Dislocations of the lower cervical spine.

In 34 cases of cervical spine facet dislocation treated between 1975 and 1979, the dislocations were reduced by closed methods and immobilized in the halo thoracic brace. If closed reduction was unsuccessful, open reduction and fusion were performed. There is a high incidence of failure of closed reduction and halo thoracic immobilization in patients with minimal or no neurologic deficit. Bilateral facet dislocation was more frequently associated with severe neurologic injury than was unilateral facet dislocation, Use of closed reduction is time consuming, often unsuccessful, and may result in…

Acute osteomyelitis of the spine with paraplegia.

A case is reported of acute osteomyelitis of the lumbar spine presenting with a psoas abscess which extended into the epidural space mimicking an epidural abscess. The patient also had severe septicaemia and a complete cauda equina lesion. The septicaemia was controlled by draining the abscess by an anterior approach to the lumbar spine. Decompression of the cord was achieved by removing the sequestrated discs, the necrotic body of the fourth lumbar vertebra and draining the epidural abscess by opening the posterior longitudinal ligament.

CT in the evaluation of spine trauma.

Fifteen patients admitted for spine trauma in an 8 month period were studied with computed tomography (CT). All the patients had initial routine plain film screening, and 10 of 15 were also examined with conventional tomography. Five patients sustained vertical fall, axial-load injuries in the thoracolumbar junction region; two others suffered missile injury to the spine. CT provided more information than plain films in all these patients due to its superior imaging of bony detail and its ability to assess soft-tissue damage. In four of these patients, conventional tomography was done but cont…

Fracture of the anterior nasal spine.

Fracture of the anterior nasal spine is so rare that clinicians do not specifically look for it if other fractures of the middle third of the face have been ruled out. However, as suggested by this case, the possibility of fracture of the nasal spine should not be overlooked.

Benign lesions of the cervical spine.

Benign tumours of the spine are infrequent. Among the bony lesions occurring in children and adolescents are osteochondroma, benign osteoblastoma, and aneurysmal bone cyst. Although more than 50% of the osteochondromas are found in patients below 20 years of age, their incidence in the vertebrae is very low. In contrast, benign osteoblastoma and aneurysmal bone cyst are rare lesions, but occur relatively commonly in the spine. A case report of each of these bony lesions is presented. The symptomatology, the characteristic radiological and pathological appearance, and the appropriate treatment …

Sagittal fractures of the cervical spine.

Two cases of sagittal fracture of cervical vertebral bodies have been studied. The findings indicate that the fractures were a consequence of forces directed forwards against the posterior surfaces of the vertebral bodies due to flattening of the vertebral rings in an anterior--posterior direction.

Traumatic kyphosis of the thoracolumbar spine.

The rationale of acute and chronic orthopedic management of traumatic throacolumbar kyphosis is determined by the type and location of the injury. The performance of laminectomy is condemned due to its historical failure in both the acute and chronic phase and its violation of simple mechanical principles.

Orthotic management of the myelomeningocele spine.

The appropriate use of spinal orthosis is an important adjunct in the rehabilitation of spinal deformity in children with myelomeningocele. The Newington developmental programme described here employs a series of orthotic devices, used at different stages of development, with the aim of achieving a developmental sequence as close as possible to the usual age for normal development.

Injuries of the cervical spine in football.

The neck is a vulnerable area in contact sports in general and in football in particular. The type of injuries encountered often vary with the age and development of the players. Five well-established mechanisms of injury have been identified. Most injuries appear to occur during the act of tackling when the well protected head sustains violent trauma which is transferred directly to the neck. The most dangerous single mechanism is that of flexion, but lateral deviation, extension and impaction also have been identified as mechanisms of injury. The pattern of injuries would suggest that severa
