Implementation and evaluation of the 3 Wishes Project in safety-net hospitals: Protocol for a hybrid effectiveness-implementation study.

BACKGROUND: Palliative care interventions in the intensive care unit have been shown to improve communication and the quality of death and dying. However, few interventions have been implemented in safety-net hospitals (SNHs), which provide healthcare to low income and uninsured patients. The 3 Wishes Project (3WP) is a low-cost intervention that aims to enhance compassionate end-of-life (EOL) experiences by empowering the clinical team to elicit and fulfill small wishes for patients who are dying in the ICU. OBJECTIVE: To implement and evaluate the 3WP in three SNHs in Los Angeles. METHODS:

Kangaroo Cuddle Transport for Neonatal Palliative Care: A Service Evaluation.

BACKGROUND: Newborn infants represent a significant percentage of childhood deaths (>60%) with over 90% of newborn end-of-life care taking place in an acute hospital setting. This is in direct contrast to adults and children where specialised hospital or hospice care is used during their palliative care experience. To support family choice, a neonatal retrieval service developed a new model of care to transfer babies from a hospital to a hospice, a hospital closer to home or a private residence for palliative care while being cuddled by a parent. AIM: Identify and evaluate (i) service workflo

Life history and meanings: end of life in an intensive and semi-intensive care unit.

This study addressed the end-of-life experience of a patient admitted to an intensive and semi-intensive care unit based on the accounts of a family member who accompanied her therapeutic journey. The study sought to understand how healthcare teams consider patients' life histories and how this influences care strategies. We also sought to identify the interaction between healthcare teams and patient family members when patients are no longer able to communicate. Immersion in an intensive and semi-intensive care unit enabled an ethnographic approach involving participant observation, in-depth

Future care planning: a first step to palliative care for all patients with advanced heart disease.

Palliative care is recommended for patients with end-stage heart failure with several recent, randomised trials showing improvements in symptoms and quality of life and more studies underway. Future care planning provides a framework for discussing a range of palliative care problems with patients and their families. This approach can be introduced at any time during the patient's journey of care and ideally well in advance of end-of-life care. Future care planning is applicable to a wide range of patients with advanced heart disease and could be delivered systematically by cardiology teams at

Perspectives on care at the close of life. Serving patients who may die soon and their families: the role of hospice and other services.

The case story of a 47-year-old man with advanced rectal carcinoma illustrates the professional services and care system strategies available to help clinicians serve patients coming to the end of life. For this patient, who understands his prognosis, primary care physician services include (1) prevention and relief of symptoms, (2) assessment of each treatment before and during implementation, (3) ensuring that the patient designates a surrogate decision-maker and makes advance plans, and (4) preparation of patient and family for the time near death. Good care may entail enduring unavoidably

Impact of perception of socioeconomic burden on advocacy for patient autonomy in end-of-life decision making: a study of societal attitudes.

We investigated the impact of perception of socioeconomic burden on beliefs regarding a patient's autonomy in end-of-life (EOL) decision making. We also sought to identify the characteristics of individuals who advocate patient autonomy and their attitudes toward other EOL issues. A total of 1055 individuals from the Korean general population were interviewed through a telephone survey using a structured questionnaire that was designed to investigate public attitudes toward various EOL issues. Of 1019 individuals included in the analysis, 635 (62.3%) specified the patient and 221 (21.7%) the f

Palliative and end-of-life care research funding: an analysis of current UK health research spending.

OBJECTIVES: Despite increasing demand for palliative and end-of-life care, the funding for research in this area has historically been low. Previous UK analyses in 2016 found that less than 0.3% of the funding for cancer research was directed towards palliative care research; current expenditure on palliative and end-of-life care research is unknown. We therefore sought to determine current UK expenditure on palliative and end-of-life care research. METHODS: Secondary analysis of publicly available research funding data-UK Health Research Analysis 2022, from the UK Clinical Research Classific

"Kusa Ikivi": Death as an Accomplishment and End-of-Life Experience of Cancer Patients in Rwanda.

BACKGROUND: In present-day Rwanda, despite efforts of the Ministry of Health to implement palliative care, end-of-life care, and death are seldom discussed, and many patients die suffering or alone. OBJECTIVES: We qualitatively studied end-of-life traditions and attitudes toward end-of-life care in present-day Rwanda that might inform optimization of palliative and end-of-life care. METHODS: We conducted qualitative interviews with 29 key informants that were audio recorded, transcribed, translated from Kinyarwanda to English, and analyzed. We also observed patients, family members, and care

Home mechanical ventilation in adults: Clinical practice recommendations from the Portuguese Respiratory Society home mechanical ventilation assembly.

