A 58-year-old man with cyclic thrombocytopenia who was initially diagnosed as idiopathic thrombocytopenic purpura (ITP), concomitant with chronic thyroiditis and ankylosing spondylitis, was reported. Serum level of T 3 (0.48 ng/ml) and T 4 (2.1 micrograms/ml) were both subnormal and that of TSH (257.1 microU/ml) was markedly elevated. The examination included 35 patients with clinical and X-ray signs of tuberculous spondylitis. Magnetic resonance tomography (MRT) was performed on a Maganview apparatus (Finland). MRT helped to diagnose an active process in 5 patients. The most manifested changes were found on T2-suspended tomograms to indicate an inflammatory reaction of the bone marrow. MRT was effective in detecting bone marrow compression, its early atrophy, hydromyelia and postoperative changes. MRT can obviously fully replace pneumomyelography and myelography with contrast substances.

The problem of correlation of rheumatic diseases with mobility of joints in children and young individuals was not frequently been considered. The first of a few authors was C.J. Sutro who was concerned with this problem in 1947. The goal of our treatment was to prevent as long as possible the onset of ankylosis in our patients. They were treated by NSAID as well as by intensive kinesi- and hydrotherapy. All these patients, besides HLA-B27 have also A2 antigen, a possible gen for hypermobility. A prospective study should explain the significance and role of A2 antigen in these patients.

We present a retrospective follow-up study of 24 patients with spondylitis or spondylodiscitis whose treatment included surgical intervention. Tuberculous spondylitis was diagnosed in 14 patients and 10 suffered from non-specific spondylitis. The average age of the patients was 50.2 years and average follow-up was 3 years. Cauda equina type of syndrome with neuropathic bladder is a rare but known complication of long-standing ankylosing spondylitis. Thirty-two patients with tubercular lumbar spondylodiskitis were studied by using traditional x-rays and echography. Computed tomography (CT) scans were also employed in six patients. Ultrasound scans detected tubercular abscesses in 17 cases, whereas traditional x-rays diagnosed abscesses in only 10. Echographic patterns are reported depending on the site and contents. Besides assessing the abscess, it was possible to diagnose a case complicated with hydronephrosis due to compression of the ureter. The polymorphonuclear leucocyte (PMN) response to a chemotactic or chemokinetic stimulus is enhanced in men with ankylosing spondylitis (AS). This effect does not parallel the severity of disease activity or the size of the acute phase response, and it is independent of non-steroidal anti-inflammatory drug treatment. Three cases of cauda equina syndrome in long-standing ankylosing spondylitis are reported. In all, vertebral scalloping and dural ectasia were confirmed by magnetic resonance imaging and computed tomography. MRI showed widening of the dural sac with signal intensity corresponding to cerebrospinal fluid. CT demonstrated asymmetrical lesions of the posterior elements of the lumbar spine. Myelography was not felt necessary to confirm the findings.

HL-A antigens were determined in Haida and Bella Coola native Indians, two communities known to have a high prevalence of ankylosing spondylitis. Tests were conducted on those with x-ray evidence of sacroiliitis and on a sample of the community at large. Sacroiliitis was found to prevail in approximately 10% of adult Haida males and in over 2% of Bella Coola adult males. Of 20 Haidas with sacroiliitis. 17 were HL-A 27 positive. Acute anterior uveitis (AAU) is more often seen in ankylosing spondylitis (AS) patients with severe disease than in HLA-B27 positive relatives with or without sacroiliitis. Possibly the prevalence of AAU is associated with the severity of AS.
