Remote discovery of an asymptomatic bowel perforation by a mid-urethral sling.

Bowel perforation is a rare complication of mid-urethral sling procedures and is usually reported shortly after the surgery. We report a remotely discovered asymptomatic bowel injury found at the time of subsequent surgery. The patient with a history of several prior pelvic surgeries underwent an uneventful retropubic mid-urethral sling placement. Five years later, during an abdominal sacrocolpopexy procedure, mesh from the mid-urethral sling was found perforating the wall of the cecum and fixating it to the right pelvic sidewall. Cecal wedge resection was performed to excise the sling mesh. A

Are male slings for post-prostatectomy incontinence a valid option?

PURPOSE OF REVIEW: Stress incontinence in men is still a common problem after surgical treatment of prostatic disease. This article reviews the techniques and results of recently described surgical slings available to treat male stress incontinence. RECENT FINDINGS: Medium-term follow-up (mean 3-4 years) of patients treated with bone-anchored slings has been recently published, suggesting a success rate (cured or improved) of 70-80%. Short-term follow-up (mean of 6-12 months) of transobturator retrourethral slings demonstrates a success rate of 62-83%. Novel sling designs include mechanisms t

Etiology of post-uterosacral suspension neuropathies.

INTRODUCTION AND HYPOTHESIS: The goal of our investigation was to find a neurological explanation for neuropathies reported following some uterosacral ligament suspension (USLS) [2-3]. METHODS: We dissected the neural structures beneath the USL in seven female, adult, embalmed cadavers. We made a literature review to determine the spinal nerve sensory fiber composition of each exposed neural structure and the dermatome(s) that it innervates. We then compared anticipated sensory neuropathies for each neural structure with neuropathies following USLS to determine which neural structure entrapme

Actinomyces infection associated with the transobturator sling.

We present a case of Actinomyces infection associated with the transobturator sling. The patient had a transobturator sling complicated only by a left vaginal sulcal perforation. She subsequently developed mesh erosion that led to two partial mesh resections performed in the operating room at 6 weeks and again at 6 months after the initial surgery. She subsequently required a groin dissection and removal of the entire left side of the sling due to persistent infection. The presence of a foreign body is believed to create an environment that makes growth of the bacteria more likely. With increa

Robotic-assisted laparoscopic approach for posterior bladder neck dissection and placement of pediatric bladder neck sling: initial experience.

OBJECTIVES: Bladder neck sling cystourethropexy is a common procedure used to correct intrinsic sphincter deficiency in children with spinal dyspharism. Various modifications of the procedure have been made but all involve circumferential dissection of the bladder neck and proximal urethra. The posterior dissection can be challenging and can result in injury to the rectum, urethra, or vagina. The posterior approach to the bladder neck as reported by Lottmann and later by de Badiola addresses these potential complications. Using these principles of the posterior approach, we performed a robotic

Assessment of TVT efficacy in the management of patients with genuine stress incontinence with the use of epidural vs intravenous anesthesia.

Our aim was to validate the use of intravenous anesthesia as an alternative to epidural anesthesia for the placement of tension-free vaginal tape (TVT) in patients with urodynamic stress incontinence. Eighty-six patients participated in this prospective study. Forty-five patients were operated with intravenous anesthesia and 41 patients with epidural anesthesia. All patients had a full history taken and a complete gynecological examination performed at initial visit. Preoperative and postoperative urodynamic investigations included filling and voiding cystometry, urethral profilometry, uroflow

Audit of compliance with NICE guidelines on the use of tension-free vaginal tape slings for stress incontinence.

The aim of this audit was to assess adherence to the National Institute of Clinical Excellence (NICE) guidelines in inserting tension-free vaginal tape (TVT) slings for urodynamic stress incontinence, in gynaecology and urology settings in a district general hospital. It included 95 patients (79 inserted by gynaecologists and 16 by urologists) who had TVT sling insertion from 2002 until 2005, at the Royal Bournemouth Hospital. Conservative measures were tried by 43 patients (45.3%) and documentation of risk factors varied between 70% for failure and intermittent catheterisation and 1% for lapa

The effect of spinal anaesthesia on urethral function.

This study was to assess the effect of spinal anaesthesia on urethral retro-resistance pressure (URP), cough pressures and tendency to leak. The population consisted of 32 women undergoing a tension-free vaginal tape (TVT) operation under a spinal anaesthetic. URP, cough pressures and an assessment of the degree of leak were performed before the spinal anaesthetic was placed. A standard anaesthetic technique was used, and measurements were repeated after the spinal anaesthetic was inserted. The degree of leak was assessed on a five-point scale with 350 ml in the bladder. The cough pressures an

Perineal cellulitis and persistent vaginal erosion after transobturator tape (Obtape)--case report and review of the literature.

Transobturator sub-urethral tapes are emerging as one of the surgical options for the management of urodynamic stress incontinence. Though with relatively less risk of injury to the bladder, a different approach as well as the different characteristics of the tape material mean a variety of complications like obturator abscess, obturator haematoma, retro-pubic haematoma, and perineal cellulitis. In this paper, we describe a case of perineal cellulitis following the insertion of a transobturator tape (Obtape). Though cellulitis was managed conservatively with intravenous antibiotics, the patien

Cecal perforation complicating placement of a transvaginal tension-free vaginal tape.

The tension-free vaginal tape has been increasingly used to treat stress urinary incontinence. This procedure has a high success rate and unique surgical complications. The patient is a 39-year-old with genuine stress urinary incontinence and underwent placement of tension-free vaginal tape for treatment. Twelve hours after the procedure, the patient had increasing abdominal pain, and an acute abdominal series showed free intraperitoneal air. Exploratory laparotomy revealed stool in the peritoneal cavity, with the vaginal tape placed through the cecum. Bowel complications are rare; however, th

Multicenter experience with the Monarc transobturator sling system to treat stress urinary incontinence.

This study was conducted to report on the safety and efficacy of a transobturator sling approach for treatment of urodynamic stress incontinence (USI). Women with urodynamic stress incontinence were offered a novel polypropylene mesh sling procedure, implanted beneath the lateral pubic rami rather than retropubically. Retrospective data were collected at three U.S. sites. Two hundred women with a mean age of 59 years (range 27-93) underwent the sling procedure. Mean follow-up was 21.5 weeks (range 3-43). Perioperative safety parameters are reported for all subjects and efficacy parameters for

Detection of Subpubic Tumor Causing Bladder Outlet Obstruction by 3D Perineal Ultrasound.

This case report shows that 3D perineal ultrasound can be superior to clinical examination and routine 2D perineal ultrasound in the detection of an unusual subpubic tumor. A 73-year-old female patient was referred to our urogynecological outpatient unit complaining of over-active bladder symptoms and voiding dysfunction for 3 years. Gynecological examination found no signs of pelvic organ prolapse or abnormality in the vaginal cavity. Routine 2D perineal ultrasound showed substantial residual urine (ca. 300 ml on catheter) and limited bladder neck mobility, but no signs of pelvic organ prolap
