cases of infections in spinal structures were reported 2010-2017 to the Swedish Health and Social Care Inspectorate (IVO), a government agency responsible for supervising health care, for missed or delayed diagnosis. All patient records were scrutinized in order to find underlying causes and common factors. The delayed diagnoses were equally found among men and women and most frequent in in the age-group 65 to 79 years of age. The diagnostic delay most probably in many cases led to patient harm and avoidable sequelae, many with severe impairment for daily life. Several of the patient

[Spontaneous spinal epidural hematoma as cause of incomplete spinal cord injury].

Spontaneous spinal epidural hematomas are rare but are of differential diagnostic importance due to the potentially dramatic progression through to irreversible neurological deficits. At the beginning the clinical symptoms are non-specific and the development of neurological deficits leads to the diagnosis. We present the case of a 73-year-old female patient who initially reported uncharacteristic neck pain and developed incomplete quadriplegia during the next day as well as the case of a 78-year-old male patient, who complained of acute back pain and developed paraplegia a short time afterwar

Spinal cord reactive-antibodies identified by serological antigen selection show prognostic value in traumatic spinal cord injury patients.

Outcome prediction after traumatic spinal cord injury (SCI) remains challenging due to patient heterogeneity, highlighting the need for better prognostic tools. Neural tissue damage and blood-spinal cord barrier disruption expose the immune system to spinal cord proteins, eliciting autoantibody responses that may be beneficial or detrimental. This study aimed to identify the (auto)antibody profile of SCI patients, and examine the prognostic antibody biomarker potential. A healthy and a SCI cDNA phage display library were screened for novel antibodies using SCI samples (n = 12/11). Antibody rea…

Inhibiting glucocorticoid receptors enhances adult spinal cord neural stem cell activity and improves outcomes in spinal cord injury.

The spinal cord injury (SCI) microenvironment undergoes significant biochemical changes that influence the behavior of endogenous neural stem/progenitor cells (NSPCs), including elevated glucocorticoid levels. Here, we investigate the effects of glucocorticoids on NSPCs in the adult mouse spinal cord. Adult spinal cord NSPCs express glucocorticoid receptors but not mineralocorticoid receptors. Glucocorticoid exposure significantly inhibits NSPC proliferation, neurosphere formation, and differentiation through glucocorticoid receptor activation. In a traumatic SCI mouse model, treatment with th…

Automated epidural spinal cord stimulation for cardiovascular regulation in spinal cord injury: from optimization to real-time implementation.

BACKGROUND: Spinal cord injury (SCI) is frequently associated with orthostatic hypotension, defined by a sustained decrease in blood pressure upon assuming an upright posture due to impaired autonomic regulation. Cardiovascular spinal cord epidural stimulation (CV-scES) can regulate systolic blood pressure (SBP) in people with SCI, but stimulation paradigms are highly individualized. To make this treatment available to more patients, we developed an algorithm to tailor individualized CV-scES paradigms that closely mimic researcher-developed paradigms. METHODS: We performed an offline analysis…

Fecal microbiota transplantation promotes functional recovery in mice with spinal cord injury by modulating the spinal cord microenvironment.

BACKGROUND: spinal cord injury (SCI) disrupts the gut microbiota, worsening the injury's impact. Fecal microbiota transplantation (FMT) is increasingly recognized as a promising strategy to improve neural function post-SCI, yet its precise mechanisms are still far from clear. The present study aims to elucidate how FMT influences motor function recovery and its underlying mechanisms utilizing a SCI mouse model. METHODS: Mice with SCI received FMT from healthy donors. We used 16 S rRNA amplicon sequencing to analyze the alterations of gut microbes. Pathological alterations in the spinal cord t…

Endothelial ephrin-B2 knockdown increases post-traumatic disruption of the blood-spinal cord barrier following spinal cord injury.

OBJECTIVES: Traumatic Spinal Cord Injury (SCI) disrupts the Blood-Spinal Cord Barrier (BSCB), amplifying injury processes. Targeted therapies require deeper insight into underlying mechanisms. Endothelial Ephrin-B2, a guidance molecule regulating cell adhesion and repulsion, might influence the BSCB, but its role after SCI remains unknown. This study investigates Ephrin-B2 in endothelial cells (ECs) and its influence on BSCB integrity in a mouse SCI model up to 28 days. METHODS: Transgenic mice with tamoxifen-inducible EC-specific Ephrin-B2 knockdown (efnb2iΔEC, n = 79) and Cre-negative litte…

Interventions to Optimize Spinal Cord Perfusion in Patients With Acute Traumatic Spinal Cord Injury: An Updated Systematic Review.

STUDY DESIGN: Systematic review update. OBJECTIVES: Interventions that aim to optimize spinal cord perfusion are thought to play an important role in minimizing secondary ischemic damage and improving outcomes in patients with acute traumatic spinal cord injuries (SCIs). However, exactly how to optimize spinal cord perfusion and enhance neurologic recovery remains controversial. We performed an update of a recent systematic review (Evaniew et al, J. Neurotrauma 2020) to evaluate the effects of Mean Arterial Pressure (MAP) support or Spinal Cord Perfusion Pressure (SCPP) support on neurologica…

Mitigating sTNF/TNFR1 activation on VGluT2 + spinal cord interneurons improves immune function after mid-thoracic spinal cord injury.

Spinal cord injury (SCI) is a devastating condition with 250,000 to 500,000 new cases globally each year. Respiratory infections, e.g., pneumonia and influenza are the leading cause of death after SCI. Unfortunately, there is a poor understanding of how altered neuro-immune communication impacts an individual's outcome to infection. In humans and rodents, SCI leads to maladaptive changes in the spinal-sympathetic reflex (SSR) circuit which is crucial to sympathetic function. The cause of the impaired immune function may be related to harmful neuroinflammation which is detrimental to homeostati…

Can minor trauma cause asymptomatic cervical spinal cord compression leading to severe cervical spinal cord injury in rats?

The study aimed to determine whether asymptomatic rats with cervical spinal cord compression (CSCC) experience more severe cervical spinal cord injury (SCI) compared with rats without CSCC under the same degree of minor trauma. Four weeks after the polyvinyl alcohol-polyacrylamide hydrogel was implanted into the C5 vertebral canal, asymptomatic rats were selected based on locomotor function score. Mild cervical SCI was subsequently established based on CSCC. The motor function, morphology, neuron loss, myelin destruction, nerve cell apoptosis, microglia activation, and neuroinflammation were e…
