PCSK9 at the intersection of lipid metabolism, immunometabolism, and psychiatric disorders: A narrative review.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a central regulator of lipid metabolism and a well-established determinant of cardiovascular risk. In addition to its hepatic role in cholesterol homeostasis, PCSK9 is increasingly linked to immunometabolic processes that overlap with biological alterations commonly observed in psychiatric disorders, including dyslipidemia, insulin resistance, and low-grade systemic and neuroinflammation. This narrative review synthesizes experimental, genetic, and clinical evidence on lipid metabolism and PCSK9-related pathways in psychiatric disorders.

Structure-guided design of a PCSK9 epitope vaccine with efficacy against hyperlipidemia and atherosclerosis.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) plays a central role in regulating low-density lipoprotein cholesterol (LDL-C) levels and has emerged as an attractive target for atherosclerotic cardiovascular disease (ASCVD) therapy. While mono-clonal antibodies targeting PCSK9 have demonstrated clinical efficacy, their high cost and need for repeated administration limit widespread use. In this study, we developed a peptide-based vaccine by identifying B-cell epitopes from PCSK9-antibody complexes using the Protein Data Bank (PDB) structural data and AlphaFold3 prediction, and fusing th

The evolving therapeutic landscape of PCSK9 inhibition.

Cardiovascular disease remains the leading cause of death worldwide with low density lipoprotein being a major, yet modifiable, risk factor. Proprotein convertase subtilisin/kexin type 9 (PCSK9) plays a central role in regulating low density lipoprotein (LDL) receptor expression. Naturally occurring loss-of-function variants in the PCSK9 gene result in lifelong lower LDL cholesterol (LDL-C) levels and significantly lower risk of atherosclerotic cardiovascular disease (ASCVD), providing strong genetic validation of PCSK9 as a therapeutic target. This insight has driven the development of therap

PCSK9 in Brain Health and Disease: Liver-Vascular-Brain Interactions in Aging and Disease.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) is a key regulator of peripheral cholesterol metabolism. Inhibitors of hepatic PCSK9 potently reduce low-density lipoprotein cholesterol (LDL-C) and have become a staple of lipid-lowering therapeutic regimens for patients with atherosclerotic cardiovascular disease. Although the role of PCSK9 in the central nervous system (CNS) is less well-defined, accumulating evidence suggests that it modulates CNS cholesterol homeostasis and responses to injury, neuroinflammation, and amyloid-β and tau protein deposition. Thus, peripheral and central PC

Cellular lipids: fundamental host factors for monkeypox virus replication and pathogenesis.

The recent outbreaks of mpox and other poxvirus diseases worldwide have highlighted the critical need for effective antiviral treatments. Throughout its life cycle, MPXV, a member of the Orthopoxvirus genus, takes advantage of the host’s lipid metabolism. In addition to being essential for maintaining cellular homeostasis, lipids are also used by viruses for entrance, reproduction, assembly, and egress. Lipid rafts (LRs), membrane microdomains rich in cholesterol that are essential for viral entry and viral envelope structure, are a crucial component of this exploitation. This study summarizes

Atherosclerotic plaque stabilization and regression.

Atherosclerotic plaques represent a typical deterioration of arterial wall in atherosclerotic process and are the source of cardiovascular events. Plaque progression and composition represent a major risk for cardiovascular events. Therefore, recently many studies have assessed changes in plaque characteristics and their response to various treatment modalities. In the last two decades, improvement in plaque imaging modalities that can assess plaque volumes and composition enable to follow plaque characteristics in a non-invasive way. Clinical trials utilizing arterial imaging modalities have

Design of optimized epigenetic regulators for durable gene silencing with application to PCSK9 in nonhuman primates.

Epigenetic editing is a promising strategy for modifying gene expression while avoiding the permanent alterations and potential genotoxicity of genome-editing technologies. Here we designed optimized epigenetic regulators (EpiRegs) by testing combinations of transcription activator-like effector (TALE)-based and catalytically deactivated Cas9 (dCas9)-based epigenetic modification effectors and fusion protein structures. TALE-based EpiReg (EpiReg-T) achieved a final efficiency of 98% in mice, surpassing the initial dCas9-based efficiency of 64%. We demonstrated the approach in macaques by intro

PCSK9 deficiency promotes the development of peripheral neuropathy.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) induces the hepatic degradation of the low-density lipoprotein receptor (LDLR), thereby increasing the concentration of LDL-cholesterol in the blood. Beyond its effects on LDL, recent studies have reported pleiotropic effects of PCSK9, notably in septic shock, vascular inflammation, viral infection, and cancer. While the functional and structural integrity of peripheral nerves are critically influenced by circulating lipids, the effect of PCSK9 on the peripheral nervous system remains unknown. In this study, we investigated the consequences

An up-to-date review of emerging biologic therapies for hypercholesterolemia.