Background: The use of home mechanical ventilation (HMV) has been increasing worldwide, driven by widening of clinical applications and improved patient survival rates. In Portugal, recent data indicate an even faster growth, although national HMV recommendations have remained unchanged for 25 years. Research question: We aimed to provide an update in clinical practice guidelines for HMV in adults, applicable to the Portuguese context, grounded on the latest available evidence and experts' opinion. Study design and methods: A multidisciplinary panel with experience in HMV in the adult populati

Beyond Bars: Evaluating End-of-Life Care and Surrogate Decision-Making for Hospitalized Incarcerated Persons.

Incarcerated persons (IPs) retain the constitutional right to health care, yet they face unique challenges in accessing palliative care (PC) and designating surrogates, especially when incapacitated. We present two cases of hospitalized IPs with life-limiting illnesses who experienced significant barriers in identifying and engaging surrogates. Both cases underscore the effect of delays in communication with surrogates and restricted end-of-life (EOL) visitation due to correctional policies. These delays limited the delivery of optimal interdisciplinary PC and bereavement support. Despite clea

Symptoms matter - symptom diversity and trajectory across different phases of heart failure: from diagnosis to end of life.

PURPOSE OF REVIEW: Heart failure is a complex, progressive and life-limiting condition that affects individuals beyond physical symptoms. Psychosocial and behavioural symptoms, such as anxiety, depression, cognitive impairment, and social withdrawal, substantially impact their quality of life. Despite increasing recognition of multidimensional symptom burden across heart failure trajectory, these non-physical symptoms are often under-recognised by clinicians and researchers. This review summarises current contemporary evidence on symptom experiences from diagnosis of heart failure through to e

[Psychiatric symptoms at the end of life, Part 3: Anxiety].

The prevalence of anxiety in palliative care is 48%, while that of anxiety disorders is around 11%. Since anxiety is one of the physiological, ultimately vital human emotions, it is often difficult to distinguish it from anxiety that requires treatment. This article presents and explains the diagnosis, aetiology and various non-pharmacological and pharmacological treatment options for the different anxiety syndromes that are relevant to general practice.

Utilization of Palliative Radiation in Pediatric Oncology Patients During the End-of-Life (EOL).

BACKGROUND: Suffering at the end-of-life (EOL) can impact the perception of a "good death" and ultimately affect bereavement for families of children with cancer. Palliative radiation (pXRT) is a tool that can address pain, mitigate suffering and improve quality of life. METHODS: A retrospective medical record review of pediatric oncology patients who died over an 11-year period was completed. Descriptive analysis and nonparametric tests to compare groups were used. RESULTS: 2202 total deaths occurred during the study period; 167 patients met study criteria, reflecting a 7.6% incidence of pX

The clinical practice of palliative sedation in patients dying from COVID-19: a retrospective chart review.

BACKGROUND: Clinical experts experienced challenges in the practice of palliative sedation (PS) during the COVID-19 pandemic. Rapid deterioration in patients' situation was observed while the indications for starting PS seemed to differ compared to other terminal patients. It is unclear to which extent clinical trajectories of PS differ for these COVID patients compared to regular clinical practice of PS. OBJECTIVES: To describe the clinical practice of PS in patients with COVID versus non-COVID patients. METHODS: A retrospective analysis of data from a Dutch tertiary medical centre was perf

Patients' preferred place of death: patients are willing to consider their preferences, but someone has to ask them.

BACKGROUND: end-of-life care is not always in line with end-of-life preferences, so patients do not always die at their preferred place of death (PPD). This study aims to identify factors associated with patients' PPD and changes in PPD. METHODS: we prospectively collected data on PPD at four time points within 6 months from 230 acutely hospitalised older patients who were part of the control group in a stepped-wedge randomised controlled trial. Associations between patient characteristics and preferences were calculated using multivariable (multinomial) logistic regression analysis. RESULTS

Symptom-based management of the idiopathic interstitial pneumonia.

The term 'idiopathic interstitial pneumonia' (IIP) encompasses a wide variety of diseases with different and often unexplained pathophysiology as well as diverse natural histories. Unfortunately, many of these diseases are progressive and some are poorly responsive to available therapies. Despite the varied nature of IIPs, patients experience common symptoms related to their chronic lung disease. Dyspnoea, cough, fatigue and depression contribute substantially to morbidity and are often difficult to manage. The psychological stress of having a chronic and often life-limiting disease further co

[Palliative (symptomatic) care of (imminently) dying patients].

The updated Current Care Guideline focuses on medical symptom treatment when curative treatment is no longer possible. Palliative care should be available to all dying patients at all health care levels. Pain should be treated prophylactically. Opioids are effective in cancer pain and should be chosen for moderate or severe pain in line with the WHO pain ladder. Treatment options for symptoms which call for acute interventions, such as intracranial hypertension, and options for dyspnoea, delirium, gastro-intestinal symptoms, ascites, dehydration and end-of-life treatment of elderly and demente
