The Effect of Reinforcement Methods with Knotted and Knotless Sutures: Comparison of Burst Pressures.

Sleeve gastrectomy operations have become popular among restrictive obesity surgeons, due to its effectiveness for losing weight and acceptable complication rates. However, leakage is a significant problem in these operations. In this ex vivo study, we tried to understand mechanisms of leakage and to examine the effects of two different types of frequently referred reinforcement techniques using sutures. The resected gastric specimens that were removed during sleeve gastrectomy operations were used in this ex vivo study. Foley catheters were inserted to stomach from two different areas along t…

Biomechanical Contribution of Tension-Reducing Rotator Cuff Sutures in 3-Part Proximal Humerus Fractures.

OBJECTIVES: Using a cadaveric 3-part fracture model and cyclic loading protocol, our study objectives were to quantify the stabilizing effect of tension-reducing rotator cuff sutures in terms of fracture displacement across the surgical neck and greater tuberosity compared with a control group in which no sutures were used. METHODS: Six matched pairs of fresh frozen specimens underwent a standardized, 3-part, proximal humerus fracture and were split into 2 groups. The control group had the fracture fixed with a plate and screw construct only while the experimental group had additional suture …

Gene expression changes between patent and fused cranial sutures in a nonsyndromic craniosynostosis population.

OBJECTIVE: Craniosynostosis is a premature fusion of 1 or more cranial sutures. It may occur with additional morphological abnormalities (syndromic) or in isolation. Studies suggest that dysregulation of normal cell proliferation, differentiation, and migration has a role in isolated or nonsyndromic craniosynostosis but the molecular mechanisms remain unknown. The aim of this research is to identify genes differentially expressed in prematurely fused human suture compared to patent suture in nonsyndromic craniosynostosis. METHODS: Bone fragments from synostosed and patent sutures of 7 infants…

The application of the V-Loc closure device for gastrointestinal sutures: a preliminary study.

BACKGROUND: Leakage is one of the major complications in gastrointestinal surgery. This preliminary study compared a new barbed absorbable thread for gastrointestinal sutures with monofil suture material in a cadaver model. METHODS: In this study, mechanical experiments were performed in 20 recently deceased individuals. Incisions were made in the small intestine, colon, and stomach, and then sutures were created with the V-Loc closure device and monofil suture material. Intestinal bursting pressure was measured by inserting a balloon and slowly filling it with air until there was a dehiscenc…

Effect of human urine on the tensile strength of sutures used for hypospadias surgery.

INTRODUCTION: Hypospadias is the most common congenital condition affecting between 1 in 250 and 300 live births. Even in experienced hands, surgery to repair this congenital defect can have a high complication rate. Wound dehiscence is reported to occur in 5% and fistula formation in 6%-40% depending on technique. The choice of suture material has been shown to affect the complication rate although there is (currently) no consensus about the best suture material to use. Ideally, the sutures used for urethroplasty should be absorbable while maintaining sufficient mechanical strength to support…

Tension of knotted surgical sutures shows tissue specific rapid loss in a rodent model.

BACKGROUND: Every surgical suture compresses the enclosed tissue with a tension that depends from the knotting force and the resistance of the tissue. The aim of this study was to identify the dynamic change of applied suture tension with regard to the tissue specific cutting reaction. METHODS: In rabbits we placed single polypropylene sutures (3/0) in skin, muscle, liver, stomach and small intestine. Six measurements for each single organ were determined by tension sensors for 60 minutes. We collected tissue specimens to analyse the connective tissue stability by measuring the collagen/prote…

Analysis of the strength of the abdominal fascia in different sutures used in abdominoplasties.

BACKGROUND: Protrusion of the abdominal wall secondary to abdominoplasty may occur in patients with weakness of the aponeurotic structures. The anterior layer of the rectus abdominis muscle consists of fibers that are transverse rather than vertical. Based on this anatomical feature, vertical sutures are suggested for the correction of diastasis recti, since they include a greater amount of fascial fibers and thus would be more resistant to tensile strength than horizontal ones. METHODS: The anterior layers of the rectus abdominis muscles of 15 fresh cadavers were dissected. Two vertical line…

Tensile strain-induced Ets-2 phosphorylation by CaMKII and the homeostasis of cranial sutures.

BACKGROUND: Mechanotransduction underpins the homeostasis of musculoskeletal tissues, including cranial sutures. Intracellular calcium, [Ca 2+]ic, and protein phosphorylation are two intermediate variables in signal relay during mechanotransduction. This project establishes a chain of cause and effect, linking cellular strain to substrate phosphorylation, and identifies the agent and target sites of phosphorylation. METHODS: Cyclic tensile force (0.5 N at 1 Hz) was applied to 1-day-old rat sagittal sutures. [Ca 2+]ic was measured by FURA-2. Ets-2 phosphorylation by CaMKII was tested using Wes…
