OBJECTIVES: Percutaneous pharmacomechanical thrombectomy is an emerging therapy for acute deep vein thrombosis and may reduce long-term incidence of post-thrombotic syndrome. This study investigates the proportion of patients presenting with lower limb deep vein thrombosis who are potentially suitable for percutaneous pharmacomechanical thrombectomy.
METHODS: A retrospective review of all duplex ultrasound scans for lower limb deep vein thrombosis over two-year period at a regional vascular unit was conducted. All acute occlusive iliofemoral deep vein thrombosis were screened for percutaneous…

Primary percutaneous coronary intervention, (pPCI), of native coronaries and saphenous vein grafts (SVGs), is the recommended reperfusion strategy for STEMI, and an early invasive approach is recommended for high risk patients with UA/NSTEMI. Although PCI effectively restores flow in the infarct related artery/culprit vessel in both situations, myocardial perfusion often remains suboptimal due to microvascular obstruction, partly attributed to distal embolization of thrombus. Hence, thrombectomy (manual or mechanical), prior to stenting may further reduce hard clinical end points in patients w…

PURPOSE: Endovascular stent fracture and migration is an extremely rare complication of arteriovenous shunt thrombectomy.
TECHNIQUE: We report a case of endovascular stent fracture following repeat arteriovenous graft thrombectomy, which was complicated by migration of a 26 millimeter stent fragment to the left main pulmonary artery. Attempts at percutaneous transvenous retrieval were unsuccessful, and an open thoracotomy to extract the stent fragment was performed.
CONCLUSIONS: Although there is no consensus for the management of endovascular stents that have migrated to the pulmonary arter…

Acute limb ischemia (ALI) remains a life-threatening condition. Studies with catheter-directed thrombolysis and percutaneous mechanical thrombectomy systems show modest improvements in mortality compared to surgery but with pitfalls of major bleeding, distal embolization, recurrent thrombosis, prolonged thrombolytic infusion and increased overall cost. We present a unique and therapeutically challenging case of bilateral acute lower limb ischemia that was managed percutaneously in one setting by a novel technique using site-specific (isolated) pharmaco-mechanical thrombolysis-thrombectomy (IPM…

PURPOSE: To report the use of a new rotational thrombectomy device for percutaneous thrombectomy of an acutely occluded subclavian artery.
CASE REPORT: A 71-year-old woman with a history of multivessel coronary disease complained of sudden onset of pain at rest and paleness of the left arm. Duplex ultrasound showed a localized thrombotic occlusion of the postvertebral subclavian artery and another at the bifurcation of the brachial artery. After angiographic confirmation, the subclavian artery was recanalized with an 8-F Rotarex device via a percutaneous transfemoral access; the bifurcation o…

Recanalization of occluded arteries during acute myocardial infarction has been proven to prolong life and improve left ventricular function. Patients who could not receive thrombolytic therapy for failed thrombolysis and/or angioplasty were treated by mechanical manipulation of the thrombus. Three techniques were used: transcatheter aspiration, clot displacement, and thrombectomy. Five patients in shock had the thrombus aspirated from the left main and right coronary arteries. Eight patients had the clot pushed by the balloon from the mid-left anterior descending (LAD) to the apical LAD in or…

In a retrospective study 55 patients suffering from acute phlebothrombosis were subjected to an examination in order to evaluate the long term results of treatment. Good results of treatment could be achieved for the compensated stages 0-II of the postthrombotic syndrome in 22 patients after thromboectomy and in 33 patients after fibrinolysis (82 or 85% respectively). The portion of bad results of both methods, in the decompensated stages III-IV of the postthrombotic syndrome, was high (18 or 15% respectively). In both methods there was only 1 patient in stage IV of the postthrombotic syndrome

OBJECTIVE: To examine the independent associations of admission eosinophil count with malignant brain edema (MBE) and with poor 3-month functional outcome after mechanical thrombectomy (MT), and to quantify the statistical contribution of MBE to the eosinophil-outcome association using mediation analysis.
METHODS: A total of 442 consecutive AIS patients who underwent MT between May 2017 and January 2023 were analyzed. Multivariable logistic regression analysis was used to assess the association of eosinophils with MBE and poor outcome. Mediation analysis was performed to quantify the statisti…

BACKGROUND: The operational definition and imaging assessment of the no-reflow phenomenon, a potential contributor to poor clinical outcome despite successful thrombectomy, remains debated. We aimed to develop expert-based international consensus on the definition, assessment, and reporting standard of cerebral no-reflow post-thrombectomy.
METHODS: Three-rounds of a modified Delphi web-based survey was conducted among corresponding authors of peer-reviewed clinical studies on cerebral no-reflow between 2020 and 2025. Responses at each round were used to develop structured questionnaires for s…

