Fixation in Intertrochanteric Fractures Using Short Proximal Femoral Nail Anti-Rotation-2: A Functional and Radiological Prospective Study.

INTRODUCTION: Femoral shaft fractures are highly prevalent. Surgical treatment is the optimal approach for managing them as it greatly decreases the rates of illness and death in injured people. The optimal therapy for these diseases entails the utilization of nail, plates, screws, and external fixation devices. At present, reamed intramedullary nailing is regarded as the optimal approach and universally recognized treatment indicated for fractures occurring in the shaft of the femur. EXPERIMENTAL PROCEDURES AND TECHNIQUES: The study done at Krishna Hospital in Karad between February 2022 and

Outcome of delayed primary internal fixation of distal radius fractures: a comparative study.

INTRODUCTION AND AIM: Operative fixation of distal radius fractures using fixed-angle devices has become increasingly common. Although good to excellent results have been reported in acute fractures, little is currently known regarding the fixation of healing displaced distal radius fractures that were presented late. The aim of this study was to evaluate the results of internal fixation of distal radius fractures presented late (>21 days) as compared with an acute-care control group. METHODS: Forty patients operated on for displaced distal radius fractures, presenting more than 21 days after

Cement technique for reducing post-operative bursitis after trochanteric fixation.

Post-operative trochanteric bursitis is a known complication secondary to the surgical approach in total hip arthroplasty. This phenomenon may be partially attributable to repetitive microtrauma generated when soft tissues rub against implanted hardware. Significant rates of post-operative trochanteric bursitis have been observed following procedures in which a trochanteric fixation device, such as a bolt-washer mechanism or a cable-grip/claw system, is used to secure the trochanteric fragment after trochanteric osteotomy. We present a simple technique for use with a bolt-washer system or grip

Monitoring of lumbosacral nerve roots during spinal instrumentation.

A technique is described for intraoperative, multimodality electrophysiologic monitoring of the spinal cord and lumbosacral nerve roots (cauda equina). Simultaneous monitoring with the techniques of electromyography, compound muscle action potentials, and somatosensory evoked potentials is found to provide highly useful information to the surgeon during lumbar spine instrumentations. Compound muscle action potential recordings are used to identify nerve roots in the lumbosacral region, especially when visual identification of the nerve is hindered by scarring or distortion of the anatomy. Conservative management of a cervically fractured zygomatic implant using a novel titanium device: a technical report with biomechanical validation and 24-month follow-up.

Fracture of a zygomatic implant is an uncommon but clinically challenging complication. Conventional management generally involves implant removal or alternative anchorage strategies, both of which may require extensive surgery, increase patient morbidity, and compromise an existing implant-supported rehabilitation. This technical report describes the development, biomechanical validation, and first clinical application of a novel prosthetic rescue device for the conservative management of cervical fractures of zygomatic implants while preserving the osseointegrated apical segment. A customize

Intraoperative Skull Fracture During Halo Application in Subcranial Le Fort III: Strategies for Managing a Rare Complication.

External rigid distraction is an established method for achieving subcranial Le Fort III advancement in severe syndromic craniosynostosis. Craniofacial surgeons commonly use halo-type devices for these corrections, as they allow for multiple vectors of pull and facilitate larger midfacial advancements. Although most complications related to their use involve pin displacement or infection, rare complications such as skull fractures have been reported. At 3 months of age, a patient with Apert Syndrome underwent endoscopic-assisted craniectomies for bilateral coronal craniosynostosis. After a yea

Biomechanical analysis of a newly developed interspinous process device conjunction with interbody cage based on a finite element model.

PURPOSE: This study aimed to investigate the biomechanical effects of a newly developed interspinous process device (IPD), called TAU. This device was compared with another IPD (SPIRE) and the pedicle screw fixation (PSF) technique at the surgical and adjacent levels of the lumbar spine. MATERIALS AND METHODS: A three-dimensional finite element model analysis of the L1-S1 segments was performed to assess the biomechanical effects of the proposed IPD combined with an interbody cage. Three surgical models-two IPD models (TAU and SPIRE) and one PSF model-were developed. The biomechanical effects

Novel Use of Pennig External Fixation for the Treatment of Pediatric Syndactyly.

Syndactyly is one of the most common congenital limb malformations, second only to polydactyly. Syndactyly is treated with surgical intervention. During separation, free skin grafting or skin flap transplantation is required to allow for adequate skin closure. We introduce a novel application method of the Pennig external fixation device for the management of pediatric syndactyly without skin grafting. The Pennig minifixator was used in syndactyly to expand the web space soft tissue in order to allow a Z-plasty to be performed in a second operation. All patients had developed sufficient skin t

Guidance for the Treatment of Femoral Neck Fracture with Precise Minimally Invasive Internal Fixation Based on the Orthopaedic Surgery Robot Positioning System.

Femoral neck fractures are among the most common fractures in orthopaedics. There are many surgical methods for the treatment of femoral neck fracture. Percutaneous cannulated lag screw fixation for the treatment of femoral neck fractures is favored by orthopaedic doctors because of its characteristics of being minimally invasive, with less bleeding and firm fixation. However, traditional freehand screw placement is limited by many factors, and the screw malposition rate is very high, which directly leads to the reduction of its biomechanical stability, and even leads to the ischemic necrosis

[The supracondylar fracture of the humerus in children (author's transl)].

To find out the cause of posttraumatic varus and valgus deformity of the elbow a long-term follow-up examination of 183 dislocated and 20 undislocated supracondylar extension-fractures of the humerus was done. There were different methods of treatment: In most of the cases closed reduction was performed and fixation in acute angle-plaster or by percutaneous radial or radial and ulnar wires. 75% showed radiologically and 55% clinically an alteration of the carrying angle. The clear reason for this deformity was a rotation displacement, which leads in oblique fractures directly, in transverse fr

[Results of treatment of acute injuries of the thoracolumbar spine by ridged plates.].

The authors evaluate the results of surgery of fresh injuries of the thoracolumbar spine achieved by transpedicular fixation, using ridged plates. At the orthopaedic clinic in Brno-Bohunice this technique was systematically introduced in 1989; in 1991 they started to use a better principle of stabilization by an internal fixation device. The authors used plates fixed in a transpedicular way in 58 injured subjects. Most frequently fractures of L1, T12 and L2 were involved, usually of a burst type. In more than half the patients they recorded a nervous deficit of varying degree. They stabilized

[Pharmacological treatment of obstructive sleep apnea syndrome].

Obstructive sleep apnea syndrome (OSAS) is a potentially life-threatening condition that is increasingly prevalent in developed countries. OSAS treatment involves multiple components, including general lifestyle modifications and specific interventions such as intraoral fixation devices and continuous positive airway pressure (CPAP). Surgical treatments, including correction of nasopharyngeal and oropharyngeal structures, orthognathic operations, and bariatric surgery, are used under strict indications. Until recently, pharmacological management was limited to nasal decongestants for rhinitis
