Clinical and Radiographic Outcomes With Assessment of the Learning Curve in Arthroscopically Assisted Latissimus Dorsi Tendon Transfer for Irreparable Posterosuperior Rotator Cuff Tears.

PURPOSE: To evaluate the clinical results of an arthroscopy-assisted latissimus dorsi tendon transfer (aLD) for irreparable posterosuperior cuff tears as a primary surgery. The secondary aim of this study was to quantify the learning curve using the log-linear model. We hypothesized that aLD significantly improved shoulder function and that there was consistent reduction of the operative time in support of a learning-curve effect. METHODS: After the arthroscopic partial repair was completed, the latissimus dorsi tendon was harvested via axillary mini-open incision and fixed with a knotless an…

Early analysis shows that endoscopic flexor hallucis longus transfer has a promising cost-effectiveness profile in the treatment of acute Achilles tendon ruptures.

PURPOSE: Current options for treating an Achilles tendon rupture (ATR) include conservative and surgical approaches. Endoscopic flexor hallucis longus (FHL) transfer has been recently proposed to treat acute ruptures, but its cost-effectiveness potential remains to be evaluated. Therefore, the objective of this study was to perform an early cost-effectiveness analysis of endoscopic FHL transfer for acute ATRs, comparing the costs and benefits of current treatments from a societal perspective. METHODS: A conceptual model was created, with a decision tree, to outline the main health events duri

Opponensplasty by the Palmaris Longus Tendon to the Rerouted Extensor Pollicis Brevis Transfer With Endoscopic Carpal Tunnel Release in Severe Carpal Tunnel Syndrome.

In patients with severe carpal tunnel syndrome (CTS), restoring thumb opposition is critical because this function is essential for proper pinching, grasping, and other complex hand movements. Opponensplasty is an effective procedure to preserve thumb function, with several methods reported. Camitz opponensplasty, using the palmaris longus (PL), is an option for patients with severe CTS. Recently, several modified Camitz procedures have been proposed to overcome the shortcomings of the original description. This article describes the surgical technique of the opponensplasty using the PL tendon

Reconstruction of a Chronic Volkmann Contracture following Forearm Revascularization with Burkhalter's Procedure and Fractional Flexor Tendon Lengthening after a Failed Stiles-Bunnell Transfer.

Volkmann ischemic contracture is a cause of limb dysfunction that can lead to varying amounts of limb deformity, stiffness, and paralysis. Several procedures have been used to improve limb function in these patients. An 11-year-old male patient sustained a domestic accident with a glass door, resulting in a severe soft tissue trauma to his proximal third of the right forearm with damage to flexor muscles and tendons, complete section of median and ulnar nerves, and radial and ulnar arteries. He was submitted to immediate repair of the radial artery and median and ulnar nerves. Ulnar artery was

Triple tendon transfer for an irreparable 10-cm radial nerve defect: a case report of functional recovery and a successful return to work as a waiter.

INTRODUCTION: Radial nerve palsy following humeral shaft fractures is common, but management of massive nerve gaps remains a challenge. While nerve grafting is an option, defects exceeding 10 cm often yield poor results. CASE PRESENTATION: We report the case of an 18-year-old male, a chronic smoker, who sustained a complete 10-cm radial nerve loss; management consisted of a triple tendon transfer. RESULTS: At 14 months, he achieved complete extension and successfully returned to his demanding professional activity. DISCUSSION: The use of the flexor carpi ulnaris for digital and thumb extens…

Open Glenohumeral Joint Reduction and Latissimus Dorsi and Teres Major Tendon Transfers for Infants and Children Following Brachial Plexus Birth Palsy.

Glenohumeral dysplasia can occur in brachial plexus birth palsy due to an imbalance of muscle forces. Most commonly this occurs in C5-C6 injuries where the shoulder external rotators are weak compared with the shoulder internal rotators. Treatment of the dysplasia with open reduction of the glenohumeral joint in combination with rebalancing the muscles can improve the development of the joint. Furthermore, the tendon transfer can improve shoulder function by decreasing the internal rotation forces about the shoulder while increasing the external rotation forces. This paper describes the indica…

Concurrent Nerve and Tendon Transfers in a Tetraplegic Patient: Long-Term Functional Recovery and Late Hardware-Associated Flexor Pollicis Longus Rupture.

Tetraplegia is a significant impairment of mobility in all four limbs, often following a cervical spinal cord injury. Recent studies in the management of tetraplegia describe the use of nerve transfer in addition to tendon transfer for upper extremity rehabilitation. While the popularity of these techniques is growing, there are limited studies describing the long-term outcomes and functional recovery following such procedures. This case describes a patient who sustained a C7 spinal cord injury, right olecranon fracture, and left distal radius fracture following a fall. Surgeons performed open…

Outcomes of flexor carpi radialis tendon transfer surgery for patients with radial nerve injury: a comparison of the wide-awake method and general anesthesia.

Advanced intraoperative anesthesia techniques have improved patient cooperation, stabilized hemodynamic status, and reduced postoperative pain and disability, especially in reconstructive limb surgeries. The Wide-Awake Local Anesthetic No Tourniquet (WALANT) technique has recently been adopted due to its peculiar advantages, but comparative studies of its benefits and limitations against general anesthesia are not well explored in FCR tendon transfer surgery. This study investigates outcomes of Flexor Carpi Radialis (FCR) tendon transfer surgery in patients with radial nerve injuries using eit…