BACKGROUND: Hypothermia reduces ischemic injury in preclinical stroke models, yet translation into the clinical setting remains challenging. We aimed to assess the feasibility of inducing adjuvant systemic hypothermia among patients with large-vessel occlusion stroke during mechanical thrombectomy.
METHODS: This was a phase II randomized feasibility-controlled trial involving 10 sites in the United States, conducted between May 2019 and May 2024. All eligible patients had confirmed proximal middle cerebral artery or internal carotid artery occlusions, an Alberta Stroke Program Early Computed …

BACKGROUND AND OBJECTIVES: The optimal target blood pressure (BP) during and after mechanical thrombectomy (MT) for acute ischemic stroke remains uncertain. While BP drops during MT are associated with worse outcomes, evidence on postprocedural BP instability is limited. We aimed to assess the impact of BP drops within the first 24 hours after MT on functional outcome.
METHODS: Retrospective observational study analyzing data from 2 tertiary stroke centers, gathered between October 2020 and September 2022 within the German Stroke Registry Endovascular Treatment, a national prospective registr…

BACKGROUND: Acute basilar artery occlusion causes devastating strokes. Despite the benefit of endovascular treatment, the optimal management remains controversial in specific subgroups, particularly patients with mild deficits, and would benefit from robust prognostic tools. Given the dense white matter networks within the posterior fossa, we tested whether quantifying disconnections from acute diffusion-weighted imaging could improve outcome prediction and identify responders to recanalization compared with conventional metrics.
METHODS: We conducted a secondary analysis of a prospective mul

Novel adjuvant hemostatic agents are increasingly used in surgical procedures to achieve hemostasis. We present a case of embolisation of hemostatic agents during a neurosurgical operation, causing massive PE requiring venoarterial extracorporeal membrane oxygenation support, in a 7-year-old boy. Embolectomy was performed using the Penumbra 0.072" catheter with aspiration of blood clots alongside gelatinous clumps of Spongostan and thrombin, leading to angiographic and clinical improvement. A Cook MPA angiographic 5FR catheter and Terumo 0.035" wire were used for catheter repositioning between

Headache is a common complication following posterior circulation stroke. While post-thrombectomy care predominantly focuses on neurological recovery, headache as a salient symptom is often overlooked, leaving a subset of patients affected by persistent chronic pain that severely compromises their quality of life. Here, we conducted a retrospective analysis of patients with good functional outcome (modified Rankin Scale score < 3) after mechanical thrombectomy for posterior circulation stroke, evaluating the incidence of headache within 3 months after surgery. Using univariate and multivariate…

BACKGROUND: Acute pulmonary embolism (PE) is a leading cause of cardiovascular morbidity and mortality, with in-hospital mortality approaching 30% in high-risk presentations. Catheter-directed thrombectomy (CDT) is increasingly used as advanced reperfusion but carries meaningful peri‑procedural risk. Contemporary data on racial disparities following CDT remain limited.
METHODS: Using the Nationwide Inpatient Sample (2016-2022), adults hospitalized with PE undergoing CDT were identified and stratified by race (White, Black, Hispanic, Asian/Pacific Islander[PI], Native American). Inverse probab…

BACKGROUND: A 45-year-old woman with a history of pulmonary embolism, Sjogren syndrome, and hypertension presented with shortness of breath and palpitations.
CASE SUMMARY: Computed tomography angiography of the chest showed nonocclusive, bilateral emboli in the main pulmonary arteries and segmental branches. Mechanical thrombectomy was performed, with transthoracic echocardiography revealing a right atrial thrombus within a patent foramen ovale (PFO). Transesophageal echocardiogram confirmed a 2.7 × 0.6 cm thrombus. Aspiration thrombectomy and patent foramen ovale closure with a Sentinel cere…

BACKGROUND: Patients with severe prestroke disability (PSD) remain underrepresented in mechanical thrombectomy studies, despite their growing relevance in aging populations. This study used data from the German Stroke Registry-Endovascular Treatment to evaluate functional recovery, mortality, and poststroke care outcomes in this high-risk population.
METHODS: We analyzed 9456 mechanical thrombectomy-treated patients with stroke from the German Stroke Registry-Endovascular Treatment (2015-2021), categorized by premorbid modified Rankin Scale (mRS): no PSD (mRS score, 0-1), moderate PSD (mPSD; …

BACKGROUND: Intensive blood pressure (BP) management after successful endovascular thrombectomy (EVT) worsens outcomes in acute ischemic stroke. White matter hyperintensity (WMH), associated with impaired cerebral autoregulation, is associated with poor outcomes after EVT; however, its role in modifying BP management effects remains unclear. We investigated whether WMH severity modifies the effect of post-EVT BP management on outcomes.
METHODS: This secondary analysis of the OPTIMAL-BP trial (Outcome in Patients Treated With Intra-Arterial Thrombectomy-Optimal Blood Pressure Control; conducte…