INTRODUCTION: Hypercholesterolemia and other lipid disorders are major causes of atherosclerotic cardiovascular disease (ASCVD). Statins have been the mainstay of lipid-lowering therapy for many years, but they may not be adequate to achieve the target low-density lipoprotein (LDL) cholesterol levels and there are other residual lipid risk factors. AREAS COVERED: This article reviews the biologic therapies in development for hypercholesterolemia identified by a PubMed search. Inhibition of proprotein convertase subtilisin/kexin type 9 (PCSK9) is a major focus, but the drugs targeting apolipop

Elevated Lp(a): Guidance for Identifying and Managing Patients.

Lipoprotein(a) (Lp(a)) is a unique low-density lipoprotein-like lipoprotein that is considered an independent and causal risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis. The Lp(a) molecule also contains apolipoprotein A and apolipoprotein B, which collectively promote atherosclerosis, thrombosis, and inflammation. Lp(a) is highly genetic and minimally responsive to nonpharmacological measures. Lp(a) serum levels ≥125 nmol/L are associated with increased ASCVD risk, but this threshold has not been accepted universally. Elevated Lp(a) is the most

Susceptibility of PCSK2 Polymorphism to Hirschsprung Disease in Southern Chinese Children.

INTRODUCTION: Hirschsprung's disease (HSCR) is a developmental defect of the enteric nervous system (ENS), which is caused by abnormal development of enteric neural crest cells. Its occurrence is caused by genetic factors and environmental factors. It has been reported that single nucleotide polymorphisms (SNPs) of proprotein convertase subtilisin/kexin type 2 (PCSK2) gene are associated with HSCR. However, the correlation of HSCR in southern Chinese population is still unclear. METHODS: We assessed the association of rs16998727 with HSCR susceptibility in southern Chinese children using TaqM

Hyperlipidemia and hypothyroidism.

Hypothyroidism is closely associated with increased serum total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C) and triglyceride (TG). The thyroid gland plays an important role in this process because thyroid hormones (THs) modulate cholesterol production, transformation and clearance. Although recent evidence suggests that thyroid-stimulating hormone (TSH) itself also participates in hyperlipidemia, the underlying mechanism remains unclear. Others demonstrated that the pathologic development of hypothyroidism-related hyperlipidemia was associated with down-regulated THs and up-r

The role of PCSK9 in NAFLD/NASH and therapeutic implications of PCSK9 inhibition.

INTRODUCTION: There are inconsistent findings regarding the effect of lipid-lowering agents on nonalcoholic fatty liver disease (NAFLD). Proprotein Convertase Subtilisin/Kexin type 9 (PCSK9) is an important player in cholesterol homeostasis and intracellular lipogenesis, and PCSK9 inhibitors (PCSK9-i) have been found to be efficient for pharmacological management of hyperlipidemia. AREAS COVERED: Whether PCSK9 (itself) or PCSK9-i affects NAFLD is still disputed. To address this question, we review published preclinical and clinical studies providing evidence for the role of PCSK9 in and the e

Novel insights into the pathological mechanisms of metabolic related dyslipidemia.

Due to the technological advances, it has been well-established that obesity is strongly correlated with various health problems. Among these problems, dyslipidemia is one of the most important concomitant symptoms under obese status which is the main driving force behind the pathological progression of cardio-metabolic disorder diseases. Importantly, the type of dyslipidemia, arising from concerted action of obesity, has been identified as "metabolic related dyslipidemia", which is characterized by increased circulating levels of Low density lipoprotein cholesterol (LDL-C), Triglycerides (TG)

Platelet-Derived PCSK9 Is Associated with LDL Metabolism and Modulates Atherothrombotic Mechanisms in Coronary Artery Disease.

Platelets play a significant role in atherothrombosis. Proprotein convertase subtilisin/kexin type 9 (PCSK9) is critically involved in the regulation of LDL metabolism and interacts with platelet function. The effect of PCSK9 in platelet function is poorly understood. The authors of this article sought to characterize platelets as a major source of PCSK9 and PCSK9's role in atherothrombosis. In a large cohort of patients with coronary artery disease (CAD), platelet count, platelet reactivity, and platelet-derived PCSK9 release were analyzed. The role of platelet PCSK9 on platelet and monocyte

Treatment of dyslipidemia in kidney transplantation.

Introduction: Lipid disorders are frequent after kidney transplantation (KT) and KT recipients are considered at high- or very-high cardiovascular risk. Among many concurring factors, a major role is played by immunosuppressants.Areas covered: General measures to manage lipid disorders first include physical activity and diet counseling. Modulating the doses of immunosuppressants also improves dyslipidemia. When lipid-lowering drugs are necessary to control elevated plasma cholesterol and/or triglycerides, statins are the cornerstone for managing hypercholesterolemia. However, side-effects (e.

A milk-derived pentapeptide reduces blood pressure in advanced hypertension in a CCK system-dependent manner.

We recently found that a peptide that activates the cholecystokinin (CCK) system effectively reduces blood pressure in spontaneously hypertensive rats (SHR) after the development of hypertension, after which hypotensive drugs are sometimes less effective. In this study, we investigated the vasorelaxation and antihypertensive effects of a peptide derived from a milk protein in SHR with advanced hypertension. The vasorelaxing activity was measured using the mesenteric artery isolated from SHR and the systemic blood pressure was measured by the tail-cuff method. KFWGK was released from bovine ser
